Shortly afterwards, I had a blood test at my doctor\u2019s office that confirmed the positive result. At the time, I didn\u2019t stop to think about how these tests actually worked, but both the at-home urine test and the blood test were looking for the same thing: elevated levels of the hormone known as human chorionic gonadotropin (hCG).<\/p>\n\n\n\n<p class=\"wp-block-paragraph\">So what is hCG, and what makes it such a reliable indicator of pregnancy? In this article, we&#8217;ll discuss what hCG is, what it does, why it&#8217;s monitored in early pregnancy (and after a miscarriage), and the various ways it&#8217;s used in treating infertility. <\/p>\n\n\n\n<h2 id=\"h-what-is-hcg\" class=\"wp-block-heading\"><span id=\"what-is-hcg\">What is hCG?<\/span><\/h2>\n\n\n\n<p class=\"wp-block-paragraph\">Human chorionic gonadotropin (hCG) is commonly known as the \u201c<a href=\"https:\/\/americanpregnancy.org\/getting-pregnant\/hcg-levels\/\" target=\"_blank\" rel=\"noreferrer noopener\">pregnancy hormone<\/a>.\u201d It is the <a href=\"https:\/\/my.clevelandclinic.org\/health\/body\/22489-human-chorionic-gonadotropin\" target=\"_blank\" rel=\"noreferrer noopener\">first hormone<\/a> produced by the placenta, which begins to form as soon as the embryo implants in the uterine lining.&nbsp;<\/p>\n\n\n\n<p class=\"wp-block-paragraph\">hCG plays an essential role in sustaining early pregnancy by signaling the body to start producing more <a href=\"https:\/\/naturalwomanhood.org\/what-is-estrogen-hormone\/\" target=\"_blank\" rel=\"noreferrer noopener\">estrogen<\/a> and <a href=\"https:\/\/naturalwomanhood.org\/fam-basics-progesterone\/\" target=\"_blank\" rel=\"noreferrer noopener\">progesterone<\/a>. By week 10 or 11 of pregnancy, hCG levels taper off, though it still remains elevated above non-pregnancy levels until after giving birth.<\/p>\n\n\n\n<figure class=\"wp-block-pullquote\"><blockquote><p>hCG plays an essential role in sustaining early pregnancy by signaling the body to start producing more estrogen and progesterone. By week 10 or 11 of pregnancy, hCG levels taper off, though it still remains elevated above non-pregnancy levels until after giving birth.<\/p><\/blockquote><\/figure>\n\n\n\n<h2 id=\"h-the-role-of-hcg-in-treating-infertility-nbsp\" class=\"wp-block-heading\"><span id=\"the-role-of-hcg-in-treating-infertility\">The role of hCG in treating infertility&nbsp;<\/span><\/h2>\n\n\n\n<p class=\"wp-block-paragraph\">hCG is used in both assisted reproductive technology (ART) procedures such as in vitro fertilization (IVF), as well as in <a href=\"https:\/\/www.factsaboutfertility.org\/fertility-and-the-female-cycle-an-interview-with-a-couple\/\" target=\"_blank\" rel=\"noreferrer noopener\">NaProTechnology protocols<\/a> in order to help couples struggling with infertility to conceive.&nbsp;<\/p>\n\n\n\n<p class=\"wp-block-paragraph\">IVF protocols utilize hCG (sometimes in combination with other injections) to stimulate the growth of multiple follicles and <a href=\"https:\/\/pmc.ncbi.nlm.nih.gov\/articles\/PMC5454971\/#sec2-ijms-18-01059:~:text=the%20invading%20blastocyst.-,2.%20hCG%20and%20Assisted%20Reproduction,-Controlled%20ovarian%20stimulation\" target=\"_blank\" rel=\"noreferrer noopener\">induce ovulation<\/a> in order to <a href=\"https:\/\/naturalwomanhood.org\/egg-retrieval-ivf-freezing-donation\/\" target=\"_blank\" rel=\"noreferrer noopener\">harvest as many eggs as possible<\/a> to be later fertilized in a lab [1].&nbsp;<\/p>\n\n\n\n<p class=\"wp-block-paragraph\">By contrast, NaProTechnology and Restorative Reproductive Medicine (RRM) procedures time hCG injections around a woman\u2019s natural ovulation, helping the dominant follicle to open enough to release an egg (which may be helpful in cases of infertility due to weak ovulation or suspected <a href=\"https:\/\/naturalwomanhood.org\/luteinized-unruptured-follicle\/\" target=\"_blank\" rel=\"noreferrer noopener\">luteinized unruptured follicles<\/a>, or LUFs).&nbsp;&nbsp;&nbsp;<\/p>\n\n\n\n<h2 id=\"h-healthy-levels-of-hcg-during-pregnancy\" class=\"wp-block-heading\"><span id=\"healthy-levels-of-hcg-during-pregnancy\">Healthy levels of hCG during pregnancy<\/span><\/h2>\n\n\n\n<p class=\"wp-block-paragraph\">hCG levels are typically measured in milli-international units per milliliter (mIU\/ml). Outside of pregnancy, hCG levels are very low\u2014typically 5 mIU\/ml or less. An hCG level of more than 25 mIU\/ml indicates a pregnancy. At its peak around week 10 of a pregnancy, hCG can reach up to 288,000 mIU\/ml.&nbsp;<\/p>\n\n\n\n<p class=\"wp-block-paragraph\">For most women, hCG reaches 25 mIU\/ml between 11 and 14 days after conception. This is <a href=\"https:\/\/naturalwomanhood.org\/wondering-whether-you-might-be-pregnant-heres-how-you-can-know-for-sure\/\" target=\"_blank\" rel=\"noreferrer noopener\">around the time<\/a> when most women are able to get an accurate pregnancy test result: most standard at-home pregnancy tests have a <a href=\"https:\/\/www.avawomen.com\/avaworld\/best-pregnancy-test\/\" target=\"_blank\" rel=\"noreferrer noopener\">threshold<\/a> of about 22 mIU\/ml, though some early detection tests claim to detect hCG as low as 6.3 mIU\/ml.&nbsp;&nbsp;<\/p>\n\n\n\n<p class=\"wp-block-paragraph\">There is wide variation among individual women when it comes to hCG levels at different stages of pregnancy. In general, however, hCG levels double every 72 hours throughout the first 8-10 weeks of pregnancy. This <a href=\"https:\/\/americanpregnancy.org\/getting-pregnant\/hcg-levels\/\">chart<\/a> shows the expected range of hCG levels throughout pregnancy.<\/p>\n\n\n\n<figure class=\"wp-block-table\"><table class=\"has-fixed-layout\"><tbody><tr><td><strong>Weeks since last menstrual period<\/strong><\/td><td><strong>hCG levels (mIU\/mL)<\/strong><\/td><\/tr><tr><td>3<\/td><td>5-50<\/td><\/tr><tr><td>4<\/td><td>5-426<\/td><\/tr><tr><td>5<\/td><td>18-7,340<\/td><\/tr><tr><td>6<\/td><td>1,080-56,500<\/td><\/tr><tr><td>7 to 8<\/td><td>7,650-229,000<\/td><\/tr><tr><td>9 to 12<\/td><td>25,700-288,000<\/td><\/tr><tr><td>13 to 16<\/td><td>13,300-254,000<\/td><\/tr><tr><td>17 to 24<\/td><td>4,060-165,400<\/td><\/tr><tr><td>25 to 40<\/td><td>3,640-117,000<\/td><\/tr><\/tbody><\/table><\/figure>\n\n\n\n<p class=\"wp-block-paragraph\">Beyond an initial test to confirm pregnancy, most healthcare professionals don\u2019t check hCG levels again unless there is concern about it being too high or low, or if there is evidence of possible miscarriage.<\/p>\n\n\n\n<h2 id=\"h-when-hcg-is-outside-the-normal-range\" class=\"wp-block-heading\"><span id=\"when-hcg-is-outside-the-normal-range\">When hCG is outside the normal range<\/span><\/h2>\n\n\n\n<p class=\"wp-block-paragraph\">There are a number of reasons why a blood test could indicate that hCG levels are outside the normal range.&nbsp;<\/p>\n\n\n\n<p class=\"wp-block-paragraph\">Sometimes hCG levels that appear too low or too high indicate that a mistake was made estimating when the pregnancy began. Like the calculation of due dates, the guidelines for hCG levels are based on the timing of the last menstrual period before pregnancy. This means that they are <em>not<\/em> necessarily aligned with the actual date of conception, especially since many women <a href=\"https:\/\/naturalwomanhood.org\/menstrual-cycle-health-data\/\">ovulate earlier or later<\/a> in their cycles than the standard 14-day point.&nbsp;<\/p>\n\n\n\n<p class=\"wp-block-paragraph\">In the case of misdating the pregnancy, having hCG levels tested again about 48 hours later can determine whether they are increasing at an appropriate rate. hCG on its own should not be used to determine gestational age.&nbsp;<\/p>\n\n\n\n<h3 id=\"h-high-hcg-nbsp\" class=\"wp-block-heading\"><span id=\"high-hcg\">High hCG&nbsp;<\/span><\/h3>\n\n\n\n<p class=\"wp-block-paragraph\">High hCG levels can also result from having more than one embryo present\u2014an early indicator of twins!<\/p>\n\n\n\n<p class=\"wp-block-paragraph\">In other cases, abnormally high hCG levels indicate a <a href=\"https:\/\/naturalwomanhood.org\/molar-pregnancy\/\" target=\"_blank\" rel=\"noreferrer noopener\">molar pregnancy<\/a>, a condition where two sperm fertilize the same egg.&nbsp;<\/p>\n\n\n\n<h3 id=\"h-low-hcg\" class=\"wp-block-heading\"><span id=\"low-hcg\">Low hCG<\/span><\/h3>\n\n\n\n<p class=\"wp-block-paragraph\">Low hCG levels can be caused by miscarriage, ectopic pregnancy, and blighted ovum.<\/p>\n\n\n\n<p class=\"wp-block-paragraph\">A miscarriage will result in low hCG levels that continue to decrease, as the placenta no longer produces hormones to support a developing baby.<\/p>\n\n\n\n<p class=\"wp-block-paragraph\">In an <a href=\"https:\/\/naturalwomanhood.org\/how-do-you-treat-ectopic-pregnancy\/\" target=\"_blank\" rel=\"noreferrer noopener\">ectopic pregnancy<\/a>, the embryo develops outside of the uterus, usually in the fallopian tube. Approximately <a href=\"https:\/\/ectopic.org.uk\/what-is-an-ectopic-pregnancy\" target=\"_blank\" rel=\"noreferrer noopener\">1 to 2 percent<\/a> of pregnancies are ectopic. In most cases, hCG levels will initially increase <a href=\"https:\/\/pubmed.ncbi.nlm.nih.gov\/2284900\/\" target=\"_blank\" rel=\"noreferrer noopener\">only slowly<\/a>, and if the developing embryo dies, hCG levels will decrease [2].&nbsp;<\/p>\n\n\n\n<p class=\"wp-block-paragraph\">A <a href=\"https:\/\/americanpregnancy.org\/pregnancy\/blighted-ovum\/\" target=\"_blank\" rel=\"noreferrer noopener\">blighted ovum<\/a> describes a pregnancy where the fertilized egg travels to the uterus and attaches to the endometrium, and the gestational sac begins to form, but the embryo does not develop due to chromosomal abnormalities. A blighted ovum is the cause of about half of all first trimester miscarriages, and it has been estimated that over <a href=\"https:\/\/www.researchgate.net\/publication\/329929927_Prevalence_of_blighted_ovum_in_first_trimester_of_pregnancy_a_hospital_based_study\" target=\"_blank\" rel=\"noreferrer noopener\">15% of pregnancies<\/a> involve a blighted ovum [3]. While hCG levels are lower than usual, they can continue to rise, so the presence of a blighted ovum is often unknown until an ultrasound shows an empty gestational sac.<\/p>\n\n\n\n<h2 id=\"h-not-a-diagnostic-tool\" class=\"wp-block-heading\"><span id=\"not-a-diagnostic-tool\">Not a diagnostic tool<\/span><\/h2>\n\n\n\n<p class=\"wp-block-paragraph\">hCG levels are never used on their own to diagnose a condition such as a blighted ovum, ectopic pregnancy, or molar pregnancy. The American Pregnancy Association warns against \u201cmaking too much of hCG numbers. A normal pregnancy may have low hCG levels and result in a perfectly healthy baby.\u201d<\/p>\n\n\n\n<h2 id=\"h-hcg-and-cancer\" class=\"wp-block-heading\"><span id=\"hcg-and-cancer\">hCG and cancer<\/span><\/h2>\n\n\n\n<p class=\"wp-block-paragraph\">Elevated hCG levels are also caused by some types of cancer. The highest levels of hCG are produced by <a href=\"https:\/\/www.hopkinsmedicine.org\/health\/conditions-and-diseases\/gestational-trophoblastic-disease\" target=\"_blank\" rel=\"noreferrer noopener\">choriocarcinoma<\/a>, a type of tumor that usually results from a molar pregnancy in which cells have turned cancerous. This is very rare: only about 1 in 1,000 pregnancies are molar, and fewer than 20% of these turn cancerous.&nbsp;<\/p>\n\n\n\n<p class=\"wp-block-paragraph\">Other <a href=\"https:\/\/neurology.testcatalog.org\/show\/BHCG\" target=\"_blank\" rel=\"noreferrer noopener\">types of cancer<\/a> that can produce hCG include <a href=\"https:\/\/naturalwomanhood.org\/does-birth-control-prevent-or-cause-cancer\/\" target=\"_blank\" rel=\"noreferrer noopener\">ovarian cancer<\/a> in women and testicular cancer in men. More rarely, hCG levels can be raised by liver, neuroendocrine, <a href=\"https:\/\/naturalwomanhood.org\/birth-control-abortion-and-breast-cancer-risk\/\" target=\"_blank\" rel=\"noreferrer noopener\">breast<\/a>, pancreatic, and gastrointestinal cancers.<\/p>\n\n\n\n<h2 id=\"h-the-bottom-line\" class=\"wp-block-heading\"><span id=\"the-bottom-line\">The bottom line<\/span><\/h2>\n\n\n\n<p class=\"wp-block-paragraph\">The presence of elevated hCG in urine triggers a positive pregnancy test result\u2014and does much more!
