If you’ve struggled with fertility or other hormonal issues and have been frustratingly told to “just relax,” you’re not alone. And while that advice can come off as tone-deaf and patronizing when it isn’t backed up with concrete, feasible ways to do it, there’s actually some real science behind relaxing worth understanding. After all, le stress chronique doesn’t just feel bad; it biochemically disrupts the very hormonal processes that govern your cycle, your ovulation, and your ability to conceive. The good news is that simple, evidence-backed breathing techniques can meaningfully counteract those effects—and there’s no remedy cheaper and more accessible than breathing exercises. 

Your nervous system is running the show

To understand why stress management matters for your cycle—and how certain breathing exercises can help—you first need to understand the stress-hormone connection. When you encounter a stressor; whether it’s a work deadline, relationship tension, or even the existential weight of trying to conceive month after month, your body activates the l'axe hypothalamo-hypophyso-surrénalien (HHS). Consequently, your system is flooded with cortisol. This is your fight-or-flight response at work. (Of note, while they have many similarities, stress is different from anxiety in that it is a response to an external circumstance rather than a “ state of ongoing fear, worry, or unease”). 

The problem? Your reproductive system operates on the hypothalamic-pituitary-ovarian (HPO) axis, and it doesn’t coexist peacefully with chronic cortisol elevation. Recherche shows that sustained psychological stress disrupts normal secretion of hypothalamic hormones, alters pituitary signaling, and directly impairs ovarian function, leading to irregular cycles and ovulatory disorders [1]. Put plainly: when your body thinks you’re in danger, it deprioritizes reproduction  (and we know that a disrupted HPO axis goes hand-in-hand with irregular periods).

A Revue systématique de 2023 on cortisol and infertility confirmed that chronically elevated cortisol is consistently linked with disrupted reproductive endocrinology [2]. Cortisol can suppress the LH surge (that is the hormonal signal that triggers ovulation) meaning that some chronically-stressed women are missing ovulation entirely, perhaps without realizing it [3]. Other recherche found that women undergoing IVF with elevated cortisol had significantly worse treatment outcomes [4].

The breathing–brain connection

Here’s where breathwork becomes genuinely exciting. Your autonomic nervous system has two modes: sympathetic (fight-or-flight) and parasympathetic (rest-and-digest). Most of us spend far too much time in sympathetic dominance, and the consequences are felt system-wide—including in our reproductive hormones.

Controlled breathing is one of the few ways you can directly and voluntarily shift your nervous system from sympathetic to parasympathetic activation. Essentially, through controlled breathing you can flip the switch from stress to calm. This works through the vagus nerve, a long and winding nerve that runs from your brainstem through your chest and abdomen. The slow and deliberate breathing—particularly with extended exhales—stimulates the vagus nerve, increasing what researchers call heart rate variability (HRV), a marker of how flexibly your heart responds to moment-to-moment demands [5]. High HRV is associated with good health, resilience to stress, and better hormonal balance.

A 2024 randomized controlled trial found that vagus nerve stimulation combined with controlled breathing significantly improved HRV and autonomic regulation in individuals with chronic stress [5]. Separate research on slow breathing in hypertensive patients found measurable improvements in baroreflex sensitivity. That’s the body’s ability to regulate blood pressure and it’s another marker of a healthy, non-overloaded nervous system [6].

The breathing techniques that actually work

Not all breathing exercises are created equal. Here’s what the science supports.

Cyclic sighing (or “physiological sigh”) 

This is currently the most research-validated breathwork technique for stress relief. It involves a double inhale through the nose. It is a full breath, followed immediately by a short “top-off” inhale; then a long, slow exhale through the mouth.

In a landmark 2023 randomized controlled study from Stanford University, participants practiced one of several breathwork styles for just five minutes a day for four weeks. Cyclic sighing came out on top: it produced greater improvement in mood and greater reductions in respiratory rate than mindfulness meditation, and significantly outperformed the other breathing techniques tested [7]. The extended exhale is key, as it’s what most powerfully activates the parasympathetic nervous system.

Diaphragmatic (belly) breathing, at 6 breaths per minute 

Slow diaphragmatic breathing at approximately six breaths per minute is the gold standard for HRV improvement. One PMC-published étude found that including rest periods after the exhale during diaphragmatic breathing significantly increased high-frequency HRV. That is a direct marker of parasympathetic activity [8]. This pace is slower than it sounds (five seconds in, five seconds out), and it takes some practice, but the physiological payoff is real.

Box breathing (or “4-4-4-4”) 

Popularized by Navy SEALs, box breathing involves inhaling for four counts, holding for four, exhaling for four, and holding again for four. While slightly less studied than cyclic sighing, it has a strong evidence base for reducing acute anxiety and resetting the nervous system, particularly under high-pressure conditions.

4-7-8 breathing 

Inhale for four counts, hold for seven, exhale slowly for eight. The extended hold and long exhale make this technique particularly effective for activating the parasympathetic response before bed or during periods of acute stress.

How much breathwork is enough to see results?

According to the 2023 Stanford study, five minutes per day of consistent breathwork produced measurable, lasting improvements in mood and physiological arousal [7]. That’s a big payoff for a time commitment that’s probably less than the length of your morning coffee ritual. For best results, pair your breathing practice with other nervous system supports: adequate sleep, regular movement, and (when possible) reduction of the chronic background stressors that keep your cortisol elevated in the first place. Breathwork isn’t a cure-all, but it is a powerful, physiologically grounded tool that supports the hormonal environment your cycle depends on.

Chronic stress is a physiological state with real downstream effects on your hormones and your cycle. Breathing techniques work; not through wishful thinking, but through well-documented mechanisms involving the vagus nerve, the autonomic nervous system, and HPA-HPO interaction. Five minutes of intentional breathing each day is one of the most accessible things you can do for your cycle health and your overall wellbeing. You don’t need to be an expert or part of a large study to see whether five minutes of intentional breathing could have a positive impact on you.
