Though it\u2019s been around for a long time, \u201cthe shot\u201d remains a much less popular option among US women than oral contraceptives and IUDs. According to survey data from the CDC, about 1 in 4 sexually active American women have <em>tried<\/em> the Depo shot. But in a <a href=\"https:\/\/www.cdc.gov\/nchs\/data\/nhsr\/nhsr195.pdf\" target=\"_blank\" rel=\"noreferrer noopener\">survey<\/a> conducted between 2017 and 2019, only about 2.4% of sexually active women reported currently using it. One cause for this disparity may be the FDA\u2019s black box warning against using the shot for more than two years, due to evidence that it significantly reduces bone mineral density (more on this below!).<\/p>\n\n\n\n<p>If you\u2019re considering the birth control shot for contraception or to regulate your menstrual flow, here\u2019s what you need to know.&nbsp;<\/p>\n\n\n\n<h2 class=\"wp-block-heading\" id=\"h-depo-provera-is-the-only-birth-control-shot-approved-in-the-u-s-nbsp\"><span id=\"depo-provera-is-the-only-birth-control-shot-approved-in-the-u-s\">Depo-Provera is the only birth control shot approved in the U.S.&nbsp;<\/span><\/h2>\n\n\n\n<p>Depo-Provera (produced by Pfizer) is the brand name of the <a href=\"https:\/\/www.mayoclinic.org\/tests-procedures\/depo-provera\/about\/pac-20392204\" target=\"_blank\" rel=\"noreferrer noopener\">drug<\/a> medroxyprogesterone acetate and is the only form of the birth control shot available in the United States. The FDA initially <a href=\"https:\/\/www.healthline.com\/health\/birth-control\/history-of-birth-control#modern-methods:~:text=1960%3A%20The%20FDA%20approves%20the%20pill%20for%20the%20purposes%20of%20birth%20control\" target=\"_blank\" rel=\"noreferrer noopener\">rejected<\/a> Depo in 1967 due to safety concerns, though it was used in other countries, especially underdeveloped countries with fewer medical safeguards and less access to healthcare. Depo was finally approved in the U.S. in 1992 [1].&nbsp;<\/p>\n\n\n\n<p>As a <a href=\"https:\/\/naturalwomanhood.org\/difference-between-synthetic-hormones-vs-natural-hormones-bioidentical-birth-control-harmful-safe-affect-body-progesterone-progestin-estrogen-08192018\/\" target=\"_blank\" rel=\"noreferrer noopener\">progestin-only<\/a> form of birth control, Depo-Provera prevents pregnancy by suppressing ovulation and thickening the cervical mucus to prevent sperm from coming into contact with an egg. It\u2019s recommended as a highly effective form of birth control that eliminates the need to take a daily pill <em>and<\/em> does not include estrogen, which can <a href=\"https:\/\/womenandbloodclots.org\/birth-control\/\" target=\"_blank\" rel=\"noreferrer noopener\">cause<\/a> blood clots and carries concerns for breast cancer.&nbsp;<\/p>\n\n\n\n<h2 class=\"wp-block-heading\" id=\"h-depo-provera-is-usually-given-as-a-shot-in-the-arm-or-buttock\"><span id=\"depo-provera-is-usually-given-as-a-shot-in-the-arm-or-buttock\">Depo-Provera is usually given as a shot in the arm or buttock<\/span><\/h2>\n\n\n\n<p>Users of Depo-Provera must visit a health care provider once every three months to receive an injection. A doctor or nurse administers the shot into a large muscle in the upper arm or buttock. If a user waits more than 13 weeks between appointments, the drug may not effectively prevent pregnancy. She will likely be required to take a pregnancy test before receiving her next shot to ensure that she\u2019s not conceived in the interim, though Depo is not known to <a href=\"https:\/\/www.thewomens.org.au\/health-information\/contraception\/depo-provera\" target=\"_blank\" rel=\"noreferrer noopener\">affect an existing pregnancy<\/a>.<\/p>\n\n\n\n<h3 class=\"wp-block-heading\" id=\"h-a-newer-version-can-now-be-self-administered-at-home\"><span id=\"a-newer-version-can-now-be-self-administered-at-home\">A newer version can now be self-administered at home<\/span><\/h3>\n\n\n\n<p>An at-home version of the shot is also available. Known as Depo-SubQ Provera 104, it still requires a prescription, but it contains a lower dosage and the hormones are injected <a href=\"https:\/\/www.nationwidechildrens.org\/family-resources-education\/health-wellness-and-safety-resources\/helping-hands\/birth-control-subcutaneous-shot#:~:text=How%20to%20Give%20the%20Shot\" target=\"_blank\" rel=\"noreferrer noopener\">just under the skin<\/a>, rather than into a muscle. Nationwide Children\u2019s Hospital provides detailed, illustrated instructions on how to self-administer the shot.<\/p>\n\n\n\n<h2 class=\"wp-block-heading\" id=\"h-the-birth-control-shot-may-decrease-menstrual-cramps-and-stop-all-bleeding\"><span id=\"the-birth-control-shot-may-decrease-menstrual-cramps-and-stop-all-bleeding\">The birth control shot may decrease menstrual cramps and stop all bleeding<\/span><\/h2>\n\n\n\n<p>According to the <a href=\"https:\/\/www.mayoclinic.org\/tests-procedures\/depo-provera\/about\/pac-20392204\" target=\"_blank\" rel=\"noreferrer noopener\">Mayo Clinic,<\/a> the birth control shot may decrease menstrual cramps and pain, lower the risk of endometrial cancer, and lighten bleeding episodes (which are often called \u201cperiods,\u201d but are actually <a href=\"https:\/\/naturalwomanhood.org\/no-cycle-on-the-pill\/\" target=\"_blank\" rel=\"noreferrer noopener\"><em>not <\/em>the same<\/a> thing as your period). About half of women who use the birth control shot eventually stop bleeding altogether [1]. Many sources promote absent periods as an added benefit, ignoring the benefits of ovulation (which always precedes menstruation) for <a href=\"https:\/\/naturalwomanhood.org\/topic\/reasons-women-need-periods\/\" target=\"_blank\" rel=\"noreferrer noopener\">women\u2019s health<\/a>. What\u2019s more, women tend to be wary when their periods go haywire (or <a href=\"https:\/\/naturalwomanhood.org\/causes-secondary-amenorrhea\/\" target=\"_blank\" rel=\"noreferrer noopener\">missing altogether<\/a>), and the number one reason women stop using Depo-Provera is \u201cirregular menstrual patterns\u201d [1].<\/p>\n\n\n\n<h2 class=\"wp-block-heading\" id=\"h-weight-gain-and-delayed-return-of-fertility-are-also-common-among-depo-users\"><span id=\"weight-gain-and-delayed-return-of-fertility-are-also-common-among-depo-users\">Weight gain and delayed return of fertility are also common among Depo users<\/span><\/h2>\n\n\n\n<p>Weight gain is the second most common reason that women discontinue using the birth control shot [2]. Weight gain can lead to insulin resistance, a <a href=\"https:\/\/naturalwomanhood.org\/insulin-resistance-pcos-fertility-prediabetes\/\" target=\"_blank\" rel=\"noreferrer noopener\">particular concern<\/a> for women with polycystic ovary syndrome.<\/p>\n\n\n\n<p>Another cause for concern is the delay in return of fertility after stopping the birth control shot. The Mayo Clinic advises readers that it may take 10 months before your period comes back. Remember that a period follows ovulation. Without ovulation, you cannot conceive.&nbsp; Unfortunately, there appears to be a lack of recent scientific interest in the link between Depo-Provera and delayed return of fertility. The most recent major studies on the topic date back to the 1980s [6].&nbsp;<\/p>\n\n\n\n<h2 class=\"wp-block-heading\" id=\"h-brain-tumors-and-breast-cancer-are-also-possible\"><span id=\"brain-tumors-and-breast-cancer-are-also-possible\">Brain tumors and breast cancer are also possible<\/span><\/h2>\n\n\n\n<p>A 2024 French <a href=\"https:\/\/www.bmj.com\/content\/384\/bmj-2023-078078\" target=\"_blank\" rel=\"noreferrer noopener\">study<\/a> published in the <em>BMJ<\/em> found that women who used Depo for longer than one year were at increased risk of developing meningioma, the most common form of brain tumor [7]. While very few Depo users overall developed meningioma (0.05%), this was <em>five times higher<\/em> than the risk for women who didn&#8217;t use progestin-containing birth control (0.01%). <\/p>\n\n\n\n<p>And while the lack of estrogen in Depo (and other forms of progestin-only birth control like O-pill) was hypothesized <em>not<\/em> to increase breast cancer risk like the pill does, the facts tell a different story. A 2023 <a href=\"https:\/\/www.ceu.ox.ac.uk\/news\/any-type-of-hormonal-contraceptive-may-increase-risk-of-breast-cancer\" target=\"_blank\" rel=\"noreferrer noopener\">study<\/a> out of Oxford University<a href=\"https:\/\/journals.plos.org\/plosmedicine\/article?id=10.1371\/journal.pmed.1004188\" target=\"_blank\" rel=\"noreferrer noopener\"> found<\/a> that <em>Depo use increased breast cancer risk just as much as the pill<\/em>, even though it doesn&#8217;t contain estrogen [8]. Both combined oral contraceptives and Depo-Provera use increased women&#8217;s breast cancer risk by 20-30%. In a May 2024 interview for <a href=\"https:\/\/www.npr.org\/sections\/health-shots\/2024\/05\/01\/1248525256\/hormones-menopause-hormone-therapy-hot-flashes\" target=\"_blank\" rel=\"noreferrer noopener\">NPR<\/a>, Northwestern University professor Dr. Lauren Streicher called Depo &#8220;highly problematic&#8221;. She noted that it&#8217;s no longer prescribed for hormone replacement therapy (HRT) to treat menopausal symptoms, in part because of its connection to breast cancer. <\/p>\n\n\n\n<h2 class=\"wp-block-heading\" id=\"h-depo-provera-significantly-increases-likelihood-of-contracting-or-transmitting-hiv\"><span id=\"depo-provera-significantly-increases-likelihood-of-contracting-or-transmitting-hiv\">Depo-Provera significantly increases likelihood of contracting or transmitting HIV<\/span><\/h2>\n\n\n\n<p>Unfortunately, missing periods, weight gain, and delayed return of fertility are far and away the most \u201cinnocuous\u201d side effects of Depo. Multiple studies have found that Depo-Provera actually <a href=\"https:\/\/naturalwomanhood.org\/risk-of-hiv-transmission-depo-provera-birth-control-shot\/\" target=\"_blank\" rel=\"noreferrer noopener\">increases<\/a> the risk of developing and transmitting HIV [2]. This is frequently glossed over by online sources (such as <a href=\"https:\/\/kidshealth.org\/en\/teens\/contraception-depo.html\" target=\"_blank\" rel=\"noreferrer noopener\">this one<\/a> for teens) which simply warn that, like all hormonal contraceptives, the shot does not protect against sexually transmitted diseases and recommend condom use for this purpose. But unlike other forms of hormonal contraception, Depo-Provera <em>does <\/em>raise HIV risk.&nbsp;<\/p>\n\n\n\n<p>A cavalier approach to the Depo-HIV connection is particularly egregious given that Depo is very popular in sub-Saharan Africa, where HIV \u201chas had the greatest impact\u201d [3]. As explained by Dr. Joel Brind in our Depo Safety Watch <a href=\"https:\/\/naturalwomanhood.org\/depo_safety_watch\/\" target=\"_blank\" rel=\"noreferrer noopener\">webinar<\/a>, Depo can \u201cdouble\u2014increase by 50 to 100%\u2014the transmissibility of HIV\u201d [4**].&nbsp;<\/p>\n\n\n\n<h3 class=\"wp-block-heading\" id=\"h-wait-didn-t-the-world-health-organization-say-depo-is-safe-for-all-women-to-use\"><span id=\"wait-didnt-the-world-health-organization-say-depo-is-safe-for-all-women-to-use\">Wait, didn\u2019t the World Health Organization say Depo is safe for all women to use?<\/span><\/h3>\n\n\n\n<p>It\u2019s true that in 2019 the World Health Organization (WHO) changed their <a href=\"https:\/\/www.who.int\/news\/item\/29-08-2019-who-revises-recommendations-on-hormonal-contraceptive-use-for-women-at-high-hiv-risk#:~:text=The%20World%20Health%20Organization%20(WHO,increased%20risk%20of%20HIV%20infection.\" target=\"_blank\" rel=\"noreferrer noopener\">recommendation<\/a> to say that even women at high-risk of HIV could \u201cuse any form of reversible contraception, including progestogen-only injectables [like Depo]\u201d. Natural Womanhood <a href=\"https:\/\/naturalwomanhood.org\/risk-of-hiv-transmission-depo-provera-birth-control-shot\/\" target=\"_blank\" rel=\"noreferrer noopener\">has previously explained<\/a> the deep flaws in the data analysis the WHO cited to justify changing their stance. Even this 2020 <a href=\"https:\/\/www.ncbi.nlm.nih.gov\/pmc\/articles\/PMC7232639\/\" target=\"_blank\" rel=\"noreferrer noopener\">review of literature<\/a> commented that the change \u201cseems ill-considered and unduly hasty\u201d [5].&nbsp;&nbsp;<\/p>\n\n\n\n<h2 class=\"wp-block-heading\" id=\"h-a-black-box-warning-from-the-fda-depo-provera-spells-bone-fracture-disaster-for-teen-girls-and-young-women\"><span id=\"a-black-box-warning-from-the-fda-depo-provera-spells-bone-fracture-disaster-for-teen-girls-and-young-women\">A black box warning from the FDA: Depo-Provera spells (bone fracture) disaster for teen girls and young women<\/span><\/h2>\n\n\n\n<p>The most damning evidence of Depo\u2019s harm to women, and especially to teens and young women, is its detrimental effect on bone mineral density (BMD). In 2004, the FDA added an official black box warning (the most serious level of warning issued by the administration) to the drug\u2019s<a href=\"https:\/\/www.accessdata.fda.gov\/drugsatfda_docs\/label\/2010\/020246s036lbl.pdf\" target=\"_blank\" rel=\"noreferrer noopener\"> label<\/a>: \u201cWomen who use Depo-Provera Contraceptive Injection may lose significant bone mineral density. Bone loss is greater with increasing duration of use and may not be completely reversible.\u201d&nbsp;<\/p>\n\n\n\n<blockquote class=\"wp-block-quote is-layout-flow wp-block-quote-is-layout-flow\">\n<p>\u201cWomen who use Depo-Provera Contraceptive Injection may lose significant bone mineral density. Bone loss is greater with increasing duration of use and may not be completely reversible.\u201d<\/p>\n<cite>Food and Drug Administration <\/cite><\/blockquote>\n\n\n\n<h3 class=\"wp-block-heading\" id=\"h-depo-doesn-t-just-limit-bone-density-growth-it-causes-bone-loss\"><span id=\"depo-doesnt-just-limit-bone-density-growth-it-causes-bone-loss\">Depo doesn\u2019t just limit bone density growth, it causes bone loss<\/span><\/h3>\n\n\n\n<p>All hormonal birth control is <a href=\"https:\/\/naturalwomanhood.org\/reasons-women-need-periods-you-need-a-period-the-role-of-the-menstrual-cycle-in-bone-health-development-2020\/\" target=\"_blank\" rel=\"noreferrer noopener\">disruptive to bone growth<\/a>, which for women is mostly complete by the age of 25. That means the teenage years are crucial for building strong bones and preventing issues like osteoporosis from developing later. The birth control shot <a href=\"https:\/\/www.accessdata.fda.gov\/drugsatfda_docs\/label\/2010\/020246s036lbl.pdf\" target=\"_blank\" rel=\"noreferrer noopener\">stands out<\/a> from other forms of hormonal birth control in that it doesn\u2019t just limit bone growth. It actually <em>decreases<\/em> BMD.<\/p>\n\n\n\n<h3 class=\"wp-block-heading\" id=\"h-young-women-are-especially-vulnerable-to-depo-s-bone-density-loss-effects\"><span id=\"young-women-are-especially-vulnerable-to-depos-bone-density-loss-effects\">Young women are especially vulnerable to Depo\u2019s bone density loss effects<\/span><\/h3>\n\n\n\n<p>The results of the clinical trials for Depo-Provera illustrate this point well. Adult women in the study suffered significant bone loss. Teenage girls experienced<em> even greater loss<\/em> over the same period of time, while their untreated counterparts had greater BMD growth than the women in the adult control group. For teenagers who used the Depo shot for more than two years, their bone mass density <em>had not fully recovered<\/em> five years after they stopped using it.&nbsp;<\/p>\n\n\n\n<p>Even for adolescents who used the Depo shot for less than two years, it took two years after stopping just to return to their baseline bone density. That means, following the FDA-recommended guidelines, girls may lose up to <em>four years<\/em> of crucial bone development.<\/p>\n\n\n\n<h3 class=\"wp-block-heading\" id=\"h-women-who-previously-used-depo-are-at-higher-risk-of-bone-fractures\"><span id=\"women-who-previously-used-depo-are-at-higher-risk-of-bone-fractures\">Women who previously used Depo are at higher risk of bone fractures<\/span><\/h3>\n\n\n\n<p>A follow-up study found that women who had previously used the Depo shot were 40% more likely to have had a<a href=\"https:\/\/www.pfizermedicalinformation.com\/depo-provera\/medguide#:~:text=14.4%20Bone%20Fracture%20Incidence%20in%20Women%20Treated%20with%20Depo%2DProvera%20CI\" target=\"_blank\" rel=\"noreferrer noopener\"> bone fracture<\/a> than those who had used other forms of contraception. Keep in mind that these are not older, postmenopausal women suffering osteoporosis. These are young women, still in their childbearing years.&nbsp;<\/p>\n\n\n\n<p>All of the above speaks plainly to the detrimental effects of the birth control shot, especially for young women.
