{"id":20012,"date":"2024-03-21T13:11:52","date_gmt":"2024-03-21T18:11:52","guid":{"rendered":"https:\/\/naturalwomanhood.org\/?p=20012"},"modified":"2024-05-07T11:17:10","modified_gmt":"2024-05-07T16:17:10","slug":"pourquoi-les-adolescents-utilisent-ils-des-larcs","status":"publish","type":"post","link":"https:\/\/naturalwomanhood.org\/fr\/why-are-teens-using-larcs\/","title":{"rendered":"Pourquoi les adolescents sont-ils plus nombreux que jamais \u00e0 utiliser les LARC ?"},"content":{"rendered":"\n<p>The final line of the worldwide hit <em>The Barbie Movie<\/em> is Barbie exclaiming \u201cI\u2019m here to see my gynecologist!\u201d A trip to her OB\/GYN is her first big task after (spoiler alert!) leaving her life in Barbieland behind to embrace the real world as an adult woman. Was this the writers\u2019 humorous clarification that Barbie was definitely a human, and no longer a toy? While I can\u2019t be sure what the intention was behind this line, to me, it sounded like the message sent to many teens by the culture, and even by their parents and doctors: contraception is a key step to becoming a woman. And increasingly, teens are using LARCs, long-acting reversible contraceptives.<\/p>\n\n\n\n<h2 id=\"what-are-larcs\" class=\"wp-block-heading\">What are LARCs?<\/h2>\n\n\n\n<p>LARCs include intrauterine devices (IUDs) and the arm implant. <a href=\"https:\/\/my.clevelandclinic.org\/health\/treatments\/23269-larc\" target=\"_blank\" rel=\"noreferrer noopener\">Examples include<\/a> Paragard, a copper IUD that lasts up to ten years; hormonal IUDs that work by releasing progestin for 3-7 years; and Nexplanon, which is inserted into the arm and releases progestin for up to 3 years. Unlike the Pill, which needs to be taken by the patient at the same time each day, LARCs require no action by the patient after their insertion. This is why they are often billed as \u201cset it and forget it\u201d forms of birth control.<\/p>\n\n\n\n<h2 id=\"why-are-long-acting-reversible-contraceptives-encouraged-for-teens\" class=\"wp-block-heading\">Why are long-acting reversible contraceptives encouraged for teens?<\/h2>\n\n\n\n<p>While rates of teen sexual activity and teen pregnancy greatly <a href=\"https:\/\/ifstudies.org\/blog\/fewer-american-high-schoolers-having-sex-than-ever-before\" target=\"_blank\" rel=\"noreferrer noopener\">declined<\/a> over the past three decades, the <a href=\"https:\/\/www.cdc.gov\/teenpregnancy\/about\/index.htm\" target=\"_blank\" rel=\"noreferrer noopener\">CDC<\/a> still reports the US teen pregnancy rate to be higher than in other industrialized nations. Some health care providers believe they have a moral duty to make sure their teen patients are on contraception to further curb rates of unintended pregnancy. Under this goal of pregnancy prevention, LARCs hold great appeal for doctors who serve teens.&nbsp;<\/p>\n\n\n\n<h3 id=\"reasons-doctors-encourage-larcs-for-teens\" class=\"wp-block-heading\">Reasons doctors encourage LARCs for teens<\/h3>\n\n\n\n<p>For one, the <a href=\"https:\/\/www.acog.org\/clinical\/clinical-guidance\/committee-opinion\/articles\/2018\/05\/adolescents-and-long-acting-reversible-contraception-implants-and-intrauterine-devices\" target=\"_blank\" rel=\"noreferrer noopener\">American College of Obstetricians and Gynecologists<\/a> (ACOG) lauds the high efficacy and continuation rates that LARCs offer [4]. Another benefit is that LARCs are not subject to user error since they are inserted or implanted in the body by a medical professional (this does not mean there cannot be <a href=\"https:\/\/naturalwomanhood.org\/birth-control-implant-migration\/\" target=\"_blank\" rel=\"noreferrer noopener\">insertion error<\/a>, though). Nor are LARC users subject to missed doses, improper storage (too hot, cold or humid), or poor absorption due to digestive problems.&nbsp;<\/p>\n\n\n\n<p>LARCs also last a long time. Even at the lower end of the range, a contraceptive effect for three years would carry a senior in high school well into young adulthood, when pregnancy is much more common than in adolescence.&nbsp;<\/p>\n\n\n\n<h3 id=\"why-some-parents-want-their-teens-to-use-larcs\" class=\"wp-block-heading\">Why some parents want their teens to use LARCs<\/h3>\n\n\n\n<p>But it\u2019s not just physicians. Many parents also want their teens to use IUDs. When <a href=\"https:\/\/naturalwomanhood.org\/teach-teens-fertility-awareness-healthy-relationships\/\" target=\"_blank\" rel=\"noreferrer noopener\">I taught sex education<\/a> to high school students, the teens invariably told me that their parents had told them nothing about sex or even dating in general. For too many parents, the idea of their children someday having sex is awkward, uncomfortable, or may bring up memories of their own past mistakes or painful experiences. It can be tempting for parents to look to LARCs as a way to prevent their teen from becoming pregnant without needing to have any difficult or uncomfortable conversations with them about sex.<\/p>\n\n\n\n<h2 id=\"what-are-the-risks-of-larc-use-in-teens-especially\" class=\"wp-block-heading\">What are the risks of LARC use in teens especially?<\/h2>\n\n\n\n<p>A quick internet search into \u201care IUDs safe?\u201d will be answered with a resounding \u201cyes!\u201d However, while major medical organizations are quick to sidestep concerns, individuals who have been harmed by LARCs strive to raise awareness about dangerous potential side effects such as severe <a href=\"https:\/\/naturalwomanhood.org\/anxiety-and-other-warning-signs-side-effects-of-iud\/\" target=\"_blank\" rel=\"noreferrer noopener\">anxiety<\/a>, <a href=\"https:\/\/naturalwomanhood.org\/removing-my-copper-iud-removed-my-depression-and-saved-my-life-2020\/\" target=\"_blank\" rel=\"noreferrer noopener\">depression<\/a>, symptoms of <a href=\"https:\/\/naturalwomanhood.org\/surviving-a-toxic-reaction-to-the-copper-iud-personal-story\/\" target=\"_blank\" rel=\"noreferrer noopener\">copper toxicity<\/a> (specific to the copper IUD), and implant <a href=\"https:\/\/naturalwomanhood.org\/birth-control-implant-may-not-stay-in-your-arm-nexplanon-side-effects-1-2019\/\" target=\"_blank\" rel=\"noreferrer noopener\">migration<\/a> (which can happen with both IUDs <em>and <\/em>the Nexplanon). Besides these side effects, the pregnancy prevention rate of LARCs may <a href=\"https:\/\/naturalwomanhood.org\/pregnancy-after-iud-or-tubal-ligation\/\" target=\"_blank\" rel=\"noreferrer noopener\">not be as high<\/a> as many companies claim.&nbsp;<\/p>\n\n\n\n<p>Additional concerns with teen LARC use include:&nbsp;<\/p>\n\n\n\n<ul class=\"wp-block-list\">\n<li><strong>Delayed treatment of reproductive issues<\/strong>. Birth control is a common treatment for irregular cycles, period pain, and conditions like PCOS and <a href=\"https:\/\/naturalwomanhood.org\/endometriosis-teens\/\">endometriosis<\/a>. However, while contraception may do a good job at managing some teens\u2019 symptoms, LARCs cannot cure disease or restore health. Delaying proper treatment can make conditions more difficult to treat after discontinuing contraceptive use, and as <a href=\"https:\/\/pubmed.ncbi.nlm.nih.gov\/7254659\/#:~:text=It%20is%20suggested%20that%20a,here%20had%20increased%20prolactin%20levels.\">this case study<\/a> demonstrates, even raise the question of a connection between IUD use and endometriosis development [1].&nbsp;<\/li>\n<\/ul>\n\n\n\n<ul class=\"wp-block-list\">\n<li><strong>Potential added pressure to have sex<\/strong>. One thing sex education models tend to agree on is that sexual behavior can have profound consequences. In a sexual risk <em>reduction <\/em>model, the emphasis is placed on merely reducing the physical consequences of sex, especially through promoting effective contraception like LARCs. But this can sometimes leave teens with the impression that sexual activity is expected of them, and that delaying sex until later is socially abnormal. How many teens want to be perceived as abnormal by their peers? Conversely, sexual risk <em>avoidance <\/em>programs tend to focus on the whole-person effects of sexual activity and support teens in delaying sexual activity.<\/li>\n<\/ul>\n\n\n\n<ul class=\"wp-block-list\">\n<li><strong>Increased STI risk<\/strong>. According to the <a href=\"https:\/\/www.cdc.gov\/std\/life-stages-populations\/stdfact-teens.htm\">CDC<\/a>, half of all STD infections in the U.S. occur in people who are between the ages of 15 and 24 [6]. <a href=\"https:\/\/www.ncbi.nlm.nih.gov\/pmc\/articles\/PMC5856484\/\">Young women<\/a> are more likely to contract an STD than adult women because they produce less cervical mucus and have a larger transformation zone on their cervixes, so the cervix is more vulnerable to infection [2]. IUDs and implants do not protect against STDs and may contribute to teens feeling more lax about risky sexual behavior.<\/li>\n<\/ul>\n\n\n\n<h2 id=\"why-is-fertility-awareness-a-better-alternative\" class=\"wp-block-heading\">Why is fertility awareness a better alternative?<\/h2>\n\n\n\n<p>Teaching fertility awareness methods (FAM) to teens goes far beyond the goal of pregnancy avoidance. Teaching a young woman about how her body works truly supports her empowerment.<em> <\/em>Practically speaking, learning to track her fertility will serve her well in predicting periods, identifying hormonal or reproductive disorders, and avoiding <em>or<\/em> achieving pregnancy when it becomes appropriate (and when natural family planning method rules can be learned and layered on top of her existing fertility awareness knowledge).&nbsp;<\/p>\n\n\n\n<p>Less tangible are the executive skills FAM gives teens the opportunities to cultivate, like self-awareness, self-discipline, self-advocacy, organization, and communication. For parents who may be concerned that fertility awareness <a href=\"https:\/\/naturalwomanhood.org\/cycle-mindfulness-what-happens-when-you-teach-fertility-awareness-to-teen-girls\/\" target=\"_blank\" rel=\"noreferrer noopener\">wouldn\u2019t be enough<\/a> to protect a potentially sexually active teen from pregnancy, <a href=\"https:\/\/pubmed.ncbi.nlm.nih.gov\/15661598\/\" target=\"_blank\" rel=\"noreferrer noopener\">research<\/a> conducted by <a href=\"https:\/\/www.teenstar.org\/\" target=\"_blank\" rel=\"noreferrer noopener\">TeenSTAR<\/a> found that 97-99% of program participants kept their virginity and 40-50% of previously sexually active <a href=\"https:\/\/naturalwomanhood.org\/how-to-teach-teen-boys-about-fertility-awareness\/\" target=\"_blank\" rel=\"noreferrer noopener\"><em>teen boys<\/em><\/a><em> ceased sexual activity<\/em> [3]. Learning to understand and respect the human body can have a profound impact for teens of both sexes.<\/p>\n\n\n\n<p><a href=\"https:\/\/naturalwomanhood.org\/mothers-of-pre-teens-online-course\/\" target=\"_blank\" rel=\"noreferrer noopener\">Teaching teens about fertility awareness<\/a> is certainly more work than scheduling an OB\/GYN appointment to get an IUD or implant. In our experience, though, teens are <a href=\"https:\/\/naturalwomanhood.org\/3-myths-about-teaching-teens-about-fertility-awareness-2019\/\" target=\"_blank\" rel=\"noreferrer noopener\">up for the challenge<\/a> and open to the <a href=\"https:\/\/naturalwomanhood.org\/improve-communication-with-daughter\/\" target=\"_blank\" rel=\"noreferrer noopener\">closer relationship <\/a>with their moms that naturally develops when they learn more about their bodies and fertility.
