The Opill, which has since been acquired by Dublin-based pharmaceutical company Perrigo, is a progestin-only hormonal contraceptive. As with any other drug seeking OTC approval, the FDA was tasked with <a href=\"https:\/\/www.fda.gov\/media\/140598\/download\" target=\"_blank\" rel=\"noreferrer noopener\">determining<\/a> that Opill is \u201csafe and effective for use by a consumer without supervision by a licensed healthcare professional.\u201d Notably, the <a href=\"https:\/\/www.acog.org\/clinical\/clinical-guidance\/committee-opinion\/articles\/2019\/10\/over-the-counter-access-to-hormonal-contraception\" target=\"_blank\" rel=\"noreferrer noopener\">American College of Obstetricians and Gynecologists<\/a> (ACOG) has been openly pushing for over-the-counter birth control since 2019, and <a href=\"https:\/\/www.acog.org\/news\/news-releases\/2023\/07\/acog-praises-fda-approval-of-over-the-counter-access-to-birth-control-pill\" target=\"_blank\" rel=\"noreferrer noopener\">therefore praised<\/a> the FDA&#8217;s recent decision on Opill.&nbsp;<\/p>\n\n\n\n<p>While the recent Supreme Court decision overturning <em>Roe v. Wade<\/em> has intensified public scrutiny on the availability of contraceptives and abortion pills to prevent or end unwanted pregnancies, efforts to make these drugs more easily accessible have been ongoing for years. HRA conducted studies for seven years prior to its application to the FDA to make Opill available without a prescription (though it bears mentioning that the company&#8217;s largest study followed users for just six months, and the research was <a href=\"https:\/\/archive.ocsotc.org\/#:~:text=The%20Oral%20Contraceptives%20(OCs)%20Over,reproductive%20age%20easier%20access%20to\" target=\"_blank\" rel=\"noreferrer noopener\">partly funded<\/a> by an advocacy group who pushes for over-the-counter birth control). The chief strategist for the company <a href=\"https:\/\/apnews.com\/article\/abortion-science-health-birth-control-861d6d425e692ee1ead1d59926e5f3cd\" target=\"_blank\" rel=\"noreferrer noopener\">argued<\/a>, \u201cFor a product that has been available for the last 50 years, that has been used safely by millions of women, we thought it was time to make it more available.\u201d<\/p>\n\n\n\n<p>Unfortunately, the reality is that birth control comes with a <a href=\"https:\/\/naturalwomanhood.org\/topic\/birth-control-side-effects\/\" target=\"_blank\" rel=\"noreferrer noopener\">myriad of side effects<\/a> ranging from uncomfortable to life-threatening, and Opill is no exception. Here I\u2019ll outline how the drug works, what the dangers are, and why making it available over-the-counter poses a threat to women\u2019s health.<\/p>\n\n\n\n<h2 class=\"wp-block-heading\" id=\"h-progesterone-vs-progestin\"><span id=\"progesterone-vs-progestin\">Progesterone vs progestin<\/span><\/h2>\n\n\n\n<p><a href=\"https:\/\/naturalwomanhood.org\/fam-basics-progesterone\/\" target=\"_blank\" rel=\"noreferrer noopener\">Progesterone<\/a> is a natural hormone produced by the corpus luteum (the <a href=\"https:\/\/pubmed.ncbi.nlm.nih.gov\/19406027\/#:~:text=The%20human%20corpus%20luteum%20is,the%20maintenance%20of%20early%20pregnancy.\" target=\"_blank\" rel=\"noreferrer noopener\">temporary endocrine gland<\/a> that forms from the empty ovarian follicle after it releases an egg during ovulation) [1]. Progesterone levels rise after ovulation during the luteal phase of the menstrual cycle, because progesterone promotes the growth of the uterine wall lining (<a href=\"https:\/\/naturalwomanhood.org\/fam-basics-everything-you-need-to-know-about-your-endometrium\/\" target=\"_blank\" rel=\"noreferrer noopener\">endometrium<\/a>), and a thickened endometrium is essential for supporting a pregnancy. If an egg is not fertilized, then progesterone levels begin to fall and the endometrium breaks down and is shed during menstruation.&nbsp;<\/p>\n\n\n\n<p>If an egg is fertilized and implants within the endometrium, then progesterone continues to be produced to maintain the endometrium. High progesterone levels prevent other eggs from maturing, support fetal development, and prepare the body for labor and breastfeeding.&nbsp;<\/p>\n\n\n\n<p>In contrast, progestin is a synthetically-produced steroid that mimics natural progesterone. There are many forms of synthetic progestins (collectively referred to as progestogens) available, and they are often <a href=\"https:\/\/patient.info\/doctor\/progestogens\" target=\"_blank\" rel=\"noreferrer noopener\">grouped<\/a> by \u201cgeneration\u201d (time since being introduced to the market) and by whether they are a derivative of testosterone or progesterone.&nbsp;<\/p>\n\n\n\n<h2 class=\"wp-block-heading\" id=\"h-so-what-is-opill\"><span id=\"so-what-is-opill\">So, what is Opill?<\/span><\/h2>\n\n\n\n<p>Opill\u2019s active ingredient is the synthetic progestin norgestrel. Notably, norgestrel is nearly identical to the chemical makeup of levonorgestrel, the synthetic active ingredient in &#8220;<a href=\"https:\/\/naturalwomanhood.org\/plan-b-vs-abortion-pills-pregnancy-prevention\/\" target=\"_blank\" rel=\"noreferrer noopener\">Plan B<\/a>&#8221; emergency contraception. But levonorgestrel is far more potent. <\/p>\n\n\n\n<p>Oral hormonal contraceptives like Opill, with progestin as the only active ingredient, are often referred to as \u201cthe mini-pill.\u201d This distinguishes it from &#8220;the Pill,&#8221; the most common form of hormonal birth control, which is a combination of progestin <em>and<\/em> synthetic estrogen.&nbsp;<\/p>\n\n\n\n<h2 class=\"wp-block-heading\" id=\"h-opill-aka-the-mini-pill-works-somewhat-differently-than-the-pill\"><span id=\"opill-aka-the-mini-pill-works-somewhat-differently-than-the-pill\">Opill, aka, \u201cthe mini-pill,\u201d works somewhat differently than the Pill<\/span><\/h2>\n\n\n\n<p>The primary mechanism of action for combined estrogen-progestin contraceptive pills (&#8220;the Pill&#8221;) is suppression of ovulation by preventing the cyclical <a href=\"https:\/\/pubmed.ncbi.nlm.nih.gov\/12308926\/#:~:text=Abstract,preventing%20follicular%20development%20and%20ovulation.\" target=\"_blank\" rel=\"noreferrer noopener\">luteinizing hormone (LH) surge<\/a> that normally triggers the release of an egg during ovulation. No egg means no possibility of conception from sex [2]&nbsp;<\/p>\n\n\n\n<p>The mini-pill\u2019s primary mechanism of action is \u201cthickening the cervical mucus to inhibit sperm penetration, lowering the midcycle LH and FSH peaks, slowing the movement of the ovum through the fallopian tubes, and altering the endometrium,\u201d according to the <a href=\"https:\/\/www.accessdata.fda.gov\/drugsatfda_docs\/label\/2017\/017031s035s036lbl.pdf\" target=\"_blank\" rel=\"noreferrer noopener\">product label<\/a>. If an egg is fertilized, either type of pill will act to prevent the newly conceived life from implanting within the endometrium, because each type of pill also functions to thin the lining of the uterus\u2014<a href=\"https:\/\/naturalwomanhood.org\/can-plan-b-cause-abortion\/\" target=\"_blank\" rel=\"noreferrer noopener\">a mechanism of action that some consider to be abortifacient<\/a>. <\/p>\n\n\n\n<p>Due to the mini-pill\u2019s lower doses of progestin, one would expect <a href=\"https:\/\/naturalwomanhood.org\/can-you-chart-your-cycle-while-on-the-pill-birth-control-2-19\/\" target=\"_blank\" rel=\"noreferrer noopener\">breakthrough ovulation<\/a> to occur more often with the mini-pill (vs. the Pill), increasing the possibility that it will function to prevent pregnancy through the endometrial thinning mechanism. The American College of Obstetricians and Gynecologists <a href=\"https:\/\/www.acog.org\/womens-health\/faqs\/progestin-only-hormonal-birth-control-pill-and-injection\" target=\"_blank\" rel=\"noreferrer noopener\">reports<\/a>, &#8220;Progestin stops&nbsp;<a>ovulation<\/a>, but it does not do so consistently. About 4 in 10 women who use progestin-only pills will continue to ovulate.&#8221;<\/p>\n\n\n\n<h2 class=\"wp-block-heading\" id=\"h-is-progestin-only-contraception-safer-than-the-pill\"><span id=\"is-progestin-only-contraception-safer-than-the-pill\">Is progestin-only contraception safer than the Pill?<\/span><\/h2>\n\n\n\n<p>Opill likely received a somewhat speedier FDA approval because progestin-only pills are commonly viewed as a safer alternative to the Pill (which, after all, <a href=\"https:\/\/www.cancer.org\/healthy\/cancer-causes\/general-info\/known-and-probable-human-carcinogens.html\" target=\"_blank\" rel=\"noreferrer noopener\">is a known carcinogen<\/a>). This is because the synthetic estrogen in the Pill is the main culprit for its most serious and potentially deadly adverse effects, like blood clots, heart attacks, and stroke.&nbsp;<\/p>\n\n\n\n<p>What many people don\u2019t know, however, is that the mini-pill <em>still<\/em> comes with a host of ugly side effects, including increased risk of <a href=\"https:\/\/pubmed.ncbi.nlm.nih.gov\/29211679\/\" target=\"_blank\" rel=\"noreferrer noopener\">breast cancer<\/a>, <a href=\"https:\/\/pubmed.ncbi.nlm.nih.gov\/26808155\/\" target=\"_blank\" rel=\"noreferrer noopener\">cervical cancer<\/a>, and brain cancer\u2014with one <a href=\"https:\/\/www.ncbi.nlm.nih.gov\/pmc\/articles\/PMC4386952\/#:~:text=WHAT%20THIS%20STUDY%20ADDS,term%20users%20of%20hormonal%20contraceptives.\" target=\"_blank\" rel=\"noreferrer noopener\">study<\/a> showing that the risk of glioma nearly triples for women taking progestin-only birth control [3][4][5].\u00a0And while the Pill is associated with a decreased risk of ovarian cancer, progestin-only contraception does <em>not<\/em> protect against ovarian cancer. [6] <\/p>\n\n\n\n<p>Progestin is also known to <a href=\"https:\/\/naturalwomanhood.org\/reasons-women-need-periods-you-need-a-period-the-role-of-the-menstrual-cycle-in-bone-health-development-2020\/\" target=\"_blank\" rel=\"noreferrer noopener\">decrease bone density<\/a>, putting women at a higher risk for osteoporosis and bone fractures. Along with all forms of hormonal birth control, progestin increases the risk of <a href=\"https:\/\/jamanetwork.com\/journals\/jamapsychiatry\/fullarticle\/2552796\" target=\"_blank\" rel=\"noreferrer noopener\">depression<\/a> [7]. The FDA additionally lists <a href=\"https:\/\/www.fda.gov\/consumers\/free-publications-women\/birth-control\" target=\"_blank\" rel=\"noreferrer noopener\">high blood pressure<\/a> as a side effect. This <a href=\"https:\/\/www.ncbi.nlm.nih.gov\/pmc\/articles\/PMC7612921\/#:~:text=Progestogen%2Donly%20pills%20were%20not,progestogen%2Donly%20contraception%20and%20IBD.\" target=\"_blank\" rel=\"noreferrer noopener\">2022 study<\/a> found an increased risk of ulcerative colitis in current users of progestin-only pills [8]. As discussed above, the mini-pill also <a href=\"https:\/\/academic.oup.com\/humrep\/article\/16\/7\/1527\/693441\" target=\"_blank\" rel=\"noreferrer noopener\">alters the endometrium<\/a> and may lead, over time, to uterine lining <a href=\"https:\/\/naturalwomanhood.org\/depo-provera-birth-control-shot-long-term-effects-fertility-miscarriage\/\" target=\"_blank\" rel=\"noreferrer noopener\">atrophy<\/a>, which can make it harder to conceive in the future [9].\u00a0<\/p>\n\n\n\n<p>A laundry list of less severe side effects also accompanies use of the mini-pill. More than 5% of women in the clinical trials for Opill reported the following adverse reactions: headache, dizziness, nausea, increased appetite, abdominal pain, cramps and bloating, fatigue, vaginal discharge, dysmenorrhea (painful menstruation), nervousness, backache, breast discomfort, and acne.&nbsp;<\/p>\n\n\n\n<p>Many women on the mini-pill may also experience irregular bleeding. Because the mini-pill does not consistently suppress ovulation, this is not necessarily a surprising effect. Nevertheless, one can easily imagine a scenario where a woman or girl with an <a href=\"https:\/\/naturalwomanhood.org\/topic\/cycle-dysfunctions\/irregular-period\/\" target=\"_blank\" rel=\"noreferrer noopener\">irregular period<\/a> is told that birth control can make her cycles more predictable and \u201cregular.\u201d If this prompts her to buy and take an over-the-counter contraceptive, she may unknowingly cause even worse bleeding patterns.<\/p>\n\n\n\n<h2 class=\"wp-block-heading\" id=\"h-opill-concerns-about-over-the-counter-availability\"><span id=\"opill-concerns-about-over-the-counter-availability\">Opill: Concerns about over-the-counter availability<\/span><\/h2>\n\n\n\n<p>Several of Opill\u2019s side effects, as found on the <a href=\"https:\/\/www.accessdata.fda.gov\/drugsatfda_docs\/label\/2017\/017031s035s036lbl.pdf\" target=\"_blank\" rel=\"noreferrer noopener\">label<\/a>, raise serious concerns about its over-the-counter availability. The label lists warnings about increased risk of <a href=\"https:\/\/naturalwomanhood.org\/how-do-you-treat-ectopic-pregnancy\/\" target=\"_blank\" rel=\"noreferrer noopener\">ectopic pregnancies<\/a> for women who do become pregnant while using it, abnormal follicular growth causing ovarian cysts potentially requiring surgery, and acute disturbances of liver function.&nbsp;<\/p>\n\n\n\n<p>Tellingly, page 10 of the label reads, \u201cThese are not all the possible side effects of Opill. Call your doctor for medical advice about side effects.\u201d Making a medication available over-the-counter when the label currently recommends conversations with a medical provider before beginning use is counterintuitive.<\/p>\n\n\n\n<h2 class=\"wp-block-heading\" id=\"h-the-negative-effects-of-deregulating-access-to-birth-control\"><span id=\"the-negative-effects-of-deregulating-access-to-birth-control\">The negative effects of deregulating access to birth control<\/span><\/h2>\n\n\n\n<p>At present, more than a dozen <a href=\"https:\/\/www.liveaction.org\/news\/birth-control-north-carolina\/\" target=\"_blank\" rel=\"noreferrer noopener\">states<\/a> already allow birth control prescriptions sans a patient-doctor relationship or prescription, requiring only approval from a pharmacist after a woman fills out a questionnaire. Undoubtedly, now that access to Opill is deregulated, many women will not hesitate to take it, believing that it must be safer because it\u2019s over-the-counter. But the widely-held illusion that there are no serious risks or side effects involved is just that\u2014an illusion. This false understanding can only be exacerbated by women being able to acquire hormonal contraceptives without even the most cursory of conversations with a healthcare provider.<\/p>\n\n\n\n<p>Women deserve <em>more<\/em> education about hormonal contraception than they\u2019re currently receiving from their healthcare providers, not less, as may now occur since Opill is easily accessible over-the-counter. Currently, doctors are not only eager to prescribe hormonal birth control, but many also downplay the risks and dismiss hesitations around the pill. (I experienced this myself when I went to an OB\/GYN for the first time as a teenager due to several missed periods. The doctor\u2019s only recommendation was hormonal birth control, which she told me had some risk of side effects \u201clike <a href=\"https:\/\/naturalwomanhood.org\/topic\/birth-control-side-effects\/blood-clots\/\" target=\"_blank\" rel=\"noreferrer noopener\">blood clots<\/a>, but it\u2019s a very, very low risk, so it\u2019s nothing you need to worry about.\u201d) Deregulating birth control means women will be even less informed than they already are about the possible side effects, even the serious or fatal ones, like mental health issues and breast cancer.&nbsp;<\/p>\n\n\n\n<p>In addition to the risks and side effects women will experience from OTC birth control use, there are a few indirect, negative implications of making birth control accessible without a physician encounter. In a <a href=\"https:\/\/naturalwomanhood.org\/no-adverse-outcome\/\" target=\"_blank\" rel=\"noreferrer noopener\">round table discussion<\/a> about the adverse outcomes of contraception use, OB\/GYN Dr. Kathleen Raviele remarked, \u201cIt takes the woman away from an encounter with a physician, a nurse practitioner or a PA, and she\u2019s going to be at high risk for having other problems that won\u2019t be picked up.\u201d As one example, office visits for contraceptives are often when women are diagnosed with sexually transmitted infections (STI)s.&nbsp;<\/p>\n\n\n\n<p>While advocates may praise the FDA for approving an over-the-counter birth control pill in the name of women\u2019s reproductive freedom, setting up women to suffer in silence because they\u2019re taking a risky drug without medical guidance is certainly no way to help them.<\/p>\n\n\n\n<p><em>This article was originally published on September 9, 2022.
