{"id":17713,"date":"2023-07-14T09:41:36","date_gmt":"2023-07-14T14:41:36","guid":{"rendered":"https:\/\/naturalwomanhood.org\/?p=17713"},"modified":"2024-09-20T07:55:29","modified_gmt":"2024-09-20T12:55:29","slug":"peut-on-ovuler-sous-contraceptif","status":"publish","type":"post","link":"https:\/\/naturalwomanhood.org\/fr\/can-you-ovulate-on-birth-control\/","title":{"rendered":"Peut-on ovuler sous contraception ?"},"content":{"rendered":"\n<p>Ever wondered, \u2018can you ovulate on birth control?\u2019 Would you believe that anywhere from <a href=\"https:\/\/www.aafp.org\/pubs\/afp\/issues\/2000\/0501\/p2605.html#afp20000501p2605-b4\">10<\/a> to <a href=\"https:\/\/pubmed.ncbi.nlm.nih.gov\/24726226\">85%<\/a> of cycles could be ovulatory, depending on the type of birth control used? This does not mean that you are likely to get pregnant in all of those instances. But ovulating on birth control, also known as breakthrough ovulation, is more common and has greater implications than you might think. So how can you know if or when breakthrough ovulation happens to you while on birth control? Are there signs of ovulation while on birth control? I\u2019ll cover the answer to that question and others here.&nbsp;<\/p>\n\n\n\n<h2 class=\"wp-block-heading\" id=\"h-what-is-breakthrough-ovulation\"><span id=\"what-is-breakthrough-ovulation\">What is breakthrough ovulation?<\/span><\/h2>\n\n\n\n<p>We all have that friend who got pregnant despite being on one, or maybe even two or three different forms of birth control. How is that possible? The truth is, that while the primary aim of the birth control pill is to stop a woman from ovulating (i.e., releasing an egg for possible fertilization), ovulation is still sometimes possible. This phenomenon is called \u201cbreakthrough ovulation.\u201d When it happens, it is possible for you to get pregnant that cycle if you have sex around the time of ovulation. Yes, this is despite taking the Pill!&nbsp;<\/p>\n\n\n\n<h3 id=\"how-the-pill-works\" class=\"wp-block-heading\">How the Pill works<\/h3>\n\n\n\n<p>Conception can occur if you have sex around the time of breakthrough ovulation because of how the Pill itself works. The Pill has three basic ways or mechanisms of action for preventing pregnancy. The first line of \u2018defense\u2019 against pregnancy is to <a href=\"https:\/\/naturalwomanhood.org\/no-cycle-on-the-pill\/\" target=\"_blank\" rel=\"noreferrer noopener\">block the hypothalamic-pituitary-ovarian (HPO) feedback loop<\/a>, which stops ovulation. The second mechanism of action is to <a href=\"https:\/\/naturalwomanhood.org\/no-cycle-on-the-pill\/\" target=\"_blank\" rel=\"noreferrer noopener\">thicken your cervical mucus<\/a>. This prevents sperm from reaching the cervix. The third and final mechanism is to <a href=\"https:\/\/naturalwomanhood.org\/no-cycle-on-the-pill\/\" target=\"_blank\" rel=\"noreferrer noopener\">thin your endometrium<\/a>, or uterine lining, to make implantation impossible.<\/p>\n\n\n\n<p>If breakthrough ovulation happens, that means the first mechanism did not work. This may be due to \u2018normal\u2019 usage of the Pill (more on perfect vs normal use in a bit), or the expected effects of the <a href=\"https:\/\/naturalwomanhood.org\/over-the-counter-birth-control\/\" target=\"_blank\" rel=\"noreferrer noopener\">estrogen-free Minipill<\/a>. If you have sex during your fertile window and the secondary mechanism also fails to thicken your cervical mucus enough, your partner\u2019s sperm can reach your egg. Fertilization is possible.&nbsp;<\/p>\n\n\n\n<p>That\u2019s when the final mechanism, thinning of the endometrial lining, comes into play. This tertiary mechanism <em>does not prevent <\/em>conception. Its purpose is to block an already conceived embryo from implanting in your uterus.<\/p>\n\n\n\n<h2 class=\"wp-block-heading\" id=\"h-what-causes-breakthrough-ovulation\"><span id=\"what-causes-breakthrough-ovulation\">What causes breakthrough ovulation?<\/span><\/h2>\n\n\n\n<p>Breakthrough ovulation occurs for a number of different reasons. Most are due to more \u2018normal\u2019 or typical use of the Pill rather than perfect use (think: you take your pill an hour later than normal, or accidentally skip a dose altogether). Up to <a href=\"https:\/\/www.insider.com\/why-you-can-get-pregnant-on-birth-control-2016-6\" target=\"_blank\" rel=\"noreferrer noopener\">9 out of 100 Pill users<\/a> will conceive every year with \u2018normal\u2019 rather than perfect usage of the Pill.&nbsp;<\/p>\n\n\n\n<p>Speaking of skipping a dose (or two), you may be <a href=\"https:\/\/pubmed.ncbi.nlm.nih.gov\/6413129\/\" target=\"_blank\" rel=\"noreferrer noopener\">especially likely<\/a> to conceive if you miss at the beginning or end of your monthly course. This is not the <em>only<\/em> way that breakthrough ovulation can occur, however. Gastrointestinal issues that cause vomiting or diarrhea can also decrease the efficacy of the drug. So can obesity or use of <a href=\"https:\/\/www.verywellhealth.com\/medications-that-can-cause-interactions-with-the-pill-906876\" target=\"_blank\" rel=\"noreferrer noopener\">other medications that interact<\/a> with the Pill.&nbsp;<\/p>\n\n\n\n<h2 class=\"wp-block-heading\" id=\"h-how-often-might-breakthrough-ovulation-on-hormonal-birth-control-occur\"><span id=\"how-often-might-breakthrough-ovulation-on-hormonal-birth-control-occur\">How often might breakthrough ovulation on hormonal birth control occur?<\/span><\/h2>\n\n\n\n<p>Even for those women who take the Pill perfectly, ovulation can still happen in <a href=\"https:\/\/www.aafp.org\/pubs\/afp\/issues\/2000\/0501\/p2605.html#afp20000501p2605-b4\" target=\"_blank\" rel=\"noreferrer noopener\">up to 10% of cycles<\/a>, which means fertilization may still be possible. It\u2019s true that in many cases, a woman who ovulates while on birth control will still not get pregnant. But breakthrough ovulation is a significant factor to consider when you decide to use any kind of birth control.&nbsp;<\/p>\n\n\n\n<p>Oral contraceptives\u2019 secondary mechanisms or back-up actions (such as the thinning of the endometrial lining) occur <em>after <\/em>fertilization. This may make implantation less likely to occur, which would lead to the loss of a <a href=\"https:\/\/pubmed.ncbi.nlm.nih.gov\/10693729\/\" target=\"_blank\" rel=\"noreferrer noopener\">newly conceived embryo<\/a>, rather than the mere prevention of its creation [2]. A woman on birth control might not even realize she has conceived and is experiencing a very early miscarriage.&nbsp;<\/p>\n\n\n\n<h3 id=\"the-minipill-and-breakthrough-ovulation\" class=\"wp-block-heading\">The Minipill and breakthrough ovulation<\/h3>\n\n\n\n<p>Chances of ovulating while on birth control can be even higher if you are taking the progestin-only Pill, also known as the Minipill. (We\u2019ve previously discussed <a href=\"https:\/\/naturalwomanhood.org\/over-the-counter-birth-control\/\">efforts to make the Minipill, or Opill, available over-the-counter<\/a> without physician oversight, which the FDA officially approved for OTC use on July 13, 2023). While the typical birth control pill includes the combination of both estrogen and progestin, the Minipill only includes progestin.&nbsp;<\/p>\n\n\n\n<p>The Minipill thickens the cervical mucus and thins the endometrial lining, but does not always stop ovulation. In fact, up to <a href=\"https:\/\/www.mayoclinic.org\/tests-procedures\/minipill\/about\/pac-20388306#:~:text=The%20minipill%20may%20keep%20you,day%20at%20the%20same%20time.\" target=\"_blank\" rel=\"noreferrer noopener\">half of women<\/a> taking the Minipill will still ovulate\u2014and that\u2019s with perfect usage! Taking this medication perfectly also requires that you take your birth control at the same time every single day, as taking it even <a href=\"https:\/\/familydoctor.org\/progestin-only-birth-control-pills\/\" target=\"_blank\" rel=\"noreferrer noopener\">a few hours late<\/a> can result in it being ineffective at preventing pregnancy.&nbsp;<\/p>\n\n\n\n<p>The main reason women choose the Minipill over the combination pill is because it doesn\u2019t come with the <a href=\"https:\/\/naturalwomanhood.org\/topic\/birth-control-side-effects\/\" target=\"_blank\" rel=\"noreferrer noopener\">side effects<\/a> of synthetic estrogen in the combination pill. This is especially important for women who <a href=\"https:\/\/naturalwomanhood.org\/is-birth-control-safe-while-breastfeeding\/\" target=\"_blank\" rel=\"noreferrer noopener\">breastfeed, for whom estrogen is discouraged for a number of reasons<\/a>. However, the progestin-only pill has its own <a href=\"https:\/\/www.liveaction.org\/news\/fda-considers-first-counter-birth-control-pill\/\" target=\"_blank\" rel=\"noreferrer noopener\">issues<\/a>. These include <a href=\"https:\/\/www.accessdata.fda.gov\/drugsatfda_docs\/label\/2017\/017031s035s036lbl.pdf\" target=\"_blank\" rel=\"noreferrer noopener\">ovarian cysts<\/a>, <a href=\"https:\/\/www.accessdata.fda.gov\/drugsatfda_docs\/label\/2017\/017031s035s036lbl.pdf\" target=\"_blank\" rel=\"noreferrer noopener\">irregular bleeding<\/a>, plus increased risks of depression, <a href=\"https:\/\/pubmed.ncbi.nlm.nih.gov\/29211679\/\" target=\"_blank\" rel=\"noreferrer noopener\">breast cancer<\/a>, and <a href=\"https:\/\/pubmed.ncbi.nlm.nih.gov\/26808155\/\" target=\"_blank\" rel=\"noreferrer noopener\">cervical cancer<\/a> [3][4].&nbsp;<\/p>\n\n\n\n<h2 class=\"wp-block-heading\" id=\"h-what-about-intrauterine-devices-iuds-nbsp\"><span id=\"what-about-intrauterine-devices-iuds\">What about intrauterine devices (IUDs)?&nbsp;<\/span><\/h2>\n\n\n\n<p>There are currently two main types of <a href=\"https:\/\/naturalwomanhood.org\/iuds-insertion-pain-perforation-risk\/\" target=\"_blank\" rel=\"noreferrer noopener\">IUDs<\/a> used in the United States. One is the <a href=\"https:\/\/naturalwomanhood.org\/never-let-daughter-get-iud\/\" target=\"_blank\" rel=\"noreferrer noopener\">hormonal IUD<\/a> (such as Mirena). The other is the <a href=\"https:\/\/naturalwomanhood.org\/anxiety-and-other-warning-signs-side-effects-of-iud\/\" target=\"_blank\" rel=\"noreferrer noopener\"><a href=\"https:\/\/naturalwomanhood.org\/anxiety-and-other-warning-signs-side-effects-of-iud\/\" target=\"_blank\" rel=\"noreferrer noopener\">non-hormonal<\/a><\/a> <a href=\"https:\/\/naturalwomanhood.org\/anxiety-and-other-warning-signs-side-effects-of-iud\/\" target=\"_blank\" rel=\"noreferrer noopener\">copper IUD (Paragard)<\/a>. Hormonal IUDs like Mirena primarily <a href=\"https:\/\/www.ncbi.nlm.nih.gov\/pmc\/articles\/PMC7000571\/\" target=\"_blank\" rel=\"noreferrer noopener\">thicken the cervical mucus<\/a> to decrease sperm mobility [5]. Ovulation prevention is not the primary goal. In fact, according to <a href=\"https:\/\/labeling.bayerhealthcare.com\/html\/products\/pi\/Mirena_PI.pdf\" target=\"_blank\" rel=\"noreferrer noopener\">Mirena\u2019s product label<\/a>, once patients reach their fourth year of use,<em> the majority of cycles are ovulatory.\u00a0<\/em><\/p>\n\n\n\n<p>The <a href=\"https:\/\/naturalwomanhood.org\/commonly-asked-questions-about-the-copper-iud-a-natural-womanhood-copper-iud-faq-copper-toxicity-2020\/\" target=\"_blank\" rel=\"noreferrer noopener\">copper IUD<\/a>, or Paragard, also does not stop ovulation. Like the Mirena, <a href=\"https:\/\/naturalwomanhood.org\/is-copper-iud-toxic-painful-cause-hormone-imbalance-side-effects-thyroid-natural-birth-control-08112018\/\" target=\"_blank\" rel=\"noreferrer noopener\">Paragard<\/a> also works to prevent fertilization by creating a <a href=\"https:\/\/www.healthline.com\/health\/birth-control\/copper-iud#what-it-is\" target=\"_blank\" rel=\"noreferrer noopener\">\u2018toxic\u2019 environment<\/a> for sperm. It secondarily works by <a href=\"https:\/\/www.paragard.com\/wp-content\/uploads\/2018\/10\/PARAGARD-PI.pdf\" target=\"_blank\" rel=\"noreferrer noopener\">preventing the implantation<\/a> of a newly-conceived embryo.<\/p>\n\n\n\n<p>Although there are differences between hormonal and <a href=\"https:\/\/naturalwomanhood.org\/surviving-a-toxic-reaction-to-the-copper-iud-personal-story\/\" target=\"_blank\" rel=\"noreferrer noopener\">copper IUDs<\/a>, one <a href=\"https:\/\/pubmed.ncbi.nlm.nih.gov\/24726226\" target=\"_blank\" rel=\"noreferrer noopener\">study<\/a> found that the average chance of ovulation within a cycle with both IUDs is the same\u2014about 85% [6]! The ovulation here isn\u2019t necessarily \u201cbreakthrough\u201d because the primary mechanism of action in either <a href=\"https:\/\/naturalwomanhood.org\/how-my-awful-experience-with-the-copper-iud-led-me-to-truly-natural-birth-control-1-19\/\" target=\"_blank\" rel=\"noreferrer noopener\">IUD<\/a> is not to stop ovulation. Regardless, the bottom line for all types of birth control is the same: when ovulation occurs, there is a chance that fertilization and conception will <em>not<\/em> be prevented if one has sex around the time of ovulation.&nbsp;<\/p>\n\n\n\n<h3 class=\"wp-block-heading\" id=\"h-why-haven-t-you-heard-any-of-this-before-nbsp\"><span id=\"why-havent-you-heard-any-of-this-before\">Why haven\u2019t you heard any of this before?&nbsp;<\/span><\/h3>\n\n\n\n<p>Keep in mind that prior to the 1960s, the <a href=\"https:\/\/www.liveaction.org\/news\/acog-abortion-population-control-redefined-beginning-life\/\" target=\"_blank\" rel=\"noreferrer noopener\">medical definition<\/a> of \u201cpregnancy\u201d was synonymous with fertilization, i.e., when sperm meets egg. Beginning with the advent of the birth control pill, major medical organizations sought to change the definition of &#8220;pregnancy&#8221; to begin at implantation, rather than at fertilization. By this newer definition, which was officially endorsed by ACOG in 1972, birth control does not technically end a pregnancy when it fails to prevent ovulation, and instead functions to prevent pregnancy by its endometrial-thinning mechanism if conception still occurs. <\/p>\n\n\n\n<p>However, the science of embryology is clear that at the moment of conception, a unique, distinct, human life is created, regardless of whether or not it is able to implant within the womb.&nbsp;Therefore, by this tertiary mechanism, birth control undoubtedly <a href=\"https:\/\/naturalwomanhood.org\/can-you-chart-your-cycle-while-on-the-pill-birth-control-2-19\/\" target=\"_blank\" rel=\"noreferrer noopener\">facilitates the death of a newly created embryonic life<\/a>. Despite the change in the definition of pregnancy, women still deserve this important information about how their birth control works, in order to make a truly informed decision about their family planning choices. <\/p>\n\n\n\n<h2 class=\"wp-block-heading\" id=\"h-women-who-are-concerned-about-the-implications-of-breakthrough-ovulation-and-possible-unintended-pregnancy-loss-have-other-family-planning-options\"><span id=\"women-who-are-concerned-about-the-implications-of-breakthrough-ovulation-and-possible-unintended-pregnancy-loss-have-other-family-planning-options\">Women who are concerned about the implications of breakthrough ovulation and possible unintended pregnancy loss have other family planning options<\/span><\/h2>\n\n\n\n<p>Unfortunately, it is extremely difficult to tell whether you\u2019ve experienced breakthrough ovulation. Even though you can monitor fertile symptoms such as temperature and cervical mucus while on the Pill (or while using an IUD), there is <a href=\"https:\/\/naturalwomanhood.org\/can-you-chart-your-cycle-while-on-the-pill-birth-control-2-19\/\" target=\"_blank\" rel=\"noreferrer noopener\">no way to know<\/a> if those signs of fertility are real or just effects of the birth control. Plus, the bleeding you experience on hormonal birth control is <a href=\"https:\/\/naturalwomanhood.org\/can-you-chart-your-cycle-while-on-the-pill-birth-control-2-19\/\" target=\"_blank\" rel=\"noreferrer noopener\">not<\/a> a true period. Because of this, it\u2019s not really possible to completely prevent the conception and loss of an early stage embryo if you are sexually active around the time of breakthrough ovulation. This is because there aren\u2019t really any true symptoms of breakthrough ovulation.\u00a0<\/p>\n\n\n\n<h2 class=\"wp-block-heading\" id=\"h-how-fertility-awareness-can-help\"><span id=\"how-fertility-awareness-can-help\">How fertility awareness can help<\/span><\/h2>\n\n\n\n<p>This doesn\u2019t mean it\u2019s impossible to know what\u2019s going on with your body and when you ovulate. <a href=\"https:\/\/naturalwomanhood.org\/what-to-expect-off-the-pill-after-years-hormonal-birth-control-contraceptive-08042018\/\" target=\"_blank\" rel=\"noreferrer noopener\">Countless women<\/a> have made the switch from birth control to <a href=\"https:\/\/naturalwomanhood.org\/topic\/fertility-awareness-methods\/what-is-fam\/\" target=\"_blank\" rel=\"noreferrer noopener\">Fertility Awareness Methods<\/a>, or FAM. When you stop birth control and instead learn your true signs of fertility, you are able to understand when ovulation occurs. You can use that knowledge to prevent or achieve pregnancy accordingly.&nbsp;<\/p>\n\n\n\n<p>There are also numerous <a href=\"https:\/\/naturalwomanhood.org\/topic\/fertility-awareness-methods\/health-benefits-of-charting\/\" target=\"_blank\" rel=\"noreferrer noopener\">health benefits<\/a> of charting your cycle. They go far beyond simply avoiding the <a href=\"https:\/\/naturalwomanhood.org\/topic\/birth-control-side-effects\/\" target=\"_blank\" rel=\"noreferrer noopener\">negative risks and side effects of birth control<\/a>.
