How can that be? Let\u2019s explore why that distinction isn\u2019t splitting hairs, what causes BV in the first place, its connection with some forms of contraceptive use, and why treatment is only temporary.\u00a0<\/p>\n\n\n\n<h2 id=\"h-what-bacterial-vaginosis-bv-is\" class=\"wp-block-heading\"><span id=\"what-bacterial-vaginosis-bv-is\">What bacterial vaginosis (BV) is<\/span><\/h2>\n\n\n\n<p class=\"wp-block-paragraph\">Bacterial vaginosis is a <a href=\"https:\/\/www.researchgate.net\/publication\/350696545_The_Female_Vaginal_Microbiome_in_Health_and_Bacterial_Vaginosis\" target=\"_blank\" rel=\"noreferrer noopener\">disruption<\/a> and imbalance in the vaginal microbiome (we\u2019ve talked about the gut microbiome before, <a href=\"https:\/\/naturalwomanhood.org\/gut-microbiome\/\" target=\"_blank\" rel=\"noreferrer noopener\">here<\/a>, including how Dad\u2019s <a href=\"https:\/\/naturalwomanhood.org\/dad-gut-microbiome\/\" target=\"_blank\" rel=\"noreferrer noopener\">gut microbiome<\/a> affects his baby in utero, and about how Mom\u2019s vaginal microbiome <a href=\"https:\/\/naturalwomanhood.org\/vaginal-seeding-after-c-section\/\" target=\"_blank\" rel=\"noreferrer noopener\">seeds<\/a> baby\u2019s gut microbiome) [1]. The vaginal microbiome <a href=\"https:\/\/www.researchgate.net\/publication\/350696545_The_Female_Vaginal_Microbiome_in_Health_and_Bacterial_Vaginosis\" target=\"_blank\" rel=\"noreferrer noopener\">contains<\/a> billions, yes, <em>billions<\/em>, of microbes (microscopic organisms) [1]. The most common types of microbes include bacteria, viruses, and fungi.&nbsp;<\/p>\n\n\n\n<p class=\"wp-block-paragraph\">We know that some microbes are healthy for the body (in other words, not all bacteria is bad!), and that<em> balance<\/em> is important: certain bacteria should exist in certain amounts in relation to other types of bacteria. <em>Lactobacillus<\/em> is a \u201cgood\u201d type of bacteria, and while the bacterial composition of the microbiome changes over the course of the menstrual cycle as well as during a woman\u2019s reproductive lifespan overall, in a healthy vagina, <em>lactobacillus<\/em> should be the dominant bacteria throughout. <em>Lactobacilli<\/em> help protect the vagina from infection.&nbsp;<\/p>\n\n\n\n<p class=\"wp-block-paragraph\">But when a woman has BV, she has an overgrowth of a different type of bacteria\u2014anaerobic bacteria, especially <em>Gardenerella vaginalis<\/em>. Fascinatingly, in BV, anaerobic bacteria increase by <em>100-1000x<\/em> the normal amount. These excessively high bacterial counts, specifically of anaerobic bacteria, lead to the development of biofilms (think of a shield or matrix) on the skin cells of the vagina. These biofilms are notoriously resistant to antibiotics. This makes a woman more vulnerable to developing infections, and the biofilms make BV hard to permanently treat.<\/p>\n\n\n\n<figure class=\"wp-block-pullquote\"><blockquote><p>In BV, anaerobic bacteria increase by <em>100-1000x<\/em> the normal amount. These excessively high bacterial counts lead to the development of biofilms on the skin cells of the vagina. These biofilms are notoriously resistant to antibiotics. This makes a woman more vulnerable to developing infections, and the biofilms make BV hard to permanently treat.<\/p><\/blockquote><\/figure>\n\n\n\n<h2 id=\"h-who-gets-bv-and-why\" class=\"wp-block-heading\"><span id=\"who-gets-bv-and-why\">Who gets BV, and why?<\/span><\/h2>\n\n\n\n<p class=\"wp-block-paragraph\">The short <a href=\"https:\/\/academic.oup.com\/cid\/article\/74\/Supplement_2\/S144\/6567952\" target=\"_blank\" rel=\"noreferrer noopener\">answer<\/a> is: many women. An estimated 23-29% of reproductive-aged women globally have BV [2]. For reference, an estimated 5-10% of women worldwide experience <a href=\"https:\/\/www.sciencedirect.com\/science\/article\/pii\/S2590161324000838\" target=\"_blank\" rel=\"noreferrer noopener\">endometriosis<\/a> [3]. In North America, African American and Hispanic women are more likely than white and Asian women to have BV.&nbsp;<\/p>\n\n\n\n<p class=\"wp-block-paragraph\">Disconcertingly, 60 years of research haven\u2019t yielded clear answers on what causes the explosion of \u201cbad\u201d bacteria and\/or the decrease of \u201cgood\u201d bacteria that characterizes BV. We do know that IUD use, especially copper IUD use, appears to raise <a href=\"https:\/\/naturalwomanhood.org\/the-troubling-link-between-the-copper-iud-and-bacterial-vaginosis\/\" target=\"_blank\" rel=\"noreferrer noopener\">risk<\/a> [2]. Male promiscuity is also associated with increased risk of BV for female partners [2]. Women who have sex with other women are also at high risk, probably because of the exchange of vaginal fluids.&nbsp;<\/p>\n\n\n\n<h2 id=\"h-having-bv-increases-your-risk-of-certain-reproductive-conditions\" class=\"wp-block-heading\"><span id=\"having-bv-increases-your-risk-of-certain-reproductive-conditions\">Having BV increases your risk of certain reproductive conditions<\/span><\/h2>\n\n\n\n<p class=\"wp-block-paragraph\">While BV is not a sexually transmitted infection or disease, it does increase your risk of contracting sexually transmitted infections, such as chlamydia and gonorrhea, as well as <a href=\"https:\/\/naturalwomanhood.org\/what-you-should-know-about-hpv\/\" target=\"_blank\" rel=\"noreferrer noopener\">human papilloma virus<\/a> (HPV) and <a href=\"https:\/\/naturalwomanhood.org\/hiv\/\" target=\"_blank\" rel=\"noreferrer noopener\">human immunodeficiency virus<\/a> (HIV) [1].&nbsp;<\/p>\n\n\n\n<p class=\"wp-block-paragraph\">BV also <a href=\"https:\/\/www.sciencedirect.com\/science\/article\/pii\/S0165037825003821\" target=\"_blank\" rel=\"noreferrer noopener\">makes women more susceptible<\/a> to reproductive system infections, especially pelvic inflammatory disease (PID), cervical infections, and endometrial infections, plus <a href=\"https:\/\/naturalwomanhood.org\/why-do-i-keep-getting-utis-a-review-of-causes-and-treatments\/\">urinary tract infections (UTIs)<\/a> [4].\u00a0<\/p>\n\n\n\n<p class=\"wp-block-paragraph\">BV is linked with miscarriage and preterm birth [1]. Being born early is associated with having a low birth weight, which in turn comes with short-term and long-term risks for the baby [4].&nbsp;<\/p>\n\n\n\n<h2 id=\"h-bv-symptoms\" class=\"wp-block-heading\"><span id=\"bv-symptoms\">BV symptoms<\/span><\/h2>\n\n\n\n<p class=\"wp-block-paragraph\">A 2022 review article in <em>Clinical Infectious Diseases<\/em> called BV \u201cthe most common cause of vaginal discharge\u201d [2]. Unlike the egg-white cervical mucus that signals health and fertility, the discharge associated with BV is often green, gray or milky-looking and\/or has a distinct fishy or metallic odor. BV commonly causes itching, which may be worse, along with the smell, after sex or during menstruation. This is because sperm and menstruation both temporarily change the vaginal pH. Still, BV is <em>not<\/em> considered a sexually transmitted infection, because many women who are not sexually active (including those who have never had sex) have BV.&nbsp;<\/p>\n\n\n\n<p class=\"wp-block-paragraph\">BV is diagnosed based on multiple criteria, including results from a vaginal swab that can be performed during a routine visit to your primary care practitioner or OB\/GYN\u2019s office. Interestingly, more than half of women with BV have no symptoms [4].&nbsp;<\/p>\n\n\n\n<h2 id=\"h-bv-is-tough-to-treat-and-comes-back-quickly\" class=\"wp-block-heading\"><span id=\"bv-is-tough-to-treat-and-comes-back-quickly\">BV is tough to treat and comes back quickly<\/span><\/h2>\n\n\n\n<p class=\"wp-block-paragraph\">Remember the biofilm mentioned above? That biofilm shields or protects the out-of-control anaerobic bacteria from \u201ccrowd control\u201d by antibiotics. Worse, antibiotics can\u2019t restore healthy <em>Lactobacillus<\/em>. For these reasons, BV is notoriously difficult to treat and comes back quickly, often by just six months later.&nbsp;<\/p>\n\n\n\n<p class=\"wp-block-paragraph\">Even though BV is tricky to treat, women with symptoms still receive round after round of antibiotics anyway in an effort to avoid the other infections listed above or, for pregnant women, the possibility of preterm birth. Treatment for pregnant women is especially precarious because antibiotics impact the woman\u2019s gut microbiome, and her gut microbiome <a href=\"https:\/\/www.sciencedirect.com\/science\/article\/abs\/pii\/S0149763423001641\" target=\"_blank\" rel=\"noreferrer noopener\">indirectly affects<\/a> her baby\u2019s brain development [4] [5]. Some research suggests that vaginal antibiotics are safer than oral antibiotics for the baby [4]. Antibiotic resistance is a possibility with repeated antibiotic rounds over time, in both pregnant and nonpregnant women.&nbsp;&nbsp;&nbsp;<\/p>\n\n\n\n<h2 id=\"h-can-probiotics-help\" class=\"wp-block-heading\"><span id=\"can-probiotics-help\">Can probiotics help?<\/span><\/h2>\n\n\n\n<p class=\"wp-block-paragraph\">Above, you read that BV keeps coming back in part because antibiotics can\u2019t restore normal levels of <em>lactobacillus<\/em>. You might know that <em>lactobacillus<\/em> is a commonly sold probiotic, and you may wonder why women can\u2019t just take a probiotic with (or instead of) conventional antibiotics?&nbsp;<\/p>\n\n\n\n<p class=\"wp-block-paragraph\">Unfortunately, research on oral probiotics hasn\u2019t yet shown clear benefit (for interested readers, details are <a href=\"https:\/\/www.sciencedirect.com\/science\/article\/pii\/S0165037825003821#bib62\" target=\"_blank\" rel=\"noreferrer noopener\">here<\/a> and <a href=\"https:\/\/www.tandfonline.com\/doi\/full\/10.2147\/IJWH.S259311\" target=\"_blank\" rel=\"noreferrer noopener\">here<\/a>) [4] [6]. As with oral probiotics, vaginal probiotics are difficult to compare because the strains of probiotics used often vary from one study to the next. Still, probiotics may represent the <a href=\"https:\/\/link.springer.com\/article\/10.1186\/s12978-022-01449-z\" target=\"_blank\" rel=\"noreferrer noopener\">most benefit for BV treatment<\/a>, once dosages and strains can be figured out [7]. One particular strain,<em> lactobacillus rhamnosus BMX 54,<\/em> has some promising research backing it, especially when combined with a simple sugar prebiotic [7] [4].\u00a0<\/p>\n\n\n\n<figure class=\"wp-block-pullquote\"><blockquote><p>Unfortunately, research on oral probiotics hasn\u2019t yet shown clear benefit. As with oral probiotics, vaginal probiotics are difficult to compare because the strains of probiotics used often vary from one study to the next. Still, probiotics may represent the most benefit for BV treatment, once dosages and strains can be figured out. <\/p><\/blockquote><\/figure>\n\n\n\n<p class=\"wp-block-paragraph\">Other potential BV treatments of the future include vaginal microbiome transplantation (transferring good bacteria from one healthy volunteer to an infected woman), medications that directly attack the biofilm, and certain viruses (bacteriophages) that reduce bacterial overgrowth while preserving healthy bacteria [4]. Some practitioners may also recommend vaginal <a href=\"https:\/\/www.healthline.com\/health\/womens-health\/can-boric-acid-help-treat-bacterial-vaginosis#does-it-work\" target=\"_blank\" rel=\"noreferrer noopener\">boric acid<\/a> as an adjuvant therapy alongside antibiotics, but it is toxic when taken orally, and should not be taken while pregnant.&nbsp;&nbsp;&nbsp;<\/p>\n\n\n\n<h2 id=\"h-the-bottom-line\" class=\"wp-block-heading\"><span id=\"the-bottom-line\">The bottom line<\/span><\/h2>\n\n\n\n<p class=\"wp-block-paragraph\">While many women don\u2019t know they have bacterial vaginosis because they don\u2019t have symptoms, it nevertheless represents a health concern because it predisposes women to other infections and pregnancy complications. Currently, treatment options are very limited and recurrence is common. The best advice we have for evidence-based options is: stay tuned.
