One recommendation almost every healthcare professional will give you is to start taking a prenatal vitamin (if you weren\u2019t taking one already!). But are all prenatal vitamins created equal? Can you choose any prenatal off the shelf at your local pharmacy and expect to receive the same benefits?\u00a0<\/p>\n\n\n\n<p>And\u2014even more importantly\u2014<em>why <\/em>should you take a prenatal vitamin in the first place?&nbsp;<\/p>\n\n\n\n<h2 class=\"wp-block-heading\" id=\"h-why-prenatal-vitamins-are-recommended-during-pregnancy\"><span id=\"why-prenatal-vitamins-are-recommended-during-pregnancy\">Why prenatal vitamins are recommended during pregnancy<\/span><\/h2>\n\n\n\n<p>The most basic reason pretty much every healthcare professional <a href=\"https:\/\/www.mayoclinic.org\/healthy-lifestyle\/pregnancy-week-by-week\/in-depth\/prenatal-vitamins\/art-20046945#:~:text=Beyond%20checking%20for%20folic%20acid,B%20vitamins%2C%20zinc%20and%20iodine.\" target=\"_blank\" rel=\"noreferrer noopener\">recommends<\/a> you take a prenatal vitamin is because your nutrient needs during pregnancy dramatically increase, and it can sometimes be difficult to meet those needs from diet alone. One of the most researched and discussed nutritional deficiencies during pregnancy is <a href=\"https:\/\/americanpregnancy.org\/getting-pregnant\/infertility\/folate-vs-folic-acid\/\" target=\"_blank\" rel=\"noreferrer noopener\">folate or folic acid<\/a>, which is crucial in preventing neural tube defects, heart defects, cleft palate, and cleft lip.&nbsp;<\/p>\n\n\n\n<p>It\u2019s actually vital to take folate <em>before <\/em>you\u2019re pregnant, as the defects that can be caused by a folate deficiency occur <a href=\"https:\/\/www.uclahealth.org\/news\/article\/why-every-woman-needs-folic-acid\" target=\"_blank\" rel=\"noreferrer noopener\">within the first few weeks<\/a> of pregnancy (often before you even know for sure that you\u2019re pregnant). Because of this, folate is one of the most vital nutrients during pregnancy and is important to take pre-conception as well. Folate (also known as vitamin B9) is naturally <a href=\"https:\/\/lilynicholsrdn.com\/folate-pregnancy-folic-acid-mthfr\/\" target=\"_blank\" rel=\"noreferrer noopener\">found<\/a> in foods like liver, egg yolks, legumes, and leafy greens, but it is typically found in prenatal vitamins in its synthetic form, folic acid.&nbsp;<\/p>\n\n\n\n<h2 class=\"wp-block-heading\" id=\"h-a-note-on-folic-acid-versus-methylated-folate-nbsp\"><span id=\"a-note-on-folic-acid-versus-methylated-folate\">A note on folic acid versus methylated folate&nbsp;<\/span><\/h2>\n\n\n\n<p>One of the more confusing distinctions between different prenatal vitamins is whether they contain the synthetic folic acid or another form of folate known as<a href=\"https:\/\/ods.od.nih.gov\/factsheets\/Folate-HealthProfessional\/\" target=\"_blank\" rel=\"noreferrer noopener\"> L-methylfolate<\/a>. Folic acid is not just commonly found in prenatal vitamins, it\u2019s also added to commercially-made fortified grain <a href=\"https:\/\/www.fda.gov\/food\/nutrition-facts-label\/folate-and-folic-acid-nutrition-and-supplement-facts-labels#:~:text=Foods%20that%20are%20fortified%20with,found%20in%20certain%20dietary%20supplements.\" target=\"_blank\" rel=\"noreferrer noopener\">products<\/a> like cereals, bread, tortillas, flour, and pasta. The majority of prenatal vitamins on the market use folic acid, but a growing number of brands are <a href=\"https:\/\/thisisneeded.com\/blogs\/the-science-of-nutrition\/folate-vs-folic-acid-what-s-the-difference-and-why-does-it-matter?utm_source=klaviyo&amp;utm_medium=email&amp;utm_campaign=1.28.25_ActiveSubs_OptifolinComms&amp;utm_id=01JJQJF9MTVEN2MF27Z13VBK37&amp;type=campaign&amp;_kx=GUzukX5Gg6R64O89fhV2hVNTVfzgfurh5n6Xgcp09qk.LDr9u9\" target=\"_blank\" rel=\"noreferrer noopener\">making the switch<\/a> to methylated folate (which will probably appear as L-methylfolate on a supplement label).&nbsp;<\/p>\n\n\n\n<p>The main difference between these two forms of folate is that while <em>both<\/em> need to undergo a conversion process to make them bioavailable for our bodies, folic acid is <a href=\"https:\/\/pmc.ncbi.nlm.nih.gov\/articles\/PMC9380836\/\" target=\"_blank\" rel=\"noreferrer noopener\"><em>more difficult<\/em><\/a> to convert if you have a genetic mutation called the MTHFR mutation [1].&nbsp;<\/p>\n\n\n\n<p>MTHFR mutations occur in around <a href=\"https:\/\/www.racgp.org.au\/afp\/2016\/april\/mthfr-genetic-testing-controversy-and-clinical-imp\" target=\"_blank\" rel=\"noreferrer noopener\">60-70%<\/a> of the general population, and essentially make it so your body has difficulty processing and utilizing folate. If you have this genetic mutation, you might have difficulty absorbing much of the folic acid you consume in either fortified grain products or in a prenatal vitamin. Luckily, this is <a href=\"https:\/\/pmc.ncbi.nlm.nih.gov\/articles\/PMC7564482\/\" target=\"_blank\" rel=\"noreferrer noopener\">bypassed<\/a> by the usage of L-methylfolate instead [2].&nbsp;<\/p>\n\n\n\n<h3 class=\"wp-block-heading\" id=\"h-is-it-a-problem-to-have-large-quantities-of-unmetabolized-folic-acid-in-the-body\"><span id=\"is-it-a-problem-to-have-large-quantities-of-unmetabolized-folic-acid-in-the-body\">Is it a problem to have large quantities of unmetabolized folic acid in the body?<\/span><\/h3>\n\n\n\n<p>Those with the MTHFR mutation risk having <a href=\"https:\/\/pmc.ncbi.nlm.nih.gov\/articles\/PMC3317000\/\" target=\"_blank\" rel=\"noreferrer noopener\">unmetabolized folic acid<\/a> if they consume folic acid in large quantities either through supplementation or fortified foods [3]. Unmetabolized folic acid is quite <a href=\"https:\/\/pmc.ncbi.nlm.nih.gov\/articles\/PMC4336532\/\" target=\"_blank\" rel=\"noreferrer noopener\">common<\/a> in Americans (largely due to the number of commercial foods that have folic acid added to them) [4]. To date, research is conflicting regarding whether high levels of unmetabolized folic acid in the body have <a href=\"https:\/\/pmc.ncbi.nlm.nih.gov\/articles\/PMC9395688\/#:~:text=High%20folate\/UMFA%20has%20repeatedly,mechanisms%20of%20action%20(8).\" target=\"_blank\" rel=\"noreferrer noopener\">adverse effects<\/a> such as increased cancer risk and\/or poorer metabolic health [5].<\/p>\n\n\n\n<p>While folic acid is much more studied than L-methylfolate to date, there are <a href=\"https:\/\/thisisneeded.com\/blogs\/the-science-of-nutrition\/why-acog-and-cdc-continue-to-recommend-folic-acid-over-folate-despite-research-advancements\" target=\"_blank\" rel=\"noreferrer noopener\">no known downsides<\/a> to using L-methylfolate rather than folic acid even if you do <em>not <\/em>have the MTHFR mutation. A 2023 literature review <a href=\"https:\/\/www.degruyter.com\/document\/doi\/10.1515\/jpm-2012-0256\/html\" target=\"_blank\" rel=\"noreferrer noopener\">found<\/a> that supplementation of the more bioavailable L-methylfolate may be a \u201cbetter alternative\u201d to supplementing with synthetic folic acid [6].&nbsp;Of note, proper absorption of L-methylfolate requires adequate amounts of Vitamin B12. For this reason, some L-methylfolate products include Vitamin B12. <\/p>\n\n\n\n<h2 class=\"wp-block-heading\" id=\"h-what-nutrients-should-your-prenatal-vitamin-contain-nbsp\"><span id=\"what-nutrients-should-your-prenatal-vitamin-contain\">What nutrients should your prenatal vitamin contain?&nbsp;<\/span><\/h2>\n\n\n\n<p>All of that being said, when you are shopping for a prenatal vitamin, consider first what form of folate is being used. Methylated folate will most likely appear as L-methylfolate but it could also be called 5-methyltetrahydrofolate. Vitamins that use this form of folate may be more effective (especially if you\u2019re in the majority camp of people with the MTHFR mutation) than folic acid.&nbsp;<\/p>\n\n\n\n<p>Besides folate, you\u2019ll also want to look for the other heavy hitters\u2014nutrients that are both important in pregnancy and in which pregnant women are commonly deficient. These include vitamin D, vitamin E, iron, zinc, and magnesium (see <em>Real Food for Pregnancy, <\/em>pg. 95). Other <a href=\"https:\/\/www.mayoclinic.org\/healthy-lifestyle\/pregnancy-week-by-week\/in-depth\/prenatal-vitamins\/art-20046945\" target=\"_blank\" rel=\"noreferrer noopener\">important nutrients<\/a> include calcium, vitamin C, vitamin A, B vitamins (including vitamin B6, which is helpful for <a href=\"https:\/\/naturalwomanhood.org\/remedies-morning-sickness\/\" target=\"_blank\" rel=\"noreferrer noopener\">morning sickness<\/a>!), zinc, and iodine. <a href=\"https:\/\/pmc.ncbi.nlm.nih.gov\/articles\/PMC9275129\/\" target=\"_blank\" rel=\"noreferrer noopener\">This 2022 literature review<\/a> sums up the nutrient deficiencies associated with the most common pregnancy complications [7].&nbsp;<\/p>\n\n\n\n<h3 class=\"wp-block-heading\" id=\"h-should-your-prenatal-contain-iron\"><span id=\"should-your-prenatal-contain-iron\">Should your prenatal contain iron?<\/span><\/h3>\n\n\n\n<p>Another important nutrient for pregnancy is iron, as anemia (iron deficiency) is <a href=\"https:\/\/www.mayoclinic.org\/healthy-lifestyle\/pregnancy-week-by-week\/in-depth\/anemia-during-pregnancy\/art-20114455\" target=\"_blank\" rel=\"noreferrer noopener\">quite common<\/a> in pregnancy due to the fact that a pregnant woman\u2019s body has double the volume of blood that it normally has. This makes it difficult to make enough red blood cells without an increased amount of iron, causing anemia. That being said, iron supplements (whether taken on their own or within a prenatal) can oftentimes cause nausea or GI discomfort as a <a href=\"https:\/\/www.ncbi.nlm.nih.gov\/books\/NBK557376\/\" target=\"_blank\" rel=\"noreferrer noopener\">side effect<\/a>, which is the last thing you want if you\u2019re already struggling with morning sickness. Some prenatal brands do not include iron due to consumer demand for a supplement that does not make you nauseous, while others don\u2019t include iron because supplementation needs may vary significantly from one woman to the next.&nbsp;<\/p>\n\n\n\n<p>Luckily, most medical professionals will routinely check iron levels throughout pregnancy so you know if you are becoming anemic or not. Personally, I take a prenatal without iron because I do experience nausea when I take an iron supplement, but I make sure my doctor knows this and checks my iron a few times throughout pregnancy. If my iron levels are low, I increase natural <a href=\"https:\/\/ods.od.nih.gov\/factsheets\/Iron-HealthProfessional\/#h3\" target=\"_blank\" rel=\"noreferrer noopener\">sources<\/a> of iron in my diet like red meat, dark leafy greens, and cooking in a cast iron pan.<\/p>\n\n\n\n<h2 class=\"wp-block-heading\" id=\"h-finding-a-quality-prenatal\"><span id=\"finding-a-quality-prenatal\">Finding a quality prenatal<\/span><\/h2>\n\n\n\n<p>Another factor to consider when shopping for a prenatal is that while all of them will list the Percent Daily Value for each nutrient and oftentimes include \u201c100% daily value\u201d for each vitamin and mineral, the recommended dietary allowance (RDA)<em> might not actually be enough <\/em>for optimal nutrition. According to prenatal dietician and author of <a href=\"https:\/\/naturalwomanhood.org\/natural-womanhood-book-review-real-food-for-pregnancy\/\" target=\"_blank\" rel=\"noreferrer noopener\"><em>Real Food for Pregnancy<\/em><\/a><em>, <\/em>Lily Nichols:&nbsp;<\/p>\n\n\n\n<p>\u201cSome prenatal vitamins are less comprehensive than they seem, either lacking certain vitamins entirely or not including adequate amounts. It\u2019s important to keep in mind that most recommended dietary allowance (RDAs) for nutrients were set using data from adult men and adjusted via complex estimates to meet the needs of pregnancy. Also, they are set as a level to prevent severe deficiency, but not necessarily at a level for optimal nutrition.\u201d (<em>Real Food for Pregnancy, <\/em>pg. 96).<\/p>\n\n\n\n<h3 class=\"wp-block-heading\" id=\"h-prenatal-vitamins-aren-t-a-one-stop-shop-for-nutritional-health-during-pregnancy\"><span id=\"prenatal-vitamins-arent-a-one-stop-shop-for-nutritional-health-during-pregnancy\">Prenatal vitamins aren&#8217;t a one-stop shop for nutritional health during pregnancy<\/span><\/h3>\n\n\n\n<p>Due to this, Nichols says pregnant women should think less of prenatal vitamins as their one-stop shop to ensure nutritional health during pregnancy, and more as an insurance policy to cover any gaps in their diet. She also cites a number of studies that show how the current RDA during pregnancy might be drastically lower than what it should be for certain nutrients\u2014including <a href=\"https:\/\/pubmed.ncbi.nlm.nih.gov\/25995278\/\" target=\"_blank\" rel=\"noreferrer noopener\">vitamin B12,<\/a> <a href=\"https:\/\/faseb.onlinelibrary.wiley.com\/doi\/abs\/10.1096\/fasebj.29.1_supplement.919.4\" target=\"_blank\" rel=\"noreferrer noopener\">vitamin B6<\/a>, and <a href=\"https:\/\/pubmed.ncbi.nlm.nih.gov\/17164508\/\" target=\"_blank\" rel=\"noreferrer noopener\">vitamin D<\/a> [9][10][11]. There are also other nutrients like <a href=\"https:\/\/lilynicholsrdn.com\/choline-pregnancy-folates-cousin\/\" target=\"_blank\" rel=\"noreferrer noopener\">choline<\/a> that are often absent altogether from prenatal vitamins, largely because they have only been studied in the past twenty years or so.&nbsp;<\/p>\n\n\n\n<p>Because many prenatal vitamins on the market are <a href=\"https:\/\/lilynicholsrdn.com\/prenatal-guidelines-updated\/\" target=\"_blank\" rel=\"noreferrer noopener\">largely insufficient<\/a>, Nichols says, we need to use a critical eye when choosing a prenatal. Luckily, she offers a <a href=\"https:\/\/realfoodforpregnancy.com\/wp-content\/uploads\/2024\/09\/Prenatal-Vitamins-Real-Food-for-Pregnancy-LilyNicholsRDN.pdf\" target=\"_blank\" rel=\"noreferrer noopener\">free guide<\/a> to prenatal brands that are comprehensive in their nutrient choices\u2014including the most bioavailable forms (such as L-methylfolate instead of folic acid) of vitamins and minerals and have evidence-based doses of the nutrients they include. Some of the brands she recommends include <a href=\"https:\/\/fullwellfertility.com\/products\/fullwell-prenatal-multivitamin\" target=\"_blank\" rel=\"noreferrer noopener\">Full Well<\/a>, <a href=\"https:\/\/www.seekinghealth.com\/collections\/best-sellers\/products\/prenatal-essentials\">Seeking Health<\/a>, <a href=\"https:\/\/www.thorne.com\/products\/dp\/basic-prenatal?gad_source=1&amp;gclid=CjwKCAiA5pq-BhBuEiwAvkzVZbuL-Za38J87InlQ38dIsUYWLHQZCmoo39DZs4VkxlSBK8vI5C7KnRoCshkQAvD_BwE\" target=\"_blank\" rel=\"noreferrer noopener\">Thorne<\/a>, and <a href=\"https:\/\/www.smartypantsvitamins.com\/search?q=prenatal&amp;options%5Bprefix%5D=last\" target=\"_blank\" rel=\"noreferrer noopener\">Smarty Pants<\/a> gummies (although she only recommends gummies for women whose nausea prevents them from taking regular prenatal vitamins). <a href=\"https:\/\/www.kuhar.com\/shop\/contents\/en-us\/p117.html\" target=\"_blank\" rel=\"noreferrer noopener\">Vita Prenatal Formula Capsules<\/a>, developed with <a href=\"https:\/\/naturalwomanhood.org\/remembering-marilyn-shannon-fertility-cycles-and-nutrition-2020\/\" target=\"_blank\" rel=\"noreferrer noopener\">nutritionist Marilyn Shannon<\/a> (author of <em>Fertility, Cycles, &amp; Nutrition<\/em>), are another good option.&nbsp;&nbsp;<\/p>\n\n\n\n<h2 class=\"wp-block-heading\" id=\"h-the-bottom-line-on-choosing-a-quality-prenatal-vitamin\"><span id=\"the-bottom-line-on-choosing-a-quality-prenatal-vitamin\">The bottom line on choosing a quality prenatal vitamin<\/span><\/h2>\n\n\n\n<p>While the sheer number of prenatal vitamins on the market might seem daunting, some are certainly better than others. But no matter which prenatal you choose, it&#8217;s important to view it as a means of filling the nutritional gaps you have, rather than an end-all-be-all to pregnancy health. Having good <a href=\"https:\/\/naturalwomanhood.org\/is-your-pregnancy-diet-up-to-date-with-science\/\" target=\"_blank\" rel=\"noreferrer noopener\">nutrition<\/a> <em>alongside<\/em> your prenatal vitamin is the best way to set yourself up for success during pregnancy.&nbsp;<\/p>\n\n\n\n<h4 class=\"wp-block-heading\" id=\"h-references-nbsp\"><span id=\"references\">References:&nbsp;<\/span><\/h4>\n\n\n\n[1] Carboni L.
