But the psychological adjustment took longer, as did baby bonding and the identity development. My usually put-together, do-it-all persona struggled to navigate common early motherhood realities: new mom worries, forgetfulness and fogginess, hair-trigger temper outbursts, mom rage, mood swings, guilt and shame, challenges with breastfeeding, and ambivalence. All of it rattled me\u201d (Introduction, x).&nbsp;<\/p>\n\n\n\n<p class=\"wp-block-paragraph\">So writes clinical psychologist Dr. Nicole Amoyal Pensak, PhD, in<em> Rattled: How to Calm New Mom Anxiety with the Power of the Postpartum Brain.<\/em> <a href=\"https:\/\/podcasts.apple.com\/us\/podcast\/postpartum-on-purpose\/id1749107654\" target=\"_blank\" rel=\"noreferrer noopener\">I host a postpartum podcast<\/a> and write regularly about the postpartum period, so when I found this book through my library\u2019s website\u2019s \u2018You might also like\u2019 feature, I was all ears, or, rather, eyes. I wondered what the average mom in the trenches could hope to gain from this book.&nbsp;&nbsp;<\/p>\n\n\n\n<p class=\"wp-block-paragraph\">Would it be an academic treatise summing up the scientific literature on mom rage and other struggles? Would <em>Rattled<\/em> be a book-length regurgitation of all the tired \u201cmotherhood is the worst, try to survive it if you can\u201d tropes we\u2019re daily confronted with on social media? Or could it actually help new moms (whether they\u2019ve just welcomed their first child or tenth) understand how to harness the features (not bugs!) of the maternal brain, leading to awe and respect for our beautiful design rather than frustration and fear?&nbsp;<\/p>\n\n\n\n<p class=\"wp-block-paragraph\">Read on for my recommendation on whether to buy, borrow, or skip<em> Rattled<\/em>.&nbsp;<\/p>\n\n\n\n<h2 id=\"h-what-is-the-author-s-background-or-credibility-to-teach-on-the-topic\" class=\"wp-block-heading\"><span id=\"what-is-the-authors-background-or-credibility-to-teach-on-the-topic\">What is the author\u2019s background or credibility to teach on the topic?<\/span><\/h2>\n\n\n\n<p class=\"wp-block-paragraph\"><a href=\"https:\/\/www.youtube.com\/watch?v=GXM2BIBxECc&amp;t=1s\" target=\"_blank\" rel=\"noreferrer noopener\">Pensak<\/a> is a mom of two and a clinical psychologist certified in perinatal mental health who did her doctoral training at University of Rhode Island, a predoctoral internship at Yale, and postdoctoral fellowships at Harvard and University of Colorado. She also serves on<em> Parents Magazine\u2019s<\/em> Expert Review Board.&nbsp;<\/p>\n\n\n\n<h2 id=\"h-who-is-the-intended-audience\" class=\"wp-block-heading\"><span id=\"who-is-the-intended-audience\">\u041a\u0442\u043e \u044f\u0432\u043b\u044f\u0435\u0442\u0441\u044f \u0446\u0435\u043b\u0435\u0432\u043e\u0439 \u0430\u0443\u0434\u0438\u0442\u043e\u0440\u0438\u0435\u0439?<\/span><\/h2>\n\n\n\n<p class=\"wp-block-paragraph\">Pensak talks straight to new moms in her book, writing \u201cThis book focuses on the primary parent or caregiver, with the premise that the majority of motherhood is really managing your own emotions and well-being. Key to this is the science of neuroplasticity and cognitive behavioral therapy. Untangling and making new pathways in your brain and increasing psychological flexibility is like getting that knot out of our fine-strand necklace\u201d (Introduction, xviii).&nbsp;<\/p>\n\n\n\n<figure class=\"wp-block-pullquote\"><blockquote><p>\u201cThis book focuses on the primary parent or caregiver, with the premise that the majority of motherhood is really managing your own emotions and well-being. Key to this is the science of neuroplasticity and cognitive behavioral therapy. Untangling and making new pathways in your brain and increasing psychological flexibility is like getting that knot out of our fine-strand necklace.\u201d<\/p><cite>Introduction, <em>Rattled<\/em><\/cite><\/blockquote><\/figure>\n\n\n\n<h2 id=\"h-what-are-the-main-content-areas-of-rattled\" class=\"wp-block-heading\"><span id=\"what-are-the-main-content-areas-of-rattled\">\u041a\u0430\u043a\u043e\u0432\u044b \u043e\u0441\u043d\u043e\u0432\u043d\u044b\u0435 \u043e\u0431\u043b\u0430\u0441\u0442\u0438 \u0441\u043e\u0434\u0435\u0440\u0436\u0430\u043d\u0438\u044f <em>Rattled<\/em>?<\/span><\/h2>\n\n\n\n<p class=\"wp-block-paragraph\">Pesnak starts off by distinguishing <em>Rattled\u2019s<\/em> approach from that of parenting books. While parenting books emphasize the care and keeping of babies, Pensak\u2019s work centers on the mother, because \u201cThe secret is that a significant portion of taking care of babies involves regulating your own emotions first.\u201d Pensak believes that \u201ceducating and encouraging mothers to use their newly developed and very real changed parent brains to their advantage will be life-changing\u201d (Intro, xviii).&nbsp;<\/p>\n\n\n\n<p class=\"wp-block-paragraph\">Pensak summed up <em>Rattled\u2019s<\/em> goal in this way: \u201cMothers need to know what they are working with under the hood: maternal brain changes, normal mom worries that are adaptive, worries that are not helpful, and how to harness it all, backed by science and therapy\u2026 By understanding both the physiological and the psychological, we can make clear-eyed decisions to nurture the process of matrescence. We can learn to accept what is typical: ambivalence, mom worries, interruptions, forgetfulness, unexpected thoughts, postpartum pressures, and new emotions. We can harness both the struggles and the gifts that motherhood provides\u201d (Introduction, xi).&nbsp;<\/p>\n\n\n\n<h3 id=\"h-part-i-how-to-really-prepare-for-motherhood\" class=\"wp-block-heading\"><span id=\"part-i-how-to-really-prepare-for-motherhood\">Part I: How to Really Prepare for Motherhood<\/span><\/h3>\n\n\n\n<p class=\"wp-block-paragraph\">Part I\u2019s five chapters are entitled \u2018The Ideal Mother,\u2019 \u2018The Real Mommy Brain,\u2019 \u2018Prepare Your Postpartum Mental Health Care Plan,\u2019 \u2018Matrescence and the Transition of Motherhood,\u2019 and \u2018Prep Your Marriage.\u2019&nbsp;<\/p>\n\n\n\n<p class=\"wp-block-paragraph\">Part I has two foci. The first is providing a foundational explanation of <em>matrescence<\/em>, pronounced similarly to \u2018adolescence,\u2019 and representing a similarly intense period of brain, hormonal, and identity changes. Whereas adolescence lasts more than a decade from initial signs of puberty through full development of the brain at age 26, a mother hurtles through matrescence at (relative) lightning speed. Pensak shares the physiological and psychological changes that occur during matrescence.&nbsp;<\/p>\n\n\n\n<p class=\"wp-block-paragraph\">Also in Part I, Pensak lays out the pregnant and postpartum brain\u2019s capacity for neuroplasticity and why she believes therapy during this time period can be particularly helpful for long-term healing and growth. As an example, in \u2018The Ideal Mother,\u2019 Pensak covers the research on the fogginess and forgetfulness we colloquially refer to as \u201cmom brain,\u201d noting that \u201cMommy brain is a very real phenomenon\u2026 In simple terms, your brain is working against you, compelling you to ignore your own needs and instead attune to your baby\u2019s\u201d (5). While research corroborates new moms\u2019 reports of forgetting things, when it comes to baby-related tasks, \u201cmemory functioning was improved and actually represented a \u2018boost in learning.\u2019\u201d (6)&nbsp;<\/p>\n\n\n\n<p class=\"wp-block-paragraph\">I\u2019d sum up Part I with \u201cwhy all your postpartum brain changes aren\u2019t a bad thing.\u201d&nbsp;<\/p>\n\n\n\n<h3 id=\"h-part-ii-how-to-adjust-to-new-mom-experiences-and-emotions\" class=\"wp-block-heading\"><span id=\"part-ii-how-to-adjust-to-new-mom-experiences-and-emotions\">Part II: How to Adjust to New Mom Experiences and Emotions<\/span><\/h3>\n\n\n\n<p class=\"wp-block-paragraph\">Part II is also five chapters, comprising \u2018Baby Bonding and the First Letdown,\u2019 \u2018The Double-Down Method to Manage Mom Guilt,\u2019 \u2018Soften on Mom Shame,\u2019 \u2018Embrace Your New Mom Rage,\u2019 and \u201cHow to Manage a Maternal Mental Health Crisis.\u2019&nbsp;<\/p>\n\n\n\n<p class=\"wp-block-paragraph\">These are the nuts-and-bolts, practical, \u201chands-on\u201d chapters, ones I\u2019d flip to throughout postpartum as a reference work of sorts. While some of Pensak\u2019s advice is repetitive from one chapter to the next, that consistency reflects the blessed crossover between many challenging aspects of motherhood: learn to cope well with one, you learn to cope well with others.&nbsp;<\/p>\n\n\n\n<h3 id=\"h-part-iii-how-to-go-from-surviving-to-thriving-in-early-motherhood\" class=\"wp-block-heading\"><span id=\"part-iii-how-to-go-from-surviving-to-thriving-in-early-motherhood\">Part III: How to Go from Surviving to Thriving in Early Motherhood<\/span><\/h3>\n\n\n\n<p class=\"wp-block-paragraph\">Part III is shorter\u2014just three chapters\u2014and includes a \u2018How to Self-Direct Maternal Neuroplasticity,\u2019 \u2018How to Handle Transitions: Back to Work,\u2019 and \u2018Up the Ante.\u2019 I\u2019d sum up this section as \u201cputting it all together.\u201d&nbsp;<\/p>\n\n\n\n<h2 id=\"h-what-are-the-strengths-of-rattled\" class=\"wp-block-heading\"><span id=\"what-are-the-strengths-of-rattled\">\u041a\u0430\u043a\u043e\u0432\u044b \u0441\u0438\u043b\u044c\u043d\u044b\u0435 \u0441\u0442\u043e\u0440\u043e\u043d\u044b <em>Rattled<\/em>?<\/span><\/h2>\n\n\n\n<p class=\"wp-block-paragraph\">Where to begin? Truly, I admired Pensak\u2019s masterful approach time and time again to common struggles such as mom brain, mom guilt (believing I did something bad), mom shame (believing I am bad), mom rage, the tendency to have a scary \u201cwhat if?\u201d thought right after having a pleasurable thought about enjoying the new baby, and more.&nbsp;<\/p>\n\n\n\n<p class=\"wp-block-paragraph\">Much of what she pointed out helped make sense of the physiological and psychological changes the brain undergoes during pregnancy and postpartum. Case in point, she clarified that \u201cWhen your brain is rewiring to increase anxiety to worry about your baby\u2019s needs, your body does not know the difference between \u2018new baby anxiety\u2019 and general anxiety. Your body just feels anxious\u201d (26).&nbsp;&nbsp;<\/p>\n\n\n\n<p class=\"wp-block-paragraph\">A smattering of content that resonated strongly includes: her \u201cdon\u2019t make it worse\u201d counsel when experiencing strong anger or feelings of rage, her encouragement to new moms to take the responsibility of self-diagnosing perinatal mood and anxiety disorders off their own plates and let healthcare professionals do their jobs, and her multiple mentions of considering dads\u2019 mental health too, since up to 10% of new <a href=\"https:\/\/naturalwomanhood.org\/ru\/%d0%bf%d0%be%d1%81%d0%bb%d0%b5%d1%80%d0%be%d0%b4%d0%be%d0%b2%d0%b0%d1%8f-%d0%b4%d0%b5%d0%bf%d1%80%d0%b5%d1%81%d1%81%d0%b8%d1%8f-%d1%83-%d0%bc%d1%83%d0%b6%d1%87%d0%b8%d0%bd\/\">dads may suffer postpartum depression<\/a> \u0442\u0430\u043a\u0436\u0435.&nbsp;<\/p>\n\n\n\n<p class=\"wp-block-paragraph\">I loved that Pensak views postpartum as a window of <em>\u0432\u043e\u0437\u043c\u043e\u0436\u043d\u043e\u0441\u0442\u044c<\/em>, from which mothers can emerge \u201cbetter than ever\u201d (8). As just one example, the \u201cincredible and numerous challenges\u201d of pregnancy and early motherhood have \u201cthe potential to increase cognitive reserve in the long term,\u201d when we understand cognitive reserve as \u201cthe brain\u2019s capacity to handle impairment due to illness, loss, and decline over time\u201d (8).&nbsp;<\/p>\n\n\n\n<figure class=\"wp-block-pullquote\"><blockquote><p>I loved that Pensak views postpartum as a window of <em>\u0432\u043e\u0437\u043c\u043e\u0436\u043d\u043e\u0441\u0442\u044c<\/em>, from which mothers can emerge \u201cbetter than ever.&#8221; As just one example, the \u201cincredible and numerous challenges\u201d of pregnancy and early motherhood have \u201cthe potential to increase cognitive reserve in the long term,\u201d when we understand cognitive reserve as \u201cthe brain\u2019s capacity to handle impairment due to illness, loss, and decline over time.&#8221;<\/p><\/blockquote><\/figure>\n\n\n\n<p class=\"wp-block-paragraph\">Pensak also explores the concept of post-traumatic growth, which is particularly good news for the high numbers of women who consider their <a href=\"https:\/\/naturalwomanhood.org\/ru\/%d0%bd%d0%b0%d0%b9%d1%82%d0%b8-%d0%b8%d1%81%d1%86%d0%b5%d0%bb%d0%b5%d0%bd%d0%b8%d0%b5-%d0%be%d1%82-%d1%82%d1%80%d0%b0%d0%b2%d0%bc%d0%b0%d1%82%d0%b8%d1%87%d0%b5%d1%81%d0%ba%d0%be%d0%b3%d0%be-%d0%be\/\" target=\"_blank\" rel=\"noreferrer noopener\">labor and birth experience traumatic<\/a>. Pensak helpfully spells out the conditions necessary for post-traumatic growth to occur (12).&nbsp;<\/p>\n\n\n\n<p class=\"wp-block-paragraph\">Best of all, Pensak doesn\u2019t limit herself to explaining the \u201cwhy\u201d behind common experiences of new motherhood. She climbs right into the fray with strategies, tactics, and tools, including proactively crafting a postpartum mental health care plan-template and all.&nbsp;<\/p>\n\n\n\n<h2 id=\"h-limitations-or-blindspots-of-rattled\" class=\"wp-block-heading\"><span id=\"limitations-or-blindspots-of-rattled\">Limitations or blindspots of <em>Rattled<\/em><\/span><\/h2>\n\n\n\n<h3 id=\"h-overreliance-on-self-care-to-handle-postpartum-challenges\" class=\"wp-block-heading\"><span id=\"overreliance-on-self-care-to-handle-postpartum-challenges\">Overreliance on self-care to handle postpartum challenges<\/span><\/h3>\n\n\n\n<p class=\"wp-block-paragraph\">Pensak heavily emphasizes the value of self-care and of filling one\u2019s own cup first, because, as she repeats throughout, \u201cyou can\u2019t fill from an empty vessel.\u201d To her credit, she doesn\u2019t encourage adopting a victim mentality. She instead acknowledges and advocates for increasing one\u2019s own capacity during postpartum, capitalizing on the power of neuroplasticity.&nbsp;<\/p>\n\n\n\n<p class=\"wp-block-paragraph\">But her lens was purely secular, with nary a mention of the role religious faith can play in helping women integrate the greater meaning and purpose behind the challenges and sufferings of new motherhood. (Christian readers may find a helpful companion book in mother of 10 Abbie Halberstadt\u2019s <em>M is for Mama: A Rebellion Against Mediocre Motherhood<\/em>, which I read alongside <em>Rattled. <\/em>Likewise, <em>\u0421\u0432\u044f\u0449\u0435\u043d\u043d\u043e\u0435 \u043d\u0430\u0447\u0430\u043b\u043e: Nurturing Your Body, Mind, &amp; Soul During Baby's First Forty Days<\/em>-<a href=\"https:\/\/naturalwomanhood.org\/ru\/%d0%be%d0%b1%d0%b7%d0%be%d1%80-%d0%ba%d0%bd%d0%b8%d0%b3-%d0%bf%d0%be-%d0%bf%d0%be%d1%81%d0%bb%d0%b5%d1%80%d0%be%d0%b4%d0%be%d0%b2%d0%be%d0%bc%d1%83-%d1%83%d1%85%d0%be%d0%b4%d1%83\/\">the subject of a previous NW Review<\/a>\u2014offers an Orthodox take<em>.<\/em>).&nbsp;<\/p>\n\n\n\n<p class=\"wp-block-paragraph\">As such, the one-note refrain of recognizing and tending to one\u2019s own needs, rehashed in myriad ways, became a limitation and, arguably, wouldn\u2019t translate well for a mother of three, four, or more children. While love multiplies, time to attend to one\u2019s own needs doesn\u2019t tend to do the same, which is where enhanced meaning and purpose from religious faith come into play for many moms. Pensak would say, and I agree, that we have to attend to the mother first. As a result, we \u201cwin\u201d both the baby\u2019s health and wellbeing and her own. But escalating levels of self-care can\u2019t be the only path forward.&nbsp;<\/p>\n\n\n\n<h3 id=\"h-lacks-guidance-on-distinguishing-mom-guilt-from-changed-priorities-new-desires\" class=\"wp-block-heading\"><span id=\"lacks-guidance-on-distinguishing-mom-guilt-from-changed-priorities-new-desires\">Lacks guidance on distinguishing mom guilt from changed priorities, new desires<\/span><\/h3>\n\n\n\n<p class=\"wp-block-paragraph\">Pensak addresses the worthwhile question of mothers going back to work in her chapter on mom guilt. She then devotes a whole chapter in the third part of the book to smoothing the transition period. I wish there had been guidance on discerning when one is suffering from mom guilt about returning to work\u2014to which her response is to \u201cdouble down\u201d on the thing your fear says you shouldn\u2019t do\u2014versus recognizing and responding to changed priorities or desires.&nbsp;<\/p>\n\n\n\n<h3 id=\"h-no-mention-of-progesterone-support-for-postpartum-depression\" class=\"wp-block-heading\"><span id=\"no-mention-of-progesterone-support-for-postpartum-depression\">No mention of progesterone support for postpartum depression<\/span><\/h3>\n\n\n\n<p class=\"wp-block-paragraph\">I thoroughly appreciated Pensak\u2019s coverage of multiple evidence-based treatment options for perinatal mood and anxiety disorders beyond what research studies call \u201cstandard treatment\u201d (aka, cognitive behavioral therapy and\/or selective serotonin reuptake inhibitor antidepressants). To her credit, she mentioned, albeit briefly, the <a href=\"https:\/\/naturalwomanhood.org\/ru\/%d1%82%d0%b0%d0%b1%d0%bb%d0%b5%d1%82%d0%ba%d0%b0-%d0%be%d1%82-%d0%bf%d0%be%d1%81%d0%bb%d0%b5%d1%80%d0%be%d0%b4%d0%be%d0%b2%d0%be%d0%b9-%d0%b4%d0%b5%d0%bf%d1%80%d0%b5%d1%81%d1%81%d0%b8%d0%b8\/\" target=\"_blank\" rel=\"noreferrer noopener\">new pill specifically intended for postpartum depression<\/a>, Zuranolone. I would have loved a mention of restorative reproductive medicine protocols for <a href=\"https:\/\/naturalwomanhood.org\/ru\/%d0%bf%d1%80%d0%b5%d0%be%d0%b4%d0%be%d0%bb%d0%b5%d0%b9%d1%82%d0%b5-%d0%bf%d0%be%d1%81%d0%bb%d0%b5%d1%80%d0%be%d0%b4%d0%be%d0%b2%d1%83%d1%8e-%d0%b4%d0%b5%d0%bf%d1%80%d0%b5%d1%81%d1%81%d0%b8%d1%8e\/\">\u0434\u043e\u0431\u0430\u0432\u043a\u0430 \u043f\u0440\u043e\u0433\u0435\u0441\u0442\u0435\u0440\u043e\u043d\u0430<\/a> for postpartum depression, but in fairness, to my knowledge, there are no research trials published in peer-reviewed journals for her to cite.&nbsp;&nbsp;<\/p>\n\n\n\n<h3 id=\"h-no-connecting-the-dots-between-hormonal-birth-control-and-postpartum-struggles\" class=\"wp-block-heading\"><span id=\"no-connecting-the-dots-between-hormonal-birth-control-and-postpartum-struggles\">No connecting the dots between hormonal birth control and postpartum struggles<\/span><\/h3>\n\n\n\n<p class=\"wp-block-paragraph\">I wished Pensak had addressed hormonal birth control use, for several reasons.&nbsp;One, developing depression from hormonal contraceptive use <a href=\"https:\/\/naturalwomanhood.org\/ru\/%d0%bf%d1%80%d0%be%d1%82%d0%b8%d0%b2%d0%be%d0%b7%d0%b0%d1%87%d0%b0%d1%82%d0%be%d1%87%d0%bd%d1%8b%d0%b5-%d1%81%d1%80%d0%b5%d0%b4%d1%81%d1%82%d0%b2%d0%b0-%d0%bf%d0%be%d1%81%d0%bb%d0%b5%d1%80%d0%be%d0%b4\/\" target=\"_blank\" rel=\"noreferrer noopener\">may be a red flag<\/a> warning to look out for postpartum depression down the road. Secondly, \u201cwhat birth control will you be using?\u201d is a near-universal question at the six-week postpartum checkup, if not before a woman even leaves the hospital. Since we know hormonal contraception can <a href=\"https:\/\/naturalwomanhood.org\/ru\/%d1%82%d0%b5%d0%bc%d0%b0\/%d0%b4%d0%b5%d0%bf%d1%80%d0%b5%d1%81%d1%81%d0%b8%d1%8f\/\" target=\"_blank\" rel=\"noreferrer noopener\">exacerbate or precipitate mental health challenge<\/a>s for some women, why wouldn\u2019t Pensak note that as a potential risk factor to be aware of, particularly for moms with a history of mental health struggles? Thirdly, we know that hormonal contraceptives can cause <a href=\"https:\/\/naturalwomanhood.org\/ru\/%d0%b1%d0%b5%d0%b7%d0%be%d0%bf%d0%b0%d1%81%d0%bd%d1%8b-%d0%bb%d0%b8-%d0%be%d1%80%d0%b0%d0%bb%d1%8c%d0%bd%d1%8b%d0%b5-%d0%ba%d0%be%d0%bd%d1%82%d1%80%d0%b0%d1%86%d0%b5%d0%bf%d1%82%d0%b8%d0%b2%d1%8b\/\" target=\"_blank\" rel=\"noreferrer noopener\">\u043d\u0435\u0434\u043e\u0441\u0442\u0430\u0442\u043a\u0438 \u043f\u0438\u0442\u0430\u043d\u0438\u044f<\/a>\u0438 \u0447\u0442\u043e <a href=\"https:\/\/naturalwomanhood.org\/ru\/%d0%bf%d0%be%d1%81%d0%bb%d0%b5%d1%80%d0%be%d0%b4%d0%be%d0%b2%d0%be%d0%b5-%d0%bf%d0%b8%d1%82%d0%b0%d0%bd%d0%b8%d0%b5\/\" target=\"_blank\" rel=\"noreferrer noopener\">nutrition can play a role in postpartum depression<\/a>. I wish Pensak had connected these dots for her readers.&nbsp;<\/p>\n\n\n\n<p class=\"wp-block-paragraph\">Finally, on an intellectual level, hormonal contraception ill-prepares women for the postpartum lack of control over their own bodies, time, energy, and more. For how many other women is this also true?&nbsp;<\/p>\n\n\n\n<h2 id=\"h-the-verdict\" class=\"wp-block-heading\"><span id=\"the-verdict\">The verdict<\/span><\/h2>\n\n\n\n<p class=\"wp-block-paragraph\">I recommend buying <em>Rattled<\/em> for pregnant or postpartum moms. While my youngest is now one and she has four older siblings, I\u2019m considering buying it for myself to thumb through from time to time. I always want to be reminded of Pensak\u2019s approach to early motherhood, as a time of testing and resilience, strength forged through struggle.
