However, many cases of HMB are linked to underlying uterine or systemic conditions. In this article, we will discuss the diagnosis and treatment of the underlying medical conditions that often cause HMB.&nbsp;<\/p>\n\n\n\n<h2 class=\"wp-block-heading\" id=\"h-improving-diagnosis-of-hmb\"><span id=\"improving-diagnosis-of-hmb\">Improving diagnosis of HMB<\/span><\/h2>\n\n\n\n<p>To improve the diagnosis of HMB, in 2011, the International Federation of Gynecology and Obstetrics (FIGO) created a standardized classification system for abnormal uterine bleeding (AUB). FIGO System 1 describes bleeding patterns using four primary descriptors: frequency, duration, regularity, and flow volume. FIGO System 2 <a href=\"https:\/\/pubmed.ncbi.nlm.nih.gov\/21345435\/\" target=\"_blank\" rel=\"noreferrer noopener\">classifies<\/a> possible causes of AUB using the acronym PALM-COEIN [1].<\/p>\n\n\n\n<ul class=\"wp-block-list\">\n<li><strong>PALM<\/strong>: structural causes (Polyp, Adenomyosis, Leiomyoma, Malignancy)<\/li>\n\n\n\n<li><strong>COEI<\/strong>: nonstructural causes (Coagulopathy, Ovulatory dysfunction, Endometrial, Iatrogenic)&nbsp;<\/li>\n\n\n\n<li><strong>N<\/strong>: not yet classified<\/li>\n<\/ul>\n\n\n\n<h3 class=\"wp-block-heading\" id=\"h-structural-causes-of-hmb-palm\"><span id=\"structural-causes-of-hmb-palm\">Structural causes of HMB (PALM)<\/span><\/h3>\n\n\n\n<ul class=\"wp-block-list\">\n<li><strong>Polyp (P):<\/strong> Small overgrowths of endometrial tissue. While often asymptomatic, they can cause irregular or heavy bleeding, particularly in women over 40. A <a href=\"https:\/\/pmc.ncbi.nlm.nih.gov\/articles\/PMC5779569\/\" target=\"_blank\" rel=\"noreferrer noopener\">1988 study<\/a> found that 28% of women with abnormal uterine bleeding had endometrial polyps, most commonly between ages 40 and 50 [2].<\/li>\n\n\n\n<li><a href=\"https:\/\/naturalwomanhood.org\/what-causes-painful-periods\/\" target=\"_blank\" rel=\"noreferrer noopener\"><strong>Adenomyosis<\/strong><\/a><strong> (A):<\/strong> Endometrial tissue invading the muscular wall of the uterus. HMB is the most common symptom of adenomyosis, although roughly one-third of women with the condition may have no symptoms. The exact mechanism behind adenomyosis isn\u2019t fully understood, but impaired uterine contractility is thought to play a role [2].<\/li>\n\n\n\n<li><strong>Leiomyoma (L) \/ <\/strong><a href=\"https:\/\/naturalwomanhood.org\/symptoms-of-uterine-fibroids-treatments-beyond-birth-control\/\" target=\"_blank\" rel=\"noreferrer noopener\"><strong>Fibroids<\/strong><\/a><strong>:<\/strong> Non-cancerous tumors that grow in the muscular wall of the uterus. A 2016 study found that 46% of women with fibroids experienced excessive bleeding, compared to 28% of those without. Fibroids are especially common in black women, and all women in their 40s and perimenopausal years, when progesterone levels drop and estrogen dominates [2].&nbsp;<\/li>\n\n\n\n<li><strong>Malignancy and hyperplasia (M):<\/strong> Although less common, <a href=\"https:\/\/naturalwomanhood.org\/what-could-go-wrong-with-your-uterus\/\" target=\"_blank\" rel=\"noreferrer noopener\">cancers of the uterus<\/a> or <a href=\"https:\/\/naturalwomanhood.org\/birth-control-cervical-cancer\/\" target=\"_blank\" rel=\"noreferrer noopener\">cervix<\/a> can cause abnormal or heavy bleeding [2].<\/li>\n<\/ul>\n\n\n\n<h3 class=\"wp-block-heading\" id=\"h-non-structural-causes-of-hmb-coein\"><span id=\"non-structural-causes-of-hmb-coein\">Non-structural causes of HMB (COEIN)<\/span><\/h3>\n\n\n\n<ul class=\"wp-block-list\">\n<li><strong>Coagulopathy (C):<\/strong> Blood clotting issues, such as <a href=\"https:\/\/pubmed.ncbi.nlm.nih.gov\/15198765\/\" target=\"_blank\" rel=\"noreferrer noopener\">von Willebrand disease<\/a>, can prevent the body from properly stopping menstrual bleeding. Some estimates suggest that up to 13% of women with menorrhagia have an undiagnosed bleeding disorder [3].<\/li>\n\n\n\n<li><strong>Ovulatory dysfunction (O):<\/strong> Common in conditions like <a href=\"https:\/\/naturalwomanhood.org\/category\/reproductive-and-menstrual-disorders\/ovarian-cysts-pcos\/\" target=\"_blank\" rel=\"noreferrer noopener\">polycystic ovary syndrome (PCOS)<\/a>, where infrequent ovulation leads to prolonged estrogen exposure without progesterone. This causes excessive endometrial buildup and heavier bleeding. Elevated androgens in some types of PCOS are converted into estrone, a potent form of estrogen that further thickens the lining [2].<\/li>\n\n\n\n<li><strong>Endometrial (E):<\/strong> Inflammation or infection of the uterine lining (<a href=\"https:\/\/naturalwomanhood.org\/could-chronic-endometritis-be-causing-your-infertility\/\" target=\"_blank\" rel=\"noreferrer noopener\">endometritis<\/a>) can cause irregular, prolonged, or heavy bleeding [2].<\/li>\n\n\n\n<li><strong>Iatrogenic (I):<\/strong> <a href=\"https:\/\/www.mayoclinic.org\/diseases-conditions\/menorrhagia\/symptoms-causes\/syc-20352829\" target=\"_blank\" rel=\"noreferrer noopener\">Certain medications<\/a>\u2014such as blood thinners, tamoxifen, and some forms of hormonal contraception\u2014can trigger or worsen heavy bleeding.<\/li>\n\n\n\n<li><strong>Not otherwise classified (N):<\/strong> Includes causes still being researched, such as the interplay between gut microbiome health and abnormal bleeding patterns.<\/li>\n<\/ul>\n\n\n\n<h2 class=\"wp-block-heading\" id=\"h-how-are-heavy-periods-diagnosed-and-treated\"><span id=\"how-are-heavy-periods-diagnosed-and-treated\">How are heavy periods diagnosed and treated?<\/span><\/h2>\n\n\n\n<p>Treatment of HMB varies depending on the cause.
