En g\u00e9n\u00e9ral, si vous avez des r\u00e8gles et que vous ne prenez pas de contraception hormonale, vous \u00eates en train d'ovuler. La plupart du temps. Mais parfois, une femme peut conna\u00eetre ce que l'on appelle un cycle anovulatoire. (Il convient de noter que cette notion est diff\u00e9rente de celle d'\"h\u00e9morragie de privation\", qui est secondaire \u00e0 l'arr\u00eat de la progest\u00e9rone). <\/p>\n\n\n\n<p>D\u00e9cortiquons les diff\u00e9rents types de conditions dans lesquelles les femmes peuvent souffrir d'anovulation en explorant les situations de quelques sc\u00e9narios diff\u00e9rents.<\/p>\n\n\n\n<h2 class=\"wp-block-heading\" id=\"h-anovulation-due-to-stress\"><span id=\"anovulation-due-to-stress\">Anovulation due au stress<\/span><\/h2>\n\n\n\n<p>Imaginons une jeune femme, Annie, qui ne prend aucune forme de contraception hormonale, dont les cycles sont g\u00e9n\u00e9ralement r\u00e9guliers et qui est \u00e9tudiante de premier cycle. Un mois, juste avant les examens et alors qu'elle cherche une nouvelle colocation, elle a ce qu'elle pense \u00eatre ses r\u00e8gles. Ce saignement est beaucoup plus l\u00e9ger que d'habitude. Ce qu'Annie ne sait pas, c'est qu'il s'agit en fait d'un saignement de rupture. Dans tout le stress li\u00e9 \u00e0 l'ach\u00e8vement des projets de fin de semestre et \u00e0 la recherche d'une nouvelle colocation, son corps a subrepticement \"saut\u00e9\" l'ovulation ce mois-ci.&nbsp;<\/p>\n\n\n\n<p>Que se passe-t-il pour Annie sur le plan hormonal ? Le stress qu'elle a subi a en fait \"envoy\u00e9\" \u00e0 ses ovaires le signal d'arr\u00eater la production de follicules ce mois-ci. Dans un cycle normal, il n'y a pas qu'un seul follicule qui est recrut\u00e9 - il y en a plusieurs qui \"attendent dans les coulisses\" - et un follicule dominant est finalement choisi pour lib\u00e9rer un ovule. Mais lorsqu'un follicule dominant n'est pas choisi en raison d'une situation de stress aigu, les follicules aspirants peuvent attendre patiemment en lib\u00e9rant de petites quantit\u00e9s d'\u0153strog\u00e8nes. <\/p>\n\n\n\n<p>En r\u00e9ponse aux niveaux continuellement bas d'\u0153strog\u00e8nes, l'endom\u00e8tre (la paroi de l'ut\u00e9rus) s'\u00e9paissit, mais seulement l\u00e9g\u00e8rement. Quelques semaines plus tard, et bien que l'ovulation n'ait pas eu lieu, l'endom\u00e8tre s'\u00e9paissit. Il s'agit en partie d'une r\u00e9ponse immunitaire, et peut \u00e9galement \u00eatre li\u00e9 \u00e0 un flux sanguin inad\u00e9quat vers l'endom\u00e8tre folliculaire \u00e9paissi, afin d'\u00e9viter une accumulation inutile d'endom\u00e8tre sans aucune perspective qu'un ovule f\u00e9cond\u00e9 vienne s'y enfouir de sit\u00f4t. Et comme les niveaux d'\u0153strog\u00e8nes \u00e9taient faibles, les saignements qui en r\u00e9sultent sont l\u00e9gers (bien que, si l'am\u00e9norrh\u00e9e se prolongeait, les saignements pourraient commencer \u00e0 devenir abondants).&nbsp;<\/p>\n\n\n\n<p>Le cycle anovulatoire d'Annie n'est pas inqui\u00e9tant, il s'agit plut\u00f4t d'une r\u00e9action aigu\u00eb \u00e0 une situation aigu\u00eb. Nina est m\u00e8re de trois enfants et, bien que ses cycles soient g\u00e9n\u00e9ralement r\u00e9guliers, ils semblent de plus en plus espac\u00e9s depuis l'ann\u00e9e derni\u00e8re. Elle a une quarantaine d'ann\u00e9es et jure qu'elle est bien trop jeune pour la m\u00e9nopause. <\/p>\n\n\n\n<p>Si la quarantaine est certainement un \u00e2ge pr\u00e9coce pour la m\u00e9nopause, il n'en est pas de m\u00eame pour les autres. <a href=\"https:\/\/naturalwomanhood.org\/fr\/graphique-perimenopause-methodes-de-sensibilisation-a-la-fertilite-signes-deroutants-cycle-changements-hormonaux\/\"><em>pas<\/em> trop jeune pour la p\u00e9rim\u00e9nopause<\/a>La m\u00e9nopause est une p\u00e9riode qui peut pr\u00e9c\u00e9der d'une d\u00e9cennie enti\u00e8re l'arr\u00eat r\u00e9el des cycles. Nina soup\u00e7onne qu'elle n'ovule plus, mais ses r\u00e8gles sont tout aussi abondantes que les ann\u00e9es pr\u00e9c\u00e9dentes, voire plus.&nbsp;<\/p>\n\n\n\n<p>Sur le plan hormonal, Nina n'ovule pas en raison de la fluctuation de ses niveaux d'hormones. Son taux d'\u0153strog\u00e8nes est peut-\u00eatre encore relativement \u00e9lev\u00e9, mais il n'est pas tr\u00e8s \u00e9lev\u00e9. <em>assez <\/em>pour lib\u00e9rer un ovule. Ces niveaux \u00e9lev\u00e9s se maintiennent juste assez longtemps pour permettre \u00e0 l'endom\u00e8tre de se d\u00e9velopper. Mais h\u00e9las, aucun ovule ne vient, et finalement, lorsque l'\u0153strog\u00e8ne se rend compte qu'il n'atteindra pas un niveau suffisamment \u00e9lev\u00e9 pour conduire \u00e0 l'ovulation, le corps d\u00e9cide \u00e0 nouveau de se d\u00e9barrasser de l'endom\u00e8tre. (\u00c0 cet \u00e2ge, il est \u00e9galement important de noter que Nina n'a plus d'ovules, car les femmes naissent avec tous les ovules qu'elles auront jamais).<\/p>\n\n\n\n<p>Bien que la p\u00e9rim\u00e9nopause soit une partie tout \u00e0 fait normale du spectre reproductif, Nina peut souhaiter travailler avec un praticien pour traiter les sympt\u00f4mes qui d\u00e9coulent souvent de niveaux \u00e9lev\u00e9s et persistants d'\u0153strog\u00e8nes, tels que l'insomnie, les bouff\u00e9es de chaleur, les changements de m\u00e9tabolisme et les maux de t\u00eate.<\/p>\n\n\n\n<p>Les autres saisons de transition pendant lesquelles les femmes peuvent naturellement conna\u00eetre des cycles anovulatoires sont les suivantes <a href=\"https:\/\/naturalwomanhood.org\/fr\/fabm-postpartum-nfp-lallaitement-previent-il-la-grossesse\/\">l'allaitement<\/a>, pr\u00e9coce <a href=\"https:\/\/naturalwomanhood.org\/fr\/cycle-pleine-conscience-ce-qui-se-passe-quand-on-enseigne-la-connaissance-de-la-fertilite-a-des-adolescentes\/\">pubert\u00e9<\/a>et quand <a href=\"https:\/\/naturalwomanhood.org\/fr\/se-debarrasser-de-la-pilule-4-moyens-cles-pour-une-transition-saine\/\">l'arr\u00eat d'une contraception hormonale<\/a>.<\/p>\n\n\n\n<h2 class=\"wp-block-heading\" id=\"h-anovulation-on-the-birth-control-pill\"><span id=\"anovulation-on-the-birth-control-pill\">Anovulation sous pilule contraceptive<\/span><\/h2>\n\n\n\n<p>Prenons maintenant le cas de Camilla. Camilla a une trentaine d'ann\u00e9es et prend la pilule combin\u00e9e, qui comprend \u00e0 la fois des \u0153strog\u00e8nes artificiels et de la progest\u00e9rone artificielle, appel\u00e9e progestatif. Elle re\u00e7oit une petite dose quotidienne de ces deux hormones synth\u00e9tiques lorsqu'elle prend sa pilule tous les matins. Mais les niveaux sont bien inf\u00e9rieurs \u00e0 ceux que son corps produirait naturellement (<a href=\"https:\/\/naturalwomanhood.org\/fr\/difference-entre-les-hormones-synthetiques-et-les-hormones-naturelles-controle-des-naissances-bioidentique-nuisible-sur-affecter-le-corps-progesterone-progestatif-oestrogene-08192018\/\">et les hormones elles-m\u00eames ne sont pas bio-identiques<\/a> soit). <\/p>\n\n\n\n<p>Toutes les trois semaines, elle prend une \"pilule placebo\" qui est essentiellement une pilule de sucre ne contenant pas d'hormones. Les saignements que Camilla subit lorsqu'elle prend la pilule placebo sont l\u00e9gers et il ne s'agit pas de r\u00e8gles, car Camilla n'ovule pas en raison de sa contraception.&nbsp;<\/p>\n\n\n\n<p>En un sens, <a href=\"https:\/\/naturalwomanhood.org\/fr\/controle-hormonal-des-naissances-reponse-au-stress-chronique-anxiete-axe-apa-2019\/\">Camilla est dans la m\u00eame situation qu'Annie<\/a>-mais celle de Camilla est induite synth\u00e9tiquement. La pilule combin\u00e9e qu'elle prend envoie un signal n\u00e9gatif \u00e0 son hypophyse pour arr\u00eater la production endog\u00e8ne (naturelle) d'\u0153strog\u00e8ne et de progest\u00e9rone en donnant \u00e0 son corps une petite dose r\u00e9guli\u00e8re d'\u0153strog\u00e8ne et de progest\u00e9rone artificiels. L'endom\u00e8tre de Camilla prolif\u00e8re l\u00e9g\u00e8rement en raison des faibles niveaux d'\u0153strog\u00e8nes synth\u00e9tiques, puis lorsque le corps re\u00e7oit le signal des pilules de sucre, il \u00e9limine l'endom\u00e8tre.&nbsp;<\/p>\n\n\n\n<h2 class=\"wp-block-heading\" id=\"h-anovulation-from-pcos-and-other-hormonal-disorders\"><span id=\"anovulation-from-pcos-and-other-hormonal-disorders\">Anovulation due au SOPK et \u00e0 d'autres troubles hormonaux<\/span><\/h2>\n\n\n\n<p>Enfin, nous avons Tamara. Tamara a une vingtaine d'ann\u00e9es et a toujours eu des r\u00e8gles irr\u00e9guli\u00e8res, douloureuses et longues. Elle peut passer des mois sans saigner et doit parfois s'absenter de son travail tant ses crampes sont fortes. Tamara a \u00e9galement <a href=\"https:\/\/naturalwomanhood.org\/fr\/le-pcos-provoque-t-il-une-depression-les-symptomes-de-linfertilite-19\/\">le syndrome des ovaires polykystiques<\/a> (SOPK) et une multitude de <a href=\"https:\/\/naturalwomanhood.org\/fr\/lien-entre-les-contraceptifs-et-les-maladies-auto-immunes-030619\/\">les maladies auto-immunes<\/a>. Certains saignements sont l\u00e9gers et d'autres sont intol\u00e9rables.<\/p>\n\n\n\n<p>La situation hormonale de Tamara est similaire \u00e0 celle de Nina, mais elle n'est pas li\u00e9e \u00e0 un stade du spectre reproductif. Dans le cas de Tamara, ce sont ses troubles endocriniens qui provoquent une ovulation irr\u00e9guli\u00e8re et, par cons\u00e9quent, des saignements irr\u00e9guliers qui r\u00e9sultent parfois de l'absence d'ovulation. Des niveaux \u00e9lev\u00e9s, persistants et incontr\u00f4l\u00e9s d'\u0153strog\u00e8nes qui n'entra\u00eenent pas d'ovulation provoquent une forte croissance de l'endom\u00e8tre, qui provoque ensuite des saignements abondants, qu'il y ait eu ou non ovulation.&nbsp;<\/p>\n\n\n\n<p>La situation de Tamara est pr\u00e9occupante et <a href=\"https:\/\/naturalwomanhood.org\/fr\/pcos-diagnostic-symptomes-comment-diagnostiquer-infertilite-gestion-traitement-tomber-enceinte-071818\/\">justifie une discussion avec un professionnel de la sant\u00e9<\/a>. Elle aurait certainement int\u00e9r\u00eat \u00e0 ce qu'un praticien examine ses taux d'hormones et tente d'\u00e9valuer comment g\u00e9rer \u00e0 la fois son SOPK et ses troubles auto-immuns.
