Como usuaria de los M\u00e9todos Basados en el Conocimiento de la Fertilidad para trazar mi ciclo, adem\u00e1s hago un seguimiento de los biomarcadores hormonales, y la combinaci\u00f3n de estas habilidades me ha permitido poner mis extra\u00f1as excursiones de glucosa en un contexto de ciclo a medida que suceden.<\/p>\n<h2 id=\"what-does-the-research-say\">\u00bfQu\u00e9 dicen los estudios?<\/h2>\n<p>En 1942, el investigador H.I. Cramer public\u00f3 sus hallazgos sobre los efectos del ciclo menstrual en los niveles de glucosa. Sin embargo, el grado en que las hormonas del ciclo interfieren en la acci\u00f3n de la insulina sigue siendo poco conocido, poco estudiado y sujeto a la singularidad del ciclo natural de cada mujer.<\/p>\n<p>En la actualidad, es una pr\u00e1ctica habitual que los m\u00e9dicos receten a las mujeres diab\u00e9ticas de tipo 1 la p\u00edldora anticonceptiva cuando las variaciones c\u00edclicas de la glucosa se vuelven problem\u00e1ticas. Las hormonas sint\u00e9ticas sustituyen la variabilidad por la previsibilidad, interrumpiendo el ciclo natural. Por ello, siguen sin estar claros los mecanismos biol\u00f3gicos exactos que causan las interferencias c\u00edclicas en la respuesta a la insulina.<\/p>\n<p>Los estudios han arrojado resultados contradictorios espec\u00edficamente para el estr\u00f3geno en su relaci\u00f3n con la insulina; se ha implicado al estr\u00f3geno en la resistencia a la insulina al tiempo que se ha identificado que ejerce una regulaci\u00f3n positiva sobre la acci\u00f3n de la insulina. Tambi\u00e9n se ha encontrado una relaci\u00f3n positiva entre la hormona foliculoestimulante (FSH) y la sensibilidad a la insulina. Una tercera hormona, la progesterona, parece alterar la captaci\u00f3n de glucosa y provocar una resistencia temporal a la insulina durante la fase l\u00fatea y el embarazo. Este tipo de informaci\u00f3n cient\u00edfica en bruto merece un estudio m\u00e1s profundo. Pero hasta que no se ponga en contexto para las pacientes, todos estos cambios hormonales resultar\u00e1n in\u00fatiles.<\/p>\n<h2 id=\"sympto-thermal-charting-a-fabm-way-to-anticipate-cyclical-changes-in-insulin-sensitivity\">Gr\u00e1ficos sintot\u00e9rmicos: Una Forma FABM De Anticipar Los Cambios C\u00edclicos En La Sensibilidad A La Insulina<\/h2>\n<p>Como no quer\u00eda tomar la p\u00edldora anticonceptiva, empec\u00e9 a utilizar el gr\u00e1fico FABM para saber cu\u00e1ndo cambiaba mi sensibilidad a la insulina. Esto es posible porque la presencia de las hormonas del ciclo se manifiesta en la calidad del fluido cervical y en los niveles de temperatura al despertar.<\/p>\n<p>Hago un seguimiento tanto del fluido cervical como de los biomarcadores de la temperatura de vigilia siguiendo los criterios del m\u00e9todo sintot\u00e9rmico. Mis gr\u00e1ficos a lo largo del tiempo, en combinaci\u00f3n con los gr\u00e1ficos de mi monitor continuo de glucosa, revelaron informaci\u00f3n sobre los efectos que las hormonas del ciclo ten\u00edan en mi sensibilidad a la insulina. Con la ayuda de mi enfermera especializada en diabetes, utilic\u00e9 esta informaci\u00f3n para formular cuatro perfiles de dosificaci\u00f3n de insulina aplicables en distintos momentos de mi ciclo.<\/p>\n<h2 id=\"my-insulin-profiles-across-my-cycle\">Mis perfiles de insulina a lo largo del ciclo<\/h2>\n<p>Lo que sigue es una descripci\u00f3n en profundidad de c\u00f3mo el seguimiento de mi ciclo menstrual con el m\u00e9todo sintot\u00e9rmico afecta a mis perfiles de dosificaci\u00f3n de insulina. Aunque estos detalles pueden sonar a chino para una persona normal sin diabetes, quienes s\u00ed la padecen pueden utilizar esta informaci\u00f3n, junto con su profesional sanitario, para ver c\u00f3mo puede aplicarse a sus situaciones individuales.<\/p>\n<p>Mi<strong> Perfil est\u00e1ndar<\/strong> se aplica a todas las situaciones en las que mi respuesta a la insulina es relativamente predecible. Se basa en el ejercicio planificado y en una combinaci\u00f3n precisa de carbohidratos, y no cubre los picos hormonales. Lo utilizo en los d\u00edas del ciclo preovulatorio en los que mis dos marcadores sintot\u00e9rmicos no indican un alto estado de fertilidad, es decir, fluido cervical de calidad f\u00e9rtil media (barra azul corta en mi gr\u00e1fico), o ninguno. Tambi\u00e9n lo aplico en los d\u00edas de fase l\u00fatea en los que observo un pico de estr\u00f3genos (barra azul) en mi gr\u00e1fico.<\/p>\n<p>Mi <strong>Perfil de alta sensibilidad<\/strong> est\u00e1 dise\u00f1ado para los d\u00edas del ciclo en los que veo flujo cervical de calidad altamente f\u00e9rtil (barra azul larga en mi gr\u00e1fico) antes de un intento de ovulaci\u00f3n, y tambi\u00e9n durante la menstruaci\u00f3n a partir del d\u00eda 2 del ciclo. Las temperaturas basales bajas en correlaci\u00f3n con el fluido cervical de calidad altamente f\u00e9rtil marcan el pico de actividad del estr\u00f3geno y reduzco mi insulina basal en 25% y mi insulina en bolo en 50% para evitar eventos hipogluc\u00e9micos debido al aumento de la sensibilidad a la insulina.<\/p>\n<p><strong>Ovulaci\u00f3n<\/strong>el principal acontecimiento del ciclo menstrual, marca un punto de inflexi\u00f3n en mi sensibilidad a la insulina.<\/p>\n<p>Mi <strong>Perfil postovulatorio<\/strong> tiene en cuenta el aumento de progesterona que se produce tras la ovulaci\u00f3n y que se opone al estr\u00f3geno durante la fase l\u00fatea. La progesterona s\u00e9rica puede fluctuar considerablemente en un per\u00edodo de 24 horas, por lo que yo establezco mi insulina basal 20% m\u00e1s alta, pero aumento mi insulina en bolo de forma incremental a lo largo del d\u00eda hasta un m\u00e1ximo de 40% m\u00e1s alta por la noche, cuando mi progesterona tiende a alcanzar su punto m\u00e1ximo.<\/p>\n<p>Mi <strong>Perfil de alta demanda<\/strong> cubre la resistencia temporal a la insulina que experimento al final de mis fases l\u00fateas y que se extiende hasta el primer d\u00eda de un nuevo ciclo. Aumento mis dosis de insulina basal y de insulina en bolo en 30% cada una. Los niveles de progesterona pueden bajar brevemente durante mi fase l\u00fatea, lo que se manifiesta en un d\u00eda de menor temperatura al despertar y en la aparici\u00f3n de flujo cervical de tipo f\u00e9rtil. Interrumpo mi Perfil de Alta Demanda en esos d\u00edas y aplico mi Perfil Est\u00e1ndar. Esta estrategia me ha ayudado a eliminar los episodios hipogluc\u00e9micos graves durante mi fase l\u00fatea. Dejo de utilizar mi Perfil de Alta Demanda en cuanto mi temperatura en vigilia desciende a niveles preovulatorios, normalmente el d\u00eda 2 del ciclo.<\/p>\n<p>Mis gr\u00e1ficos utilizan un lenguaje visual de iconos para representar el flujo cervical en categor\u00edas sencillas, en relaci\u00f3n con la actividad hormonal. El icono de la barra azul corta representa el flujo cervical de calidad f\u00e9rtil media, que indica cierta actividad de los estr\u00f3genos. El icono de la barra azul larga expresa el fluido cervical de calidad altamente f\u00e9rtil, resultado de los niveles m\u00e1ximos de estr\u00f3geno. Despu\u00e9s del d\u00eda pico y de la ovulaci\u00f3n, el icono de la nube amarilla significa fluido cervical bajo la influencia de la progesterona asociado a una resistencia menor o mayor a la insulina, dependiendo de lo alto que suba mi temperatura.<\/p>\n<h2 id=\"why-sympto-thermal\">\u00bfPor qu\u00e9 Sympto-Thermal?<\/h2>\n<p>La clave para aplicar mis perfiles c\u00edclicos de insulina es la confirmaci\u00f3n de la ovulaci\u00f3n. Sin una confirmaci\u00f3n concluyente de la ovulaci\u00f3n, aumentar mi dosis de insulina supondr\u00eda un riesgo indebido. Adem\u00e1s, como diab\u00e9tica me encuentro en una edad premenop\u00e1usica en la que los ciclos pueden presentar irregularidades. El m\u00e9todo sintot\u00e9rmico me permite ver claramente mis ondas f\u00e9rtiles a medida que se producen cada vez que mi cuerpo hace un intento de ovulaci\u00f3n, y confirmar la ovulaci\u00f3n una vez que tiene \u00e9xito. Incluso si el intento fracasa, me pone temporalmente en un estado de mayor sensibilidad a la insulina. Si el intento de ovulaci\u00f3n no tiene \u00e9xito, simplemente vuelvo a mi perfil est\u00e1ndar. Una vez que tengo un cambio sostenido en la calidad del fluido cervical, as\u00ed como un aumento de las temperaturas, se confirma la ovulaci\u00f3n y puedo aplicar mi perfil de insulina post-ovulatorio. Todos mis perfiles permiten correcciones, pero la idea es eliminar las fluctuaciones causadas por cambios hormonales identificables.<\/p>\n<p>El gr\u00e1fico #79 muestra una ovulaci\u00f3n directa. Saber por qu\u00e9 y cu\u00e1ndo cambia mi sensibilidad a la insulina ha cambiado mi diabetes de tipo 1 al conseguir curvas de glucosa m\u00e1s planas y reducir el estr\u00e9s asociado a las excursiones de glucosa.<\/p>\n<p>Aunque compleja, la gesti\u00f3n de las influencias hormonales en el control gluc\u00e9mico es posible sin hormonas sint\u00e9ticas como las de los anticonceptivos hormonales.
