Pourtant, on estime que 20 millions d'Am\u00e9ricains souffrent d'une forme ou d'une autre de maladie thyro\u00efdienne ; une femme sur huit d\u00e9veloppera un trouble thyro\u00efdien au cours de sa vie. <\/p>\n\n\n\n<p>Jusqu'\u00e0 90% de l'hypothyro\u00efdie est subclinique, c'est-\u00e0-dire que le niveau de thyr\u00e9ostimuline dans le sang est normal, et le diagnostic passe souvent inaper\u00e7u. L'observation des sympt\u00f4mes d'un dysfonctionnement de la thyro\u00efde, ainsi que la connaissance de l'interpr\u00e9tation de votre diagramme de la m\u00e9thode de connaissance de la fertilit\u00e9 (FAM), peuvent aider \u00e0 diagnostiquer cette maladie insaisissable et ouvrir la voie \u00e0 un traitement efficace si elle est d\u00e9tect\u00e9e \u00e0 un stade suffisamment pr\u00e9coce.&nbsp;<\/p>\n\n\n\n<p>Cet article est le premier d'une s\u00e9rie de trois articles pour Natural Womanhood consacr\u00e9e au dysfonctionnement de la thyro\u00efde et \u00e0 sa relation avec la fertilit\u00e9. Dans cet article, j'esp\u00e8re expliquer la physiologie de la thyro\u00efde, les signes et les sympt\u00f4mes du dysfonctionnement de la thyro\u00efde, et les laboratoires de diagnostic qui recherchent la cause premi\u00e8re du dysfonctionnement de la thyro\u00efde. Dans la deuxi\u00e8me partie, j'expliquerai le traitement non seulement de l'hypothyro\u00efdie, mais aussi de ses causes profondes. Dans la troisi\u00e8me partie, nous nous pencherons sur la thyro\u00efdite de Hashismoto, une maladie auto-immune qui est la cause la plus fr\u00e9quente de dysfonctionnement de la thyro\u00efde chez les jeunes femmes.&nbsp;<\/p>\n\n\n\n<p>Mon objectif en \u00e9crivant cette s\u00e9rie est de vous donner les moyens de mieux conna\u00eetre votre corps et d'avoir confiance en vous pour parler \u00e0 votre m\u00e9decin afin d'obtenir les tests et les traitements n\u00e9cessaires pour gu\u00e9rir les dysfonctionnements de la thyro\u00efde.&nbsp;<\/p>\n\n\n\n<p>Sans plus attendre, commen\u00e7ons par un cours acc\u00e9l\u00e9r\u00e9 sur la physiologie de la thyro\u00efde...&nbsp;&nbsp;<\/p>\n\n\n\n<h2 class=\"wp-block-heading\" id=\"h-thyroid-hormones-nbsp-and-their-functions-nbsp\"><span id=\"thyroid-hormones-and-their-functions\">Les hormones thyro\u00efdiennes et leurs fonctions&nbsp;<\/span><\/h2>\n\n\n\n<p>Tout d'abord, l'hormone de stimulation de la thyro\u00efde (TSH) provenant de l'hypophyse (dans le cerveau) signale \u00e0 la glande thyro\u00efde (\u00e0 l'avant du cou) de produire principalement de la thyroxine (T4) et un peu de triiodothyronine (T3). La T4 n'est pas une hormone utilisable, elle doit donc \u00eatre convertie en T3 dans le foie et les intestins. La T3 est ensuite li\u00e9e \u00e0 des prot\u00e9ines et distribu\u00e9e dans tout l'organisme pour d\u00e9clencher et maintenir de nombreuses fonctions.&nbsp;<\/p>\n\n\n\n<p>Il existe \u00e9galement une hormone \"d'arr\u00eat\" appel\u00e9e Reverse T3. La Reverse T3 arr\u00eate le transfert de T4 en T3 lorsque l'organisme a suffisamment de T3 pour fonctionner. Cela permet de stocker une r\u00e9serve de T4 en cas de besoin (stress important, maladie ou blessure). Enfin, une faible quantit\u00e9 de T3 et de T4 dans l'organisme d\u00e9clenche l'hormone de lib\u00e9ration de la thyrotropine (TRH) dans l'hypothalamus (dans le cerveau), qui demande \u00e0 l'hypophyse de lib\u00e9rer de la TSH, et le cycle se poursuit.&nbsp;<\/p>\n\n\n\n<p>Les fonctions que ces hormones thyro\u00efdiennes remplissent dans l'organisme sont tr\u00e8s \u00e9tendues. Les hormones thyro\u00efdiennes sont impliqu\u00e9es dans : la r\u00e9gulation du rythme cardiaque et du d\u00e9bit cardiaque, la r\u00e9gulation du rythme respiratoire et de l'oxyg\u00e9nation, la r\u00e9gulation de la temp\u00e9rature, la contraction musculaire, la stabilisation de l'humeur, le m\u00e9tabolisme alimentaire, la r\u00e9gulation du taux de cholest\u00e9rol, la r\u00e9gulation de la croissance de la peau, des cheveux et des ongles, et la r\u00e9gulation des mouvements intestinaux. En outre, les hormones thyro\u00efdiennes ont un impact sur les hormones sexuelles qui r\u00e9gulent l'ovulation et les menstruations. Les hormones thyro\u00efdiennes collaborent \u00e9galement avec la glande surr\u00e9nale pour soutenir la r\u00e9ponse immunitaire et maintenir les neurotransmetteurs pour fournir de l'\u00e9nergie.<\/p>\n\n\n\n<h2 class=\"wp-block-heading\" id=\"h-symptoms-of-low-thyroid-function-aka-nbsp-underactive-thyroid-or-nbsp-hypothyroidism-nbsp\"><span id=\"symptoms-of-low-thyroid-function-aka-underactive-thyroid-or-hypothyroidism\">Sympt\u00f4mes d'une insuffisance thyro\u00efdienne, ou thyro\u00efde sous-active ou hypothyro\u00efdie&nbsp;<\/span><\/h2>\n\n\n\n<p>Le fait que les hormones thyro\u00efdiennes aient un impact aussi important sur l'ensemble de l'organisme explique pourquoi les signes et les sympt\u00f4mes d'une hypothyro\u00efdie sont si vari\u00e9s. La myriade de sympt\u00f4mes de l'hypothyro\u00efdie peut inclure : fatigue, faiblesse musculaire, long temps de r\u00e9cup\u00e9ration apr\u00e8s l'exercice, besoin de plus de sommeil que la plupart des gens, insomnie, d\u00e9pression, syndrome pr\u00e9menstruel (non am\u00e9lior\u00e9 apr\u00e8s un essai de remplacement de la progest\u00e9rone), anxi\u00e9t\u00e9, sensation de froid, reflux acide ou br\u00fblures d'estomac, manque d'app\u00e9tit (surtout le matin), constipation, sourcils clairsem\u00e9s (surtout le tiers externe), peau s\u00e8che, acn\u00e9, yeux secs, maux de t\u00eate ou migraines, visage, doigts ou pieds gonfl\u00e9s, ongles qui se cassent ou s'\u00e9caillent facilement, cheveux clairsem\u00e9s, cils clairsem\u00e9s, t\u00eate brumeuse, incapacit\u00e9 \u00e0 se concentrer, mauvaise m\u00e9moire, difficult\u00e9 \u00e0 perdre du poids, r\u00e8gles longues et\/ou irr\u00e9guli\u00e8res, et mauvaise fonction immunitaire\/des maladies fr\u00e9quentes.&nbsp;<\/p>\n\n\n\n<p>Les signes biologiques mesurables de l'hypothyro\u00efdie peuvent inclure : des temp\u00e9ratures corporelles basses, un rythme cardiaque lent au repos, un taux \u00e9lev\u00e9 de LDL (mauvais cholest\u00e9rol) et un faible taux de HDL (bon cholest\u00e9rol).<\/p>\n\n\n\n<p>Si une femme pr\u00e9sente plusieurs de ces signes ou sympt\u00f4mes, il est possible qu'elle souffre d'une insuffisance de la fonction thyro\u00efdienne.&nbsp;&nbsp;<\/p>\n\n\n\n<h2 class=\"wp-block-heading\" id=\"h-detecting-signs-of-hypothyroidism-in-your-fertility-chart\"><span id=\"detecting-signs-of-hypothyroidism-in-your-fertility-chart\">D\u00e9tecter les signes d'hypothyro\u00efdie dans votre tableau de fertilit\u00e9<\/span><\/h2>\n\n\n\n<p>L'utilisation d'une m\u00e9thode de connaissance de la fertilit\u00e9 (MCF) pour \u00e9tablir le diagramme de votre cycle de fertilit\u00e9 peut \u00eatre tr\u00e8s utile pour identifier un trouble thyro\u00efdien. L'utilisation d'un diagramme de fertilit\u00e9 aide la femme \u00e0 identifier la date approximative de l'ovulation, et les troubles thyro\u00efdiens peuvent se manifester dans un diagramme de fa\u00e7on r\u00e9v\u00e9latrice par rapport \u00e0 l'ovulation.&nbsp;&nbsp;&nbsp;<\/p>\n\n\n\n<p>Par exemple, la phase folliculaire (qui correspond au premier jour du cycle jusqu'\u00e0 l'ovulation) doit durer environ 11 \u00e0 23 jours.  Alors que la dur\u00e9e de la phase folliculaire peut varier d'un cycle \u00e0 l'autre, la dur\u00e9e de la phase lut\u00e9ale (c'est-\u00e0-dire la p\u00e9riode entre l'ovulation et le d\u00e9but des r\u00e8gles suivantes) devrait \u00eatre pratiquement constante d'un mois \u00e0 l'autre (la dur\u00e9e moyenne de la phase lut\u00e9ale est de 11 \u00e0 17 jours).&nbsp;&nbsp;<\/p>\n\n\n\n<p>L'hypothyro\u00efdie peut se manifester par une phase folliculaire longue et une phase lut\u00e9ale variable. Une femme dont la fonction thyro\u00efdienne est sous-active peut \u00e9galement constater des r\u00e8gles abondantes et des saignements bruns \u00e0 la fin de ses r\u00e8gles (pendant plus de deux jours).  En outre, si elle note ses temp\u00e9ratures, elle constatera qu'elles sont constamment basses, m\u00eame apr\u00e8s l'ovulation\/pendant la phase lut\u00e9ale, qui est cens\u00e9e montrer une augmentation de la temp\u00e9rature en raison de la pr\u00e9sence de progest\u00e9rone.&nbsp;&nbsp;&nbsp;<\/p>\n\n\n\n<h2 class=\"wp-block-heading\" id=\"h-how-hypothyroidism-and-hashimoto-s-thyroiditis-is-diagnosed\"><span id=\"how-hypothyroidism-and-hashimotos-thyroiditis-is-diagnosed\">Comment l'hypothyro\u00efdie (et la thyro\u00efdite de Hashimoto) est-elle diagnostiqu\u00e9e ?<\/span><\/h2>\n\n\n\n<p>Si ces signes et sympt\u00f4mes font suspecter une hypothyro\u00efdie, il est temps de demander \u00e0 un professionnel de la sant\u00e9 de proc\u00e9der \u00e0 un bilan biologique complet afin de d\u00e9terminer la cause profonde des sympt\u00f4mes.<\/p>\n\n\n\n<p>Un bilan thyro\u00efdien complet comprend des tests visant \u00e0 d\u00e9terminer les taux de TSH, de T4 libre et totale, de T3 libre et totale, de T3 inverse et d'anticorps thyro\u00efdiens dans le sang. Les deux anticorps susceptibles d'affecter la glande thyro\u00efde sont les anticorps anti-TPO (anticorps anti-thyro\u00efdienne peroxydase) et les anticorps anti-TgAb (anticorps anti-thyroglobuline). Ces anticorps attaquent la glande thyro\u00efde, la rendant moins efficace dans son travail.&nbsp;<\/p>\n\n\n\n<p>Un test positif pour ces anticorps permet de diagnostiquer une thyro\u00efdite de Hashimoto, une affection qui doit \u00eatre trait\u00e9e, que les autres analyses de la thyro\u00efde soient normales ou non, afin d'\u00e9viter que la thyro\u00efde ne subisse d'autres dommages. (Remarque : nous approfondirons la question de la thyro\u00efdite d'Hashimoto dans la troisi\u00e8me partie de cette s\u00e9rie sur les dysfonctionnements de la thyro\u00efde).&nbsp;<\/p>\n\n\n\n<p>Si le diagnostic d'hypothyro\u00efdie ou de thyro\u00efdite de Hashimoto est pos\u00e9, le travail de votre m\u00e9decin n'est pas termin\u00e9. En tant que m\u00e9decin, je pense qu'il ne suffit pas de donner un diagnostic \u00e0 un patient et de se contenter de g\u00e9rer ses sympt\u00f4mes. Je crois fermement que la source du dysfonctionnement de la thyro\u00efde doit \u00eatre trouv\u00e9e et trait\u00e9e afin de r\u00e9tablir la sant\u00e9 du patient.&nbsp;&nbsp;<\/p>\n\n\n\n<h2 class=\"wp-block-heading\" id=\"h-similarities-in-the-prescription-of-birth-control-and-levothyroxine-t4\"><span id=\"similarities-in-the-prescription-of-birth-control-and-levothyroxine-t4\">Similitudes dans la prescription de contraceptifs et de Levothyroxine (T4)<\/span><\/h2>\n\n\n\n<p>Je pense que le contr\u00f4le des naissances est une grave injustice pour les femmes parce qu'il ne fait que masquer les sympt\u00f4mes de troubles tels que l'endom\u00e9triose, le syndrome pr\u00e9menstruel et le syndrome des ovaires polykystiques, au lieu de traiter le trouble sous-jacent. La l\u00e9vothyroxine (T4) peut \u00eatre utilis\u00e9e de la m\u00eame mani\u00e8re pour masquer les sympt\u00f4mes d'un trouble thyro\u00efdien au lieu de traiter la cause profonde du dysfonctionnement. Si nous rempla\u00e7ons simplement les hormones thyro\u00efdiennes sans rechercher et traiter le trouble sous-jacent, nous ne ferons qu'aggraver le dysfonctionnement.<\/p>\n\n\n\n<p>Afin de diagnostiquer la cause profonde du dysfonctionnement de la thyro\u00efde, des analyses suppl\u00e9mentaires pourraient s'av\u00e9rer tr\u00e8s utiles : Cortisol salivaire et DHEA sur 24 heures et DHEA-S s\u00e9rique (pour la fonction surr\u00e9nalienne), tableau m\u00e9tabolique complet (y compris la fonction h\u00e9patique), h\u00e9mogramme complet (CBC pour l'an\u00e9mie), fer et ferritine, B-12, zinc, vit D 25-OH, s\u00e9l\u00e9nium, iode (urine du premier matin), prolactine, FSH et LH, globuline liant les hormones sexuelles (SHBG), \u0153strog\u00e8ne du troisi\u00e8me jour du cycle et \u0153strog\u00e8ne et progest\u00e9rone du septi\u00e8me jour apr\u00e8s l'ovulation. En outre, une \u00e9chographie est n\u00e9cessaire en cas d'hypertrophie de la thyro\u00efde ou de nodule.&nbsp;<\/p>\n\n\n\n<p>Si l'un des sympt\u00f4mes d\u00e9crits dans cet article vous semble familier, cela vaut la peine de faire part de vos inqui\u00e9tudes \u00e0 votre m\u00e9decin.
