I knew the word \u201cadrenal\u201d\u2014as in the gland that produces cortisol and a few other hormones\u2014and I am also well acquainted with the word \u201cfatigue,\u201d having had both chronic fatigue syndrome (CFS) and fibromyalgia for 20 years (and a small child for the past one-and-a-half). But \u201cadrenal fatigue?\u201d I had no clue what this now-ubiquitous buzzphrase could mean.<\/p>\n\n\n\n<p>Turns out, I probably hadn\u2019t heard of \u201cadrenal fatigue\u201d before because it\u2019s not an accepted medical diagnosis. But as someone with both fibromyalgia and CFS\u2014two mysterious chronic illnesses\u2014I also know that just because something isn\u2019t \u201caccepted\u201d doesn\u2019t mean that there isn\u2019t real suffering going on behind it. (Fibromyalgia and CFS are now accepted medical diagnoses, but that hasn\u2019t always been the case, and even today, symptoms like chronic pain and chronic fatigue are often dismissed.)<\/p>\n\n\n\n<p>So, is adrenal fatigue real? Or is it social media hokum? And how might HPA axis dysregulation play a role in adrenal fatigue symptoms? Let\u2019s dig deeper:<\/p>\n\n\n\n<h2 class=\"wp-block-heading\" id=\"h-what-is-adrenal-fatigue-nbsp\"><span id=\"what-is-adrenal-fatigue\">What is adrenal fatigue?&nbsp;<\/span><\/h2>\n\n\n\n<p>According to the <a href=\"https:\/\/www.endocrine.org\/patient-engagement\/endocrine-library\/adrenal-fatigue\">Endocrine Society<\/a>, \u201cadrenal fatigue\u201d is a term used to describe \u201ca group of symptoms that are said to occur in people who are under long-term mental, emotional, or physical stress.\u201d It is said to be caused when the adrenal glands can\u2019t keep up with the body\u2019s demand for cortisol, which is the hormone that responds to stress. This condition results in symptoms such as fatigue, difficulty falling asleep or waking up, craving salt or sugar, and needing caffeine or other stimulants to get through the day.<\/p>\n\n\n\n<p>The Mayo Clinic points out that <a href=\"https:\/\/www.mayoclinic.org\/diseases-conditions\/addisons-disease\/expert-answers\/adrenal-fatigue\/faq-20057906#:~:text=Adrenal%20insufficiency%20can%20be%20diagnosed,insufficiency%20caused%20by%20chronic%20stress.\"><em>adrenal insufficiency<\/em><\/a> is an accepted medical diagnosis referring to inadequate production of one or more of the hormones produced by the adrenal glands. Symptoms include fatigue, body aches, unexplained weight loss, low blood pressure, lightheadedness, loss of hair on the body, and hyperpigmentation. It is diagnosable through blood- and other diagnostic tests. According to the Mayo Clinic, advocates of the adrenal fatigue diagnosis believe that it is a milder form of adrenal insufficiency, undetectable by a blood test.<\/p>\n\n\n\n<h2 class=\"wp-block-heading\" id=\"h-adrenal-fatigue-or-hpa-axis-dysregulation\"><span id=\"adrenal-fatigue-or-hpa-axis-dysregulation\">Adrenal fatigue or HPA axis dysregulation?<\/span><\/h2>\n\n\n\n<p>According to <a href=\"https:\/\/www.frontiersin.org\/articles\/10.3389\/fnbeh.2020.601939\/full#:~:text=A%20major%20component%20of%20the,autonomic%20nervous%20system%20(ANS).\">this article<\/a> in <em>Frontiers in Behavioral Neuroscience, <\/em>the HPA (hypothalamic-pituitary-adrenal) axis is the \u201cneuroendocrine mechanism that mediates the effects of stressors by regulating numerous physiological processes, such as metabolism, immune responses, and the autonomic nervous system\u201d [1]. Some people use the phrase \u201cHPA axis dysregulation\u201d or \u201cHPA axis dysfunction\u201d as a <a href=\"https:\/\/centrespringmd.com\/adrenal-dysfunction-fatigue-cortisol-hpa-axis-dysfunction\/\">synonym for adrenal fatigue<\/a>. Others believe that it is actually a more accurate term than adrenal fatigue.<\/p>\n\n\n\n<p>For example, well-known naturopath Dr. Jolene Brighten <a href=\"https:\/\/drbrighten.com\/hpa-axis-healing-anxiety\/\">writes that<\/a> \u201cwhile your adrenal symptoms and anxiety are very real, adrenal fatigue is not. Your adrenals (unlike your ovaries) should never give out. Instead, this is more accurately HPA dysregulation, that is your brain and adrenal communication is breaking down.\u201d<\/p>\n\n\n\n<p>Chronic stress <a href=\"https:\/\/www.ncbi.nlm.nih.gov\/pmc\/articles\/PMC6057754\/\">can cause dysfunction<\/a> in the HPA axis [2]. This dysfunction <a href=\"https:\/\/physoc.onlinelibrary.wiley.com\/doi\/full\/10.14814\/phy2.14644\">can play a role<\/a> in cardiovascular disease, metabolic disease (such as type 2 diabetes), and inflammatory and autoimmune disease, <a href=\"https:\/\/www.pointinstitute.org\/wp-content\/uploads\/2012\/10\/standard_v_9.2_hpa_axis.pdf\">as well as<\/a> some mental illnesses, chronic fatigue syndrome, fibromyalgia, asthma, and eczema [3].<\/p>\n\n\n\n<h2 id=\"how-hormonal-birth-control-dysregulates-the-hpa-axis\" class=\"wp-block-heading\">How hormonal birth control dysregulates the HPA axis<\/h2>\n\n\n\n<p>Several decades of research, summarized in evolutionary psychologist <a href=\"https:\/\/naturalwomanhood.org\/new-book-explores-how-hormonal-birth-control-affects-womens-brains-2019\/\">Sarah E. Hill\u2019s<\/a> book<em> <\/em><a href=\"https:\/\/www.sarahehill.com\/your-brain-on-birth-control\/\"><em>This Is Your Brain on Birth Control<\/em><\/a>, suggests that <a href=\"https:\/\/naturalwomanhood.org\/hormonal-birth-control-chronic-stress-response-anxiety-apa-axis-2019\/\">women who take hormonal birth control experience two key stress response changes<\/a>. Firstly, these women appear to have a blunted response to stressors, even happy stressors like an unexpected job promotion, or surprise flowers from a loved one. While a blunted response to stressors might sound like a good thing, as Natural Womanhood\u2019s Grace Emily Stark pointed out previously: <\/p>\n\n\n\n<blockquote class=\"wp-block-quote is-layout-flow wp-block-quote-is-layout-flow\">\n<p>\u201cThe stress response is our body\u2019s way of letting us know that something significant is happening and helping us react accordingly. Some stress is normal (in fact, needed) to help us live meaningful, interesting, lives\u2014and to make a quick getaway in dicey situations.\u201d Normally, \u201ca healthy person will exhibit increased cortisol in dangerous or exciting situations, but will also experience a drop in cortisol once the precipitating event has passed.\u201d&nbsp;<\/p>\n<\/blockquote>\n\n\n\n<p>In addition to having a muted response to both positive and negative stressors, women on the Pill tend to show signs of chronic stress as evidenced by \u201c<a href=\"https:\/\/www.ncbi.nlm.nih.gov\/pubmed\/29074884\">higher than average levels of total cortisol<\/a>, high levels of <a href=\"https:\/\/www.contraceptionjournal.org\/article\/0010-7824(95)00098-U\/fulltext\">corticosteroid binding globulins (CBGs<\/a>), and <a href=\"https:\/\/pubmed.ncbi.nlm.nih.gov\/24773147-circulating-cortisol-levels-after-exogenous-cortisol-administration-are-higher-in-women-using-hormonal-contraceptives-data-from-two-preliminary-studies\/\">dysregulated responses to exogenously administered cortisol<\/a>,\u201d according to Hill\u2019s <a href=\"https:\/\/www.sarahehill.com\/pill_and_stress\/\">website<\/a>. Hill summarizes in her book: <\/p>\n\n\n\n<blockquote class=\"wp-block-quote is-layout-flow wp-block-quote-is-layout-flow\">\n<p>\u201cThe Pill-taking women\u2019s pattern of HPA-axis function looks suspiciously similar to that of someone who has experienced chronic stress, suggesting that the Pill might actually cause the HPA axis to go into overdrive, requiring it to take coordinated action to blunt itself.\u201d&nbsp;<\/p>\n<\/blockquote>\n\n\n\n<p>Concerningly, Hill writes on the website, \u201c[this stress response pattern] is often observed in <a href=\"https:\/\/www.ncbi.nlm.nih.gov\/pmc\/articles\/PMC3580170\/\">children who have been abused or abandoned<\/a> and those with a joint diagnosis of <a href=\"https:\/\/www.ncbi.nlm.nih.gov\/pmc\/articles\/PMC5435820\/#!po=40.6250\">PTSD and major depressive disorder<\/a>.\u201d She concludes, <\/p>\n\n\n\n<blockquote class=\"wp-block-quote is-layout-flow wp-block-quote-is-layout-flow\">\n<p><strong>\u201cWe should all be alarmed by the fact that the stress hormone profiles of women who are on the birth control pill <\/strong><a href=\"https:\/\/www.ncbi.nlm.nih.gov\/pmc\/articles\/PMC5658328\/\"><strong>look more like those belonging to trauma victims<\/strong><\/a><strong> than they do like those belonging to otherwise healthy young women.\u201d&nbsp;&nbsp;<\/strong><\/p>\n<\/blockquote>\n\n\n\n<h2 class=\"wp-block-heading\" id=\"h-coping-with-stress-nbsp\"><span id=\"coping-with-stress\">Coping with stress&nbsp;<\/span><\/h2>\n\n\n\n<p>Both the Endocrine Society and the Mayo Clinic express concern that having a \u201cdiagnosis\u201d of adrenal fatigue can keep a patient from getting a \u201creal\u201d diagnosis, such as depression or fibromyalgia, which can have similar symptoms. Without a proper diagnosis of the underlying issue(s), they say, a patient can\u2019t receive effective treatment.<\/p>\n\n\n\n<p>In an article for <a href=\"https:\/\/www.health.harvard.edu\/blog\/is-adrenal-fatigue-real-2018022813344\">Harvard Health Publishing<\/a>, Dr. Marcelo Campos writes that if a thorough medical examination of someone complaining of adrenal fatigue turns up no answers, it\u2019s important to examine one\u2019s lifestyle. \u201cWhy would your adrenals be drained?\u201d he writes. \u201cTake a better look at what types of stress might be affecting you.\u201d He also notes that while some medical professionals prescribe cortisol analogs to treat adrenal fatigue, \u201ccortisol replacement can be dangerous even in small doses. Unintended consequences can include osteoporosis, diabetes, weight gain, and heart disease.\u201d<\/p>\n\n\n\n<p>Stress reduction is key to restoring the HPA axis to healthy functioning. She also says that exercise, <a href=\"https:\/\/naturalwomanhood.org\/how-an-anti-inflammatory-diet-can-help-balance-hormones\/\">an anti-inflammatory diet<\/a>, improving sleep, and reducing caffeine can help. Oh, and if you\u2019re on hormonal birth control, getting off of it might help, too.&nbsp;&nbsp;<\/p>\n\n\n\n<p>Finally, be sure to&nbsp; <a href=\"https:\/\/naturalwomanhood.org\/14-health-concerns-fertility-awareness-charting-can-reveal-part-2\/\">chart your symptoms<\/a> in whatever app or paper chart you use to chart your menstrual cycle.
