{"id":14904,"date":"2022-06-02T22:03:29","date_gmt":"2022-06-03T03:03:29","guid":{"rendered":"https:\/\/naturalwomanhood.org\/?p=14904"},"modified":"2025-02-10T08:41:31","modified_gmt":"2025-02-10T14:41:31","slug":"gels-contraceptifs-masculins-pilules-de-controle-des-naissances","status":"publish","type":"post","link":"https:\/\/naturalwomanhood.org\/fr\/male-birth-control-contraceptive-gels-pills\/","title":{"rendered":"Le point sur les gels et pilules contraceptifs masculins"},"content":{"rendered":"\n<p>Minnesota mice <a href=\"https:\/\/www.medicalnewstoday.com\/articles\/safe-and-99-effective-birth-control-pill-for-men-may-soon-become-reality#:~:text=%E2%80%9CSince%20men%20do%20not%20have,to%20avoid%20hormonal%20side%20effects.\" target=\"_blank\" rel=\"noreferrer noopener\">made news<\/a> recently when researchers at the University of Minnesota shared promising findings from animal trials of a novel non hormonal oral contraceptive billed as \u201cthe Pill for men.\u201d According to the researchers, the pill was \u201c99% effective in preventing pregnancy, without any observable side effects\u201d and furthermore, its effects were reversible within 4-6 weeks of discontinuing the drug. But will the pill work in humans?<\/p>\n\n\n\n<p>Even though every few years there\u2019s media buzz about one or more potentially forthcoming innovative male contraceptive(s), decades\u2019 worth of experiments have amounted to exactly nothing new on the market to date, leaving, so far as most men know, the same-old, same-old options of <a href=\"https:\/\/naturalwomanhood.org\/the-intact-gentleman-vasectomy-escape-route-why-nobody-needs-to-go-under-the-knife\/\" target=\"_blank\" rel=\"noreferrer noopener\">vasectomies<\/a> or condoms. The former, of course, are not easily <a href=\"https:\/\/naturalwomanhood.org\/is-a-vasectomy-reversal-effective-enough-to-help-you-become-a-dad-again\/\" target=\"_blank\" rel=\"noreferrer noopener\">reversible<\/a>, and can have <a href=\"https:\/\/naturalwomanhood.org\/the-reality-of-vasectomy-risks-and-side-effects\/\" target=\"_blank\" rel=\"noreferrer noopener\">serious side effects<\/a>, and the latter are single-use and <a href=\"https:\/\/www.cdc.gov\/reproductivehealth\/contraception\/index.htm\" target=\"_blank\" rel=\"noreferrer noopener\">less effective<\/a> than most female contraceptives.&nbsp;<\/p>\n\n\n\n<p>Why is it so difficult to develop effective, reversible male contraceptives? What options are currently being studied, and what are the pros and cons of each? Here we\u2019ll explore the answers to all the above, and, most importantly, highlight a <a href=\"https:\/\/naturalwomanhood.org\/tubal-ligation-vs-vasectomy-possible-relationship-effects-of-sterilization\/\" target=\"_blank\" rel=\"noreferrer noopener\">holistic option for family planning<\/a> that\u2019s built on a couple\u2019s <em>shared<\/em> responsibility for the gift of their fertility.&nbsp;&nbsp;<\/p>\n\n\n\n<h2 class=\"wp-block-heading\" id=\"h-why-is-it-so-difficult-to-develop-effective-reversible-male-contraceptives-nbsp\"><span id=\"why-is-it-so-difficult-to-develop-effective-reversible-male-contraceptives\">Why is it so difficult to develop effective, reversible male contraceptives?&nbsp;<\/span><\/h2>\n\n\n\n<h3 class=\"wp-block-heading\" id=\"h-male-biology\"><span id=\"male-biology\">Male biology<\/span><\/h3>\n\n\n\n<p>There are several reasons effective, reversible male contraceptives don\u2019t already exist. The first reason is biological. Whereas female contraceptives seek to prevent a once-a-cycle event (ovulation), male contraceptives have to continually inhibit the production of, or <a href=\"https:\/\/www.malecontraceptive.org\/mechanisms-of-male-contraception.html\" target=\"_blank\" rel=\"noreferrer noopener\">otherwise render ineffective<\/a>, <em>millions<\/em> of sperm at a time, with millions more being produced<em> every day.&nbsp;<\/em><\/p>\n\n\n\n<h3 class=\"wp-block-heading\" id=\"h-double-standards-concerning-side-effects\"><span id=\"double-standards-concerning-side-effects\">Double standards concerning side effects<\/span><\/h3>\n\n\n\n<p>Another reason male contraception hasn\u2019t meaningfully materialized is due to a disconcerting <a href=\"https:\/\/naturalwomanhood.org\/reproductive-respect-birth-control-history-inequality-international-womens-day-2018\/\" target=\"_blank\" rel=\"noreferrer noopener\">double standard<\/a> when it comes to acceptable side effects of birth control for men vs. women. Hormonal birth control comes with a <a href=\"https:\/\/naturalwomanhood.org\/topic\/birth-control-side-effects\/\" target=\"_blank\" rel=\"noreferrer noopener\">whole host of side effects<\/a> regardless of whether its consumers are male or female, but, as Natural Womanhood <a href=\"https:\/\/naturalwomanhood.org\/whats-the-status-on-male-birth-control-2019\/\" target=\"_blank\" rel=\"noreferrer noopener\">previously noted<\/a>, tolerance for side effects in men has been notoriously low. As an example, one <a href=\"https:\/\/academic.oup.com\/jcem\/article\/101\/12\/4779\/2765061\" target=\"_blank\" rel=\"noreferrer noopener\">2016 male hormonal contraceptive study<\/a> was terminated early by an external safety review committee due to \u201cacne, injection site pain, increased libido, and mood disorders\u201d in the (all male) study subjects.<\/p>\n\n\n\n<p>In contrast, a few women enrolled in the Puerto Rican trials for the original birth control pill (Enovid) <em>died<\/em>, and yet the trial continued, and the Pill was approved by the FDA a few years later. Sadly, over 60 years later, nothing has changed; as recently as 2020, pharmaceutical giant Bayer was made to <a href=\"https:\/\/www.liveaction.org\/news\/bayer-billion-settle-birth-control-lawsuits\/\" target=\"_blank\" rel=\"noreferrer noopener\">pay out <\/a>$1.6 billion to settle 39,000 lawsuits alleging harm due to <a href=\"https:\/\/naturalwomanhood.org\/essure-bayer-birth-control-bleeding-edge-netflix-documentary-health-risks-08012018\/\" target=\"_blank\" rel=\"noreferrer noopener\">Essure<\/a>, its now-defunct permanent birth control device.&nbsp;<\/p>\n\n\n\n<p>In an interview with <a href=\"https:\/\/www.washingtonpost.com\/wellness\/2022\/04\/14\/male-birth-control-pill-gel\/\" target=\"_blank\" rel=\"noreferrer noopener\"><em>The Washington Post<\/em><\/a>, Heather Vahdat, executive director of the Male Contraceptive Institute (MCI), summarized the reasons effective, reversible male contraceptives do not exist, citing \u201climited funding, lengthy regulatory procedures, biological challenges and societal issues such as the long-standing belief that the people who can get pregnant should bear the responsibility for preventing pregnancies.\u201d University of Minnesota graduate student Md Abdullah al-Noman, who presented the research team\u2019s findings from the mice trials mentioned above, <a href=\"https:\/\/www.medicalnewstoday.com\/articles\/safe-and-99-effective-birth-control-pill-for-men-may-soon-become-reality#:~:text=%E2%80%9CSince%20men%20do%20not%20have,to%20avoid%20hormonal%20side%20effects.\" target=\"_blank\" rel=\"noreferrer noopener\">underscored Vahdat\u2019s last point, opining<\/a> bluntly that \u201csince men do not have to suffer the consequences of pregnancy, the threshold for side effects from birth control pills is rather low.\u201d&nbsp;<\/p>\n\n\n\n<h2 class=\"wp-block-heading\" id=\"h-male-birth-control-pills-and-contraceptive-gels-the-latest-birth-control-options-being-studied-for-men\"><span id=\"male-birth-control-pills-and-contraceptive-gels-the-latest-birth-control-options-being-studied-for-men\">Male birth control pills and contraceptive gels: The latest birth control options being studied for men<\/span><\/h2>\n\n\n\n<p>MCI keeps a <a href=\"https:\/\/www.malecontraceptive.org\/nhrmc-database.html\" target=\"_blank\" rel=\"noreferrer noopener\">running tally<\/a> of ongoing research into various forms of male contraception, mostly funded by the Eunice Kennedy Shriver National Institute of Child and Health Development. Here we\u2019ll cover the three male contraceptives garnering the most media attention in recent months.&nbsp;<\/p>\n\n\n\n<h3 class=\"wp-block-heading\" id=\"h-the-pill-for-men\"><span id=\"the-pill-for-men\">The Pill for men<\/span><\/h3>\n\n\n\n<p>In March 2022, researchers at the University of Minnesota <a href=\"https:\/\/www.acs.org\/content\/acs\/en\/pressroom\/newsreleases\/2022\/march\/non-hormonal-pill-could-soon-expand-mens-birth-control-options.html\" target=\"_blank\" rel=\"noreferrer noopener\">presented their findings<\/a> from animal trials of a reversible non hormonal oral contraceptive at the American Chemical Society spring meeting. The pill specifically inhibits the action of Vitamin A, which is vital for sperm production.&nbsp; Md Abdullah al-Noman explained, \u201cmen produce around 1,500 sperm per heartbeat. This massive production of sperm needs vitamin A in different stages. But we need vitamin A for many essential physiological processes as well. So, we specifically blocked the action of vitamin A in such a way that it (YCT529) reduces sperm production without interfering with other functions of vitamin A to preclude side effects.\u201d&nbsp;<\/p>\n\n\n\n<p>According to the U of M <a href=\"https:\/\/www.acs.org\/content\/acs\/en\/pressroom\/newsreleases\/2022\/march\/non-hormonal-pill-could-soon-expand-mens-birth-control-options.html\" target=\"_blank\" rel=\"noreferrer noopener\">press release<\/a>, \u201cWhen given orally to male mice for 4 weeks, YCT529 dramatically reduced sperm counts and was 99% effective in preventing pregnancy, without any observable side effects. The mice could father pups again 4-6 weeks after they stopped receiving the compound.\u201d &nbsp;<\/p>\n\n\n\n<p>In an interview with <a href=\"https:\/\/www.medicalnewstoday.com\/articles\/safe-and-99-effective-birth-control-pill-for-men-may-soon-become-reality?c=838897177061#Nonhormonal-male-contraceptive\" target=\"_blank\" rel=\"noreferrer noopener\"><em>Medical News Today<\/em><\/a>, al-Noman acknowledged that researchers have no idea whether the resounding success in mice will translate to human subjects. He commented, \u201cit\u2019s hard to predict how the human clinical trial will go since that\u2019s the most significant barrier for a drug candidate to prove its safety and efficacy. We licensed this candidate to <a href=\"https:\/\/www.yourchoicetx.com\/\" target=\"_blank\" rel=\"noreferrer noopener\">YourChoice Therapeutics<\/a>, and they are planning to take it to clinical trial by the end of this year.\u201d&nbsp;<\/p>\n\n\n\n<h3 class=\"wp-block-heading\" id=\"h-nes-t-contraceptive-gel\"><span id=\"nes-t-contraceptive-gel\">NES\/T contraceptive gel<\/span><\/h3>\n\n\n\n<p>The National Institutes of Health (NIH) is funding research on a reversible hormonal contraceptive gel called NES\/T. NES\/T has two active ingredients: Nestorone (the NES in NES\/T), a synthetic form of progesterone, and Testosterone (the T in NES\/T). The synthetic progesterone blocks the body\u2019s own production of testosterone, and the supplemental <a href=\"https:\/\/naturalwomanhood.org\/fam-basics-testosterone-in-men-and-women\/\" target=\"_blank\" rel=\"noreferrer noopener\">testosterone<\/a> replaces what the body isn\u2019t making in order to reduce the side effects (think weight gain, depression, and high cholesterol) that would otherwise occur due to low testosterone.&nbsp;<\/p>\n\n\n\n<p>Currently, NES\/T is in Phase II testing, which began in 2018 and is expected to wrap up in 2023 at 15 sites in the United States and abroad. A Phase III trial will presumably follow, with an expected duration of 5 years. Researchers told <a href=\"https:\/\/www.cosmopolitan.com\/health-fitness\/a38954452\/male-contraceptive-gel-trial\/\" target=\"_blank\" rel=\"noreferrer noopener\"><em>Cosmo<\/em><\/a><em> <\/em>that the gel, applied once daily to the shoulders and upper arms, is perhaps 10 years from becoming commercially available.<\/p>\n\n\n\n<p>One potential barrier to widespread uptake is the lengthy time for total absorption of the gel into the user\u2019s skin. Participants in the Phase II trial have to wait four hours every single day after application before their bare shoulders can touch anyone else\u2019s skin.&nbsp;<\/p>\n\n\n\n<p>The primary proof of the gel\u2019s effectiveness is the complete absence of unintended pregnancies amongst trial participants to date, and the secondary proof is measurably low sperm counts. And despite what Vahdat (of MCI) called the \u201cbiological challenge\u201d of dramatically reducing sperm counts via medication, NES\/T researcher Dr. Brian Nguyen reassured <em>The<\/em> <em>Los Angeles Times<\/em> that the goal isn\u2019t to get sperm counts to zero.&nbsp;<\/p>\n\n\n\n<p>He commented, \u201cYour normal semen contains about 40 million to 300 million sperm per milliliter, which is huge, right? And if you ask yourself, why evolutionarily does a man need that much sperm per milliliter of semen in order to impregnate women?&#8230;The process of getting that sperm and egg is extremely perilous. So even at 10 [million] to 20 million, you\u2019re expecting this man is going to have difficulty achieving a pregnancy.\u201d&nbsp;<\/p>\n\n\n\n<p>But while the researchers celebrated the lack of unintended pregnancies in particular, it\u2019s difficult to say just how generalizable these results would be in the real world since the study required just one act of sexual intercourse without a condom or other barrier method each month.<\/p>\n\n\n\n<h3 class=\"wp-block-heading\" id=\"h-adam-implanted-contraceptive-hydrogel\"><span id=\"adam-implanted-contraceptive-hydrogel\">ADAM implanted contraceptive hydrogel<\/span><\/h3>\n\n\n\n<p>Researchers are calling a non hormonal, implanted male contraceptive hydrogel named ADAM the&nbsp; \u201cIUD for men,\u201d and <a href=\"http:\/\/www.contraline.com\/\" target=\"_blank\" rel=\"noreferrer noopener\">say<\/a> it\u2019s \u201cthe first major development in male contraception since the vasectomy.\u201d&nbsp; Like IUDs and other forms of long-acting reversible contraception (LARC), ADAM is intended to be a set-it-and-forget-it contraceptive. Normally, when a man is sexually aroused, sperm travels from the testes through the vas deferens for ejaculation from the urethra. But the implanted ADAM gel effectively blocks the way, preventing sperm\u2019s exit from the vas deferens.&nbsp;<\/p>\n\n\n\n<p>Keven Eisenfrats, co-founder and CEO of <a href=\"https:\/\/www.malecontraceptive.org\/contraline.html\" target=\"_blank\" rel=\"noreferrer noopener\">Contraline<\/a>, the medical device company that invented ADAM, noted in a <a href=\"https:\/\/www.youtube.com\/watch?v=BcFRJ2Z7THk&amp;t=78s\" target=\"_blank\" rel=\"noreferrer noopener\">video interview with MCI<\/a> that ADAM lasts reliably for several years, after which time it \u201cdegrades\u201d or liquefies on its own. ADAM is explained in greater detail <a href=\"http:\/\/www.contraline.com\/product\/\" target=\"_blank\" rel=\"noreferrer noopener\">here<\/a>. Eisenfrats acknowledged that a similar concept has been unsuccessfully tested in India, Indonesia, and China in decades past. Without going into detail, Eisenfrats insisted that Contraline\u2019s approach takes the problems observed in previous research on occlusive gels and seeks to solve them \u201cfrom scratch.\u201d While IUDs can be physically removed in a doctor\u2019s office, Eisenfrats did not describe how the ADAM gel could be removed for contraceptive reversibility. Eisenfrats told <em>The Washington Post<\/em> that some 2,000 men have \u201cexpressed interest\u201d in participating in a clinical trial.&nbsp;<\/p>\n\n\n\n<h2 class=\"wp-block-heading\" id=\"h-towards-a-more-equitable-future-shared-responsibility-for-family-planning\"><span id=\"towards-a-more-equitable-future-shared-responsibility-for-family-planning\">Towards a more equitable future: shared responsibility for family planning<\/span><\/h2>\n\n\n\n<p>A <em>Medical News Today<\/em> <a href=\"https:\/\/www.medicalnewstoday.com\/articles\/safe-and-99-effective-birth-control-pill-for-men-may-soon-become-reality?c=838897177061#The-birth-control-burden\" target=\"_blank\" rel=\"noreferrer noopener\">article<\/a> on the University of Minnesota mice research quoted the British Pregnancy Advisory Service saying, \u201cThis isn\u2019t just a question of benefits for men\u2014it\u2019s a matter of working toward ensuring that fertility and contraception are <em>a shared burden <\/em>and that women are no longer expected to spend the bulk of their fertile years avoiding pregnancy without an active contribution from their partners.\u201d&nbsp;<\/p>\n\n\n\n<p>While ADAM and NES\/T and the Vitamin A inhibitor (YCT529) are just the latest iterations of maybe-someday-effective reversible male contraceptives, <a href=\"https:\/\/naturalwomanhood.org\/male-birth-control-contraception-for-men-fabm-fertility-awareness-2020\/\" target=\"_blank\" rel=\"noreferrer noopener\">couples who use evidence-based fertility awareness methods<\/a> are <em>already <\/em>utilizing the science of male and female fertility to responsibly plan their families without side effects. Rather than suppressing his or her fertility, couples work with their bodies\u2019 natural design to achieve or avoid pregnancy.
