Definitely a welcome pregnancy, it\u2019s my first, but I have emetophobia [fear of vomiting] and have had constant anxiety about the impending morning sickness literally since I saw that first positive test.\u201d<\/em>\u00a0<\/p>\n\n\n\n<p>In response to this Reddit <a href=\"https:\/\/www.reddit.com\/r\/BabyBumps\/comments\/12fyyg7\/how_bad_is_morning_sickness_really_im_afraid_of\/\" target=\"_blank\" rel=\"noreferrer noopener\">post<\/a> from April 2023, a chorus of well wishers shared their experiences and compared their own symptoms to their mothers\u2019 and sisters\u2019. The range of responses attested to the unpredictability of \u201cmorning sickness\u201d\u2014the nausea, vomiting, and food aversion that define early pregnancy for many women but inexplicably leave others untouched. Here\u2019s the frustrating reality: despite its high prevalence among expecting women, historically, no one has known what causes morning sickness. And because no one has known what causes it, treatments for morning sickness have been rather thin on the ground.\u00a0<\/p>\n\n\n\n<p>But all that could be changing, thanks to a 2023 study by Cambridge University [1]. Researchers found that the hormone \u201cgrowth differentiation factor 15\u201d (<em>GDF15<\/em>) plays a major role in morning sickness. Even more helpful, researchers discovered that women can become immune to <em>GDF15\u2019s<\/em> effects through exposure to the hormone <em>before<\/em> pregnancy. Major news outlets such as <em>The New York Times<\/em> <a href=\"https:\/\/www.nytimes.com\/2023\/12\/13\/health\/morning-sickness-hyperemesis-gravidarum.html?utm_source=dash%2520hudson&amp;utm_medium=instagram&amp;utm_campaign=likeshopme&amp;utm_content=ig-nyt_well\" target=\"_blank\" rel=\"noreferrer noopener\">celebrated<\/a> the discovery as a helpful breakthrough for women everywhere, and expressed hope it could lead to the development of better morning sickness treatments.<\/p>\n\n\n\n<h2 class=\"wp-block-heading\" id=\"h-nausea-and-vomiting-in-pregnancy-nvp-aka-morning-sickness-vs-hyperemesis-gravidarum\"><span id=\"nausea-and-vomiting-in-pregnancy-nvp-aka-morning-sickness-vs-hyperemesis-gravidarum\">Nausea and vomiting in pregnancy (NVP), aka morning sickness, vs. hyperemesis gravidarum<\/span><\/h2>\n\n\n\n<p>At least <a href=\"https:\/\/americanpregnancy.org\/healthy-pregnancy\/pregnancy-health-wellness\/morning-sickness-during-pregnancy\/#:~:text=More%20than%2050%25%20of%20pregnant%20women%20experience%20morning%20sickness.%C2%A0Morning%20sickness%20refers%20to%20the%20nauseous%20feeling%20you%20may%20have%20during%20the%C2%A0first%20trimester%20of%20pregnancy\" target=\"_blank\" rel=\"noreferrer noopener\">half<\/a> of pregnant women experience symptoms of nausea and vomiting in pregnancy (NVP), known colloquially as \u201cmorning sickness.\u201d As countless women can verify, \u201cmorning sickness\u201d is a bit of a misnomer since the symptoms can occur at any time of day. In most cases, morning sickness symptoms disappear around the end of the first trimester. For between 0.3% and 18% of women, however, the symptoms are both severe <em>and<\/em> last throughout the entire pregnancy [2]. This condition is known as hyperemesis gravidarum (HG). Until very recently, the underlying causes of morning sickness and HG were considered a mystery by the medical community.\u00a0<\/p>\n\n\n\n<h2 class=\"wp-block-heading\" id=\"h-the-connection-between-gdf15-morning-sickness-and-hyperemesis-gravidarum-nbsp\"><span id=\"the-connection-between-gdf15-morning-sickness-and-hyperemesis-gravidarum\">The connection between <em>GDF15<\/em>, morning sickness, and hyperemesis gravidarum&nbsp;<\/span><\/h2>\n\n\n\n<p>The major breakthrough from the 2023 Cambridge study involves a hormone called <em>GDF15<\/em>, and it\u2019s normally present in low levels in men and non-pregnant women. On the average day, <em>GDF15 <\/em>helps the body adapt to cellular stress caused by low oxygen levels, cigarette smoking, endurance <a href=\"https:\/\/www.nature.com\/articles\/s41574-021-00529-7#Sec20:~:text=to%20be%20determined.-,Exercise%20and%20GDF15,-Similar%20to%20metformin\" target=\"_blank\" rel=\"noreferrer noopener\">exercise<\/a> and more [1][6]. Increased <em>GDF15<\/em> levels actually <em>lower<\/em> inflammation.\u00a0<\/p>\n\n\n\n<p><em>GDF15<\/em> is also well known as an important factor in appetite suppression and weight loss. Metformin, commonly prescribed to treat <a href=\"https:\/\/www.nature.com\/articles\/s41574-021-00529-7#Sec19:~:text=Metformin%20is%20the%20most%20commonly%20prescribed%20medication%20for%20the%20treatment%20of%20T2DM%20due%20to%20its%20glucose%2Dlowering%20effects%2C%20which%20are%20linked%20to%20drug%20action%20in%20the%20gastrointestinal%20tract%20and%20liver%2C%20where%20drug%20concentrations%20are%C2%A0the%20highest\" target=\"_blank\" rel=\"noreferrer noopener\">diabetes<\/a> (as well as <a href=\"https:\/\/naturalwomanhood.org\/insulin-resistance-pcos-fertility-prediabetes\/\" target=\"_blank\" rel=\"noreferrer noopener\">polycystic ovary syndrome<\/a>), increases <em>GDF15 <\/em>levels [7]. Researchers first discovered <em>GDF15<\/em> back in 2000 and observed that it was naturally high in pregnant women [3].<\/p>\n\n\n\n<p>Later research found that women with hyperemesis gravidarum had much higher levels of <em>GDF15<\/em> than women who didn\u2019t experience HG, and other research determined that high levels of <em>GDF15 <\/em>come mostly from the baby (in its trophoblastic stage of development) rather than from the mother [4][5]. Prior to the 2023 Cambridge study though, research couldn\u2019t definitively answer whether higher <em>GDF15<\/em> levels lead to HG or are caused by it.&nbsp;&nbsp;<\/p>\n\n\n\n<h3 class=\"wp-block-heading\" id=\"h-cambridge-research-high-gdf15-levels-cause-morning-sickness-and-hg-nbsp\"><span id=\"cambridge-research-high-gdf15-levels-cause-morning-sickness-and-hg\">Cambridge research: High <em>GDF15<\/em> levels cause morning sickness and HG&nbsp;<\/span><\/h3>\n\n\n\n<p>After studying over 18,000 participants, the 2023 Cambridge research team similarly found that <em>GDF15<\/em> is higher in women who have nausea and vomiting of pregnancy or hyperemesis gravidarum, compared to pregnant women who have neither. They also confirmed that high <em>GDF15<\/em> levels actually <em>cause <\/em>NVP and HG.&nbsp;<\/p>\n\n\n\n<p>Most surprisingly, the researchers found that higher levels of circulating <em>GDF15<\/em> <em>p<\/em>rior to pregnancy were associated with <em>reduced <\/em>risk of HG and morning sickness. This suggests that women can become desensitized to the effects of <em>GDF15<\/em>, and not suffer from morning sickness or HG, if they\u2019ve had previous exposure before getting pregnant. This makes sense, since the surge of <em>GDF15<\/em> happens in all pregnant women, but not all pregnant women experience morning sickness and very few, by the numbers, suffer hyperemesis gravidarum.&nbsp;<\/p>\n\n\n\n<p>The Cambridge researchers concluded that fetal-produced <em>GDF15 only<\/em> triggers a response in women who weren\u2019t sufficiently desensitized to <em>GDF15 \u2018s <\/em>effects before pregnancy. Since 75% of women who experience HG in one pregnancy will experience it in later pregnancies, it might mean that <em>GDF15<\/em> desensitization is crucial before the <em>first<\/em> pregnancy a woman has.&nbsp;<\/p>\n\n\n\n<h3 class=\"wp-block-heading\" id=\"h-mice-research-mimics-the-surge-of-gdf15-that-happens-during-pregnancy\"><span id=\"mice-research-mimics-the-surge-of-gdf15-that-happens-during-pregnancy\">Mice research mimics the surge of <em>GDF15<\/em> that happens during pregnancy<\/span><\/h3>\n\n\n\n<p>To mimic the natural elevation in <em>GDF15<\/em> during pregnancy, the Cambridge study included a mice testing component to see whether previous <em>GDF15 <\/em>exposure could lead to less nausea and vomiting at higher doses. Researchers found that exposure to small amounts of <em>GDF15 <\/em>prevented symptoms of food aversion and weight loss due to vomiting when the mice were given a larger dose of the hormone. The smaller initial dose effectively made the rats immune to the effects of the larger dose later on.&nbsp;&nbsp;<\/p>\n\n\n\n<h3 class=\"wp-block-heading\" id=\"h-women-with-gdf15-exposure-before-pregnancy-are-less-likely-to-suffer-morning-sickness\"><span id=\"women-with-gdf15-exposure-before-pregnancy-are-less-likely-to-suffer-morning-sickness\">Women with GDF15 exposure before pregnancy are less likely to suffer morning sickness<\/span><\/h3>\n\n\n\n<p>The Cambridge study also studied whether non-pregnant women with abnormally high levels of <em>GDF15 <\/em>were protected from NVP and HG once they became pregnant. The researchers conducted a subanalysis on a group of women with \u03b2-thalassaemia, a rare blood disorder that causes higher-than-average amounts of <em>GDF15<\/em> in the body [1]. When these women became pregnant, only about 5% experienced NVP. This is far less than the 60% of women in the control group (who didn\u2019t have the blood disorder) who experienced morning sickness.<\/p>\n\n\n\n<h2 class=\"wp-block-heading\" id=\"h-how-does-this-new-knowledge-about-the-cause-of-morning-sickness-help-us-better-treat-it\"><span id=\"how-does-this-new-knowledge-about-the-cause-of-morning-sickness-help-us-better-treat-it\">How does this new knowledge about the cause of morning sickness help us better treat it?<\/span><\/h2>\n\n\n\n<p>The Cambridge researchers opined, \u201cThe fact that high <em>GDF15<\/em> levels in the non-pregnant state appear to protect against the development of NVP and HG suggests that strategies which safely increase circulating <em>GDF15<\/em> levels before pregnancy may be useful in the prevention of these conditions.\u201d And for those wondering about the safety of <em>GDF15<\/em> as a possible treatment for morning sickness, the researchers had the following to say: \u201cThe safety of recombinant <em>GDF15<\/em>, at least in the short term and outside of pregnancy, has been demonstrated in Phase 1 clinical trials and this could be administered at low doses before pregnancy with the intention of inducing <em>GDF15\/GFRAL<\/em> desensitization.\u201d&nbsp;<\/p>\n\n\n\n<h3 class=\"wp-block-heading\" id=\"h-what-about-metformin\"><span id=\"what-about-metformin\">What about Metformin?<\/span><\/h3>\n\n\n\n<p>Remember how I mentioned diabetes and PCOS drug Metformin\u2019s effect on <em>GDF15 <\/em>above? The researchers suggested, \u201cAlternatively, metformin robustly increases <em>GDF15<\/em> in humans and could be tested as a prophylactic agent for HG,\u201d but acknowledged that Metformin given to women with gestational diabetes might slow down fetal growth.&nbsp;<\/p>\n\n\n\n<h2 class=\"wp-block-heading\" id=\"h-the-bottom-line-on-the-cause-of-morning-sickness\"><span id=\"the-bottom-line-on-the-cause-of-morning-sickness\">The bottom line on the cause of morning sickness<\/span><\/h2>\n\n\n\n<p>Now that we know what causes morning sickness, more research is needed to learn how we can safely prevent it. Given how much HG can impact both the mom and the preborn baby\u2019s health, researchers should prioritize exploration of whether desensitization in between pregnancies (whether with Metformin or <em>GDF15<\/em> or something else) could prevent repeat HG.
