{"id":22126,"date":"2025-01-28T08:00:00","date_gmt":"2025-01-28T14:00:00","guid":{"rendered":"https:\/\/naturalwomanhood.org\/?p=22126"},"modified":"2025-07-14T09:37:02","modified_gmt":"2025-07-14T14:37:02","slug":"frottis-cervical-douloureux","status":"publish","type":"post","link":"https:\/\/naturalwomanhood.org\/fr\/painful-pap-smear\/","title":{"rendered":"La douleur emp\u00eache les femmes de se soumettre r\u00e9guli\u00e8rement \u00e0 un test de Papanicolaou"},"content":{"rendered":"\n<p>We previously published a primer on what to expect during a pap smear (which you can read <a href=\"https:\/\/naturalwomanhood.org\/pap-smear-primer\/\" target=\"_blank\" rel=\"noreferrer noopener\">here<\/a>), which did not adequately address the pain many women experience during this procedure. In this companion piece, let\u2019s talk about why pap smears are so painful for some women and, more importantly, what can be done to decrease pap smear pain.&nbsp;<\/p>\n\n\n\n<p>By way of a refresher, pap smears are valuable because they screen for cervical cancer. They can also detect human papilloma virus (HPV) infection, which causes the vast majority of cervical cancer cases. But if women skip regular pap smears, which are recommended every 3 years for the average woman, they miss out on a key window to detect HPV infection before it progresses to precancerous and then cancerous cells. This matters especially for women on hormonal birth control, since prolonged use <a href=\"https:\/\/naturalwomanhood.org\/birth-control-cervical-cancer\/\" target=\"_blank\" rel=\"noreferrer noopener\">increases cervical cancer risk<\/a>.&nbsp;<\/p>\n\n\n\n<h2 class=\"wp-block-heading\" id=\"h-when-pap-smears-hurt-women-stop-getting-them\"><span id=\"when-pap-smears-hurt-women-stop-getting-them\">When pap smears hurt, women stop getting them<\/span><\/h2>\n\n\n\n<p>Unsurprisingly, a <a href=\"https:\/\/pubmed.ncbi.nlm.nih.gov\/15917039\/\" target=\"_blank\" rel=\"noreferrer noopener\">2005 study<\/a> of African American women found that painful pap smears don\u2019t make for willing repeat customers. What <em>was<\/em> surprising, however, was the study\u2019s finding that the usual barriers to screening (including lower education and socioeconomic status, poor access to care, knowledge of and exposure to known risk factors of invasive cervical cancer, cancer fatalism, and perceived racism) were not enough to keep African American women away from pap smears. Instead, it was pain (especially combined with difficulty paying) that was the strongest predictor of women being unwilling to undergo future pap smears. 20 years later, the issue of payment is now a moot point, since pap smears have to be covered at 100% by insurance as preventative services\u2013yet the pain (and fear of pain) during pap smears still abides.&nbsp;&nbsp;<\/p>\n\n\n\n<h2 class=\"wp-block-heading\" id=\"h-why-are-pap-smears-painful\"><span id=\"why-are-pap-smears-painful\">Why are pap smears painful?<\/span><\/h2>\n\n\n\n<p>The reasons why pap smears are painful vary, but all are fairly intuitive. As any pelvic floor therapist will tell you, women commonly <a href=\"https:\/\/naturalwomanhood.org\/pelvic-floor-physical-therapy\/\" target=\"_blank\" rel=\"noreferrer noopener\">hold stress in their pelvic floor<\/a>. Anything that increases stress may cause women to clench or tense up their bodies (including their pelvic floor), causing their vagina to resist the pressure of the speculum (the device used during a pap smear to open the vaginal walls, making the cervix more accessible to the healthcare professional). Unsurprisingly, this can result in feelings of pain.\u00a0<\/p>\n\n\n\n<p>Other reasons women may experience pain during a pap smear may include (but are not limited to):<\/p>\n\n\n\n<ul class=\"wp-block-list\">\n<li>Feelings of vulnerability due to the way a woman is positioned for pap smears (which may be amplified if the healthcare professional is male);\u00a0<\/li>\n\n\n\n<li>History of sexual trauma, which may induce flashbacks, other PTSD symptoms, or anxiety before or during a pap smear;<\/li>\n\n\n\n<li>Rough or clumsy technique due to an inexperienced healthcare professional;<\/li>\n\n\n\n<li>A standard size speculum, which may be more painful for a woman who has never had sex before.<\/li>\n<\/ul>\n\n\n\n<h2 class=\"wp-block-heading\" id=\"h-how-can-we-make-pap-smears-less-painful\"><span id=\"how-can-we-make-pap-smears-less-painful\">How can we make pap smears less painful?<\/span><\/h2>\n\n\n\n<h3 class=\"wp-block-heading\" id=\"h-include-a-nonpainful-step\"><span id=\"include-a-nonpainful-step\">Include a \u201cnonpainful step\u201d<\/span><\/h3>\n\n\n\n<p>Some of the solutions for painful pap smears are equally intuitive. A fascinating <a href=\"https:\/\/pubmed.ncbi.nlm.nih.gov\/37043729\/\" target=\"_blank\" rel=\"noreferrer noopener\">2023 study <\/a>reported that when a \u201cnonpainful step\u201d was added to the end of a pap smear, women were significantly <em>less<\/em> likely to recall the pap smear as painful 5 minutes afterward, and significantly <em>more<\/em> likely to follow guidelines for repeat pap smears later on. What was the \u201cnonpainful step?\u201d A 15-second lag time was introduced between the rotation of the speculum (after the cervical cells specimen was collected) and its removal from the woman\u2019s body. That one simple, brief pause made a real difference.&nbsp;<\/p>\n\n\n\n<h3 class=\"wp-block-heading\" id=\"h-use-a-smaller-speculum\"><span id=\"use-a-smaller-speculum\">Use a smaller speculum<\/span><\/h3>\n\n\n\n<p>Women can also request a pediatric-size speculum be used. They can also request use of a lubricant on the speculum (though one wonders why this has to be requested, and isn\u2019t already the norm!).&nbsp;<\/p>\n\n\n\n<h3 class=\"wp-block-heading\" id=\"h-change-positions\"><span id=\"change-positions\">Change positions<\/span><\/h3>\n\n\n\n<p>According to a <a href=\"https:\/\/www.medicalnewstoday.com\/articles\/anxious-about-cervical-screening-what-you-need-to-know#How-much-discomfort-to-expect\"><em>Medical News Today<\/em> article<\/a>, trying a position other than the standard laying-on-back-with-knees-splayed-open posture may be helpful for alleviating pain during a pap smear.&nbsp; Dr. Zahra Ameen, a UK-based OB\/GYN, told <em>Medical News Today<\/em>, \u201cSome people find that different positions reduce discomfort, such as putting your hands under your bottom to tilt your pelvis up and allow [your] legs to gently open and relax, allowing a more comfortable clinical examination.\u201d<\/p>\n\n\n\n<h3 class=\"wp-block-heading\" id=\"h-insert-the-speculum-yourself\"><span id=\"insert-the-speculum-yourself\">Insert the speculum yourself<\/span><\/h3>\n\n\n\n<p>Dr. Apurva Shah, a Massachusetts-based OB\/GYN and member of the <a href=\"https:\/\/naturalwomanhood.org\/femtech-device\/\" target=\"_blank\" rel=\"noreferrer noopener\">Mira<\/a> medical board further recommended asking to insert the speculum yourself, saying \u201cIf you feel more comfortable inserting the speculum yourself, go ahead and ask to do that. You would be surprised how much easier it is for you.\u201d&nbsp;<\/p>\n\n\n\n<h2 class=\"wp-block-heading\" id=\"h-the-1-way-to-prevent-painful-pap-smears\"><span id=\"the-1-way-to-prevent-painful-pap-smears\">The #1 way to prevent painful pap smears<\/span><\/h2>\n\n\n\n<p>The single-most impactful way to reduce pain, Dr. Shah noted, is \u201cFeeling in control and seeing a provider you can trust\u2026\u201d\u00a0<\/p>\n\n\n\n<p>\u201cOnce you are in a position of trust,\u201d he continued, \u201cit really impacts how your body and your pelvic floor responds to an exam.\u201d He added, \u201cThe more relaxed and in control you feel, the less likely it is to have discomfort during the screening\u201d since \u201cthe pelvic floor which would otherwise resist the speculum will relax and allow for a much gentler exam and an overall better experience.\u201d<\/p>\n\n\n\n<h2 class=\"wp-block-heading\" id=\"h-for-women-who-have-experienced-trauma\"><span id=\"for-women-who-have-experienced-trauma\">For women who have experienced trauma<\/span><\/h2>\n\n\n\n<p>For some women, past trauma (sexual, medical, or otherwise) can seriously impact their experience of pelvic exams or pap smears. It is important for these women to be aware of their rights and options so they can advocate for their needs and avoid reinforcing a past traumatic experience. Additionally, healthcare professionals need to be prepared and equipped to respond to clients&#8217; trauma responses.<\/p>\n\n\n\n<p>Dr. Pebble Kranz, a sexual medicine specialist, recently <a href=\"https:\/\/www.medscape.com\/viewarticle\/consent-compassion-control-trauma-informed-approach-pelvic-2025a1000f4o\" target=\"_blank\" rel=\"noreferrer noopener\">wrote about her trauma-informed approach to pelvic exams<\/a> for <em>Medscape<\/em>. In her practice, Dr. Kranz seeks to equip patients with self-advocacy tools, recognize and respond to subtle signs of discomfort, and \u201cacknowledge the parts of [the patients] that seek healing alongside those that deeply fear vulnerability.\u201d This approach has allowed her to help her patients pursue the healthcare they need and desire. By taking time to learn about each patient&#8217;s history, concerns, and questions, Dr. Kranz believes healthcare professionals can meet women where they are, and minimize future negative experiences.<\/p>\n\n\n\n<p>She urges healthcare professionals to <em>slow down<\/em>: paying attention to the patient\u2019s bodily cues (not just their words), empowering patients to lead the process, creating an environment (WITH the patient) that feels safe, and explaining the necessity and rationale of exams to patients.\u00a0<\/p>\n\n\n\n<h2 class=\"wp-block-heading\" id=\"h-the-bottom-line-on-painful-pap-smears\"><span id=\"the-bottom-line-on-painful-pap-smears\">The bottom line on painful pap smears<\/span><\/h2>\n\n\n\n<p>If you have had a painful pap smear in the past, or have never had one and fear that a pap smear may be painful for you, talk to your healthcare professional about your options for easing the experience.
