She offered this comment in a recent <a href=\"https:\/\/www.washingtonpost.com\/wellness\/2023\/06\/26\/endometriosis-period-pain-teens\/\" target=\"_blank\" rel=\"noreferrer noopener\"><em>Washington Post<\/em><\/a> article about endometriosis and teens. While the majority of women with endometriosis aren\u2019t diagnosed until adulthood\u2014 often after other misdiagnoses or being told nothing was wrong\u2014 the <em>Post <\/em>article explored whether the inflammatory disease may start during the teen years and manifest as painful periods.&nbsp;<\/p>\n\n\n\n<p>My painful periods started with my first cycle, at age 13. The pain was so intense that I was distraught at the thought of getting a period every month for most of the rest of my life. I finally looked for some help at age 17, and it came in the form of the birth control pill.<\/p>\n\n\n\n<p>I didn\u2019t get any diagnosis other than \u201cirregular cycles\u201d and \u201cheavy and painful periods.\u201d Still, I didn\u2019t question the gynecologist. The pill helped immensely with my symptoms, and, unlike many women, I didn\u2019t experience any negative side effects from it. Only after I decided to <a href=\"https:\/\/naturalwomanhood.org\/coming-off-the-pill-taryns-story\/\" target=\"_blank\" rel=\"noreferrer noopener\">go off the pill<\/a> and learned to chart my cycle 10 years later, was I finally diagnosed with PCOS. A little later, when my menstrual pain returned, my NaPro doctor (fortunately also <a href=\"https:\/\/naturalwomanhood.org\/considering-endometriosis-surgery-heres-how-to-find-a-good-endo-surgeon\/\" target=\"_blank\" rel=\"noreferrer noopener\">well-trained in endometriosis surgery<\/a>) diagnosed and excised my endometriosis.<\/p>\n\n\n\n<h2 class=\"wp-block-heading\" id=\"h-why-do-so-many-women-have-to-wait-years-for-an-endo-diagnosis\"><span id=\"why-do-so-many-women-have-to-wait-years-for-an-endo-diagnosis\">Why do so many women have to wait years for an endo diagnosis?<\/span><\/h2>\n\n\n\n<p>After my diagnosis and treatment, I read up on endometriosis and learned that this illness (in which tissue similar to the lining of the uterus grows outside the uterus) is typically diagnosed, like it was for me, well into adulthood\u2014and by that time a woman may be unable to get pregnant. <a href=\"https:\/\/www.ncbi.nlm.nih.gov\/pmc\/articles\/PMC3679489\/\" target=\"_blank\" rel=\"noreferrer noopener\">One study<\/a> found an average delay of 6.7 years between onset of symptoms and diagnosis [1]. Other estimates unfortunately report even longer delays of up to 10 or even 12 years from symptom onset to diagnosis.&nbsp;<\/p>\n\n\n\n<h3 class=\"wp-block-heading\" id=\"h-how-hormonal-birth-control-masks-endometriosis-in-teens-and-women\"><span id=\"how-hormonal-birth-control-masks-endometriosis-in-teens-and-women\">How hormonal birth control masks endometriosis&nbsp;in teens and women<\/span><\/h3>\n\n\n\n<p>Why the delay? In my case, it was because my doctor thought the Pill was adequate treatment for my problems. Many women are dismissed by health care providers or family members because they believe that menstrual irregularities, including pain, are a normal part of a woman\u2019s experience. In many cases, women are told to expect cramping for generations. Mothers then pass on this mistaken belief to their daughters out of ignorance, not malice.<\/p>\n\n\n\n<p>According to <a href=\"https:\/\/naturalwomanhood.org\/commonly-asked-questions-about-endometriosis-causes-treatments-cures-napro-technology-a-natural-womanhood-endo-faq-2020\/\" target=\"_blank\" rel=\"noreferrer noopener\">this Natural Womanhood Q&amp;A<\/a>, many doctors do not want to perform the laparoscopic surgery required to diagnose endometriosis definitively on teenage girls, so they put them on the Pill instead. Indeed, while there is, as of 2017, <a href=\"https:\/\/pubmed.ncbi.nlm.nih.gov\/27342684\/\" target=\"_blank\" rel=\"noreferrer noopener\">reportedly<\/a> no consensus on whether surgery should be avoided among adolescents, this <a href=\"https:\/\/www.ncbi.nlm.nih.gov\/pmc\/articles\/PMC7457195\/#cit0089\" target=\"_blank\" rel=\"noreferrer noopener\">expert opinion issued in 2018<\/a> recommended that surgery be pursued only as a <em>last<\/em> stage of diagnosis, after contraceptives and NSAIDs have been unsuccessful in relieving pain [2][3]. This approach reflects the<a href=\"https:\/\/www.acog.org\/clinical\/clinical-guidance\/committee-opinion\/articles\/2018\/12\/dysmenorrhea-and-endometriosis-in-the-adolescent\" target=\"_blank\" rel=\"noreferrer noopener\"> position<\/a> (last reaffirmed in 2021) of the American College of Obstetricians and Gynecologists (ACOG) as well. <\/p>\n\n\n\n<p>It\u2019s important to note, however, that responding to the Pill is not a substitute for treatment. While it often helps <em>manage<\/em> endometriosis symptoms, it doesn\u2019t <em>resolve<\/em> them. Instead, the endometriosis continues and <a href=\"https:\/\/naturalwomanhood.org\/manage-endometriosis-pain-with-napro-fertility-awareness\/\" target=\"_blank\" rel=\"noreferrer noopener\">can even become worse<\/a>. And when a woman stops taking the Pill to try to get pregnant, <a href=\"https:\/\/naturalwomanhood.org\/twelve-stripes-deep\/\" target=\"_blank\" rel=\"noreferrer noopener\">often, she finds that she can\u2019t conceive<\/a>.<\/p>\n\n\n\n<h2 class=\"wp-block-heading\" id=\"h-managing-endometriosis-in-teens\"><span id=\"managing-endometriosis-in-teens\">Managing endometriosis in teens <\/span><\/h2>\n\n\n\n<p>So, what should you do if your teenage daughter is experiencing symptoms of endometriosis \u2014which can include, in addition to painful and\/or irregular periods, gastrointestinal problems, neuropathy, fatigue, painful ovulation, and premenstrual spotting?<\/p>\n\n\n\n<h3 class=\"wp-block-heading\" id=\"h-enroll-her-in-a-cycle-charting-course\"><span id=\"enroll-her-in-a-cycle-charting-course\">Enroll her in a cycle charting course&nbsp;<\/span><\/h3>\n\n\n\n<p>The first step is to help her learn to chart her cycle. Several organizations have specific fertility awareness method (FAM) education programs for adolescents, including <a href=\"https:\/\/femmhealth.org\/teacher-directory\/?_search_teachers=teen\" target=\"_blank\" rel=\"noreferrer noopener\">FEMM<\/a>, <a href=\"https:\/\/www.pearlandthistle.com\/for-parents.html#\/\" target=\"_blank\" rel=\"noreferrer noopener\">Pearl and Thistle<\/a>, and the <a href=\"https:\/\/guidingstarproject.com\/cycle-show\/\" target=\"_blank\" rel=\"noreferrer noopener\">Guiding Star Project<\/a>. Natural Womanhood\u2019s Mothers of (Pre)Teens <a href=\"https:\/\/naturalwomanhood.org\/mothers-of-teens-online-course-2\/\" target=\"_blank\" rel=\"noreferrer noopener\">program<\/a> is another excellent resource for helping young girls understand and contextualize the experience of puberty and periods\u2014 including what&#8217;s normal, and how to recognize and get help for what is not. And while an app should never replace instruction from a teacher trained in a particular method, <a href=\"https:\/\/naturalwomanhood.org\/fertility-tracking-apps-for-teens\/\" target=\"_blank\" rel=\"noreferrer noopener\">these teen-friendly fertility awareness apps<\/a> may be helpful supplements.\u00a0<\/p>\n\n\n\n<h3 class=\"wp-block-heading\" id=\"h-learn-for-yourself-and-then-teach-her-what-s-normal\"><span id=\"learn-for-yourself-and-then-teach-her-whats-normal\">Learn for yourself and then teach her what\u2019s normal<\/span><\/h3>\n\n\n\n<p>It\u2019s also important to understand what\u2019s normal in an adolescent\u2019s cycle versus what\u2019s not normal. <a href=\"https:\/\/naturalwomanhood.org\/naturalwomanhood-org-teen-periods-part-1\/\" target=\"_blank\" rel=\"noreferrer noopener\">For instance<\/a>, it can take up to three years for a girl\u2019s cycle to become regular (i.e., of a consistent length). It\u2019s not uncommon for a teen cycle to be as long as 60 days. Teen girls are also more likely than adult women to experience some pain and\/or heavy bleeding during their periods. However, <strong>any level of pain or bleeding that interferes with a girl\u2019s ability to function normally is a red flag that should be brought to a health care provider\u2019s attention<\/strong>.<\/p>\n\n\n\n<h3 class=\"wp-block-heading\" id=\"h-be-wary-of-attempts-to-go-straight-to-hormonal-birth-control-to-treat-period-pain-or-other-problems\"><span id=\"be-wary-of-attempts-to-go-straight-to-hormonal-birth-control-to-treat-period-pain-or-other-problems\">Be wary of attempts to go straight to hormonal birth control to treat period pain or other problems<\/span><\/h3>\n\n\n\n<p>No health care provider should prescribe hormonal contraception to a teenage girl without disclosing the risks,<em> including the <\/em><a href=\"https:\/\/naturalwomanhood.org\/naturalwomanhood-org-teen-periods-part-2\/\" target=\"_blank\" rel=\"noreferrer noopener\"><em>risks that are particular to<\/em><\/a><em> an adolescent body<\/em>, to her and her parent(s). Ultimately, it\u2019s up to the patient and her parents whether or not she goes on birth control, but they need to be able to give informed consent. It\u2019s important, if possible, to try to find the cause of her symptoms and treat them directly. Look for a provider trained in NaPro, FEMM, or NeoFertility methods of medical management, or one who uses another restorative reproductive medicine (RRM) approach. <a href=\"https:\/\/naturalwomanhood.org\/find-a-doctor\/\" target=\"_blank\" rel=\"noreferrer noopener\">Here is a good place to start<\/a>.<\/p>\n\n\n\n<h3 class=\"wp-block-heading\" id=\"h-what-difference-might-earlier-diagnosis-of-endometriosis-in-teens-make\"><span id=\"what-difference-might-earlier-diagnosis-of-endometriosis-in-teens-make\">What difference might earlier diagnosis of endometriosis in teens make?<\/span><\/h3>\n\n\n\n<p><a href=\"https:\/\/www.ncbi.nlm.nih.gov\/pmc\/articles\/PMC4432718\/\" target=\"_blank\" rel=\"noreferrer noopener\">In 2015, a small study<\/a> of 25 girls under 21 years of age concluded that: <\/p>\n\n\n\n<blockquote class=\"wp-block-quote is-layout-flow wp-block-quote-is-layout-flow\">\n<p>\u201cTimely referral to a gynecologist experienced with laparoscopic diagnosis and treatment of endometriosis is critical to expedite care for adolescents with pelvic pain. Once the disease is diagnosed and treated, these patients have favorable outcomes with hormonal and nonhormonal therapy\u201d [4]. <\/p>\n<\/blockquote>\n\n\n\n<p>Notably, <a href=\"https:\/\/naturalwomanhood.org\/can-endometriosis-come-back-after-surgery-signs-of-endometriosis-returning\/\" target=\"_blank\" rel=\"noreferrer noopener\">in research studies<\/a> of women with endometriosis, <em>excision<\/em> surgery (rather than ablation surgery) is more effective in removing endometriosis. Excision surgery is also more likely than ablation to keep endometriosis lesions from returning.<\/p>\n\n\n\n<p>Endometriosis can be a challenging illness to diagnose, live with, and treat. But there is hope, even for teenagers with painful periods. By learning to chart and finding a good surgeon, teen girls with endometriosis can receive a diagnosis earlier.
