By 2020, that theoretical woman\u2019s theoretical granddaughter wouldn\u2019t give birth for the first time until age 27. In fact, during 2020, 19% of all pregnancies and 11% of first time pregnancies were in women aged 35 and over, according to the <a href=\"https:\/\/stacks.cdc.gov\/view\/cdc\/112078\" target=\"_blank\" rel=\"noreferrer noopener\">CDC<\/a>. <a href=\"https:\/\/pmc.ncbi.nlm.nih.gov\/articles\/PMC10616531\/\" target=\"_blank\" rel=\"noreferrer noopener\">Delayed childbearing<\/a>, meaning purposeful delay of childbearing into one\u2019s late 30s or beyond, is associated with decreased fertility, but that\u2019s not all [1]. Statistics suggest that over the age of 40 a woman\u2019s risk of death during pregnancy or early postpartum rises dramatically.&nbsp;<\/p>\n\n\n\n<p>Here at Natural Womanhood, we\u2019ve addressed <a href=\"https:\/\/naturalwomanhood.org\/addressing-our-maternal-mortality-crisis-with-fertility-awareness-postpartum-health-2020\/\" target=\"_blank\" rel=\"noreferrer noopener\">maternal mortality<\/a> <a href=\"https:\/\/naturalwomanhood.org\/the-business-of-being-born\/\" target=\"_blank\" rel=\"noreferrer noopener\">many times<\/a>, especially the unacceptably high rates of maternal mortality amongst <a href=\"https:\/\/naturalwomanhood.org\/black-moms-breastfeeding\/\" target=\"_blank\" rel=\"noreferrer noopener\">black women<\/a> in the United States. But when we stratify pregnant women by <em>age<\/em>, women over age 40 <em>of all races<\/em> are significantly more likely to die than younger women. Why might this be? And what can we do about it?&nbsp;<\/p>\n\n\n\n<h2 class=\"wp-block-heading\" id=\"h-how-much-more-likely-are-women-over-aged-40-to-die-compared-to-younger-women\"><span id=\"how-much-more-likely-are-women-over-aged-40-to-die-compared-to-younger-women\">How much more likely are women over aged 40 to die compared to younger women?<\/span><\/h2>\n\n\n\n<p>According to the <a href=\"https:\/\/www.cdc.gov\/nchs\/data\/hestat\/maternal-mortality\/2023\/maternal-mortality-rates-2023.htm\" target=\"_blank\" rel=\"noreferrer noopener\">CDC<\/a>, using data from 2023, the most recent year for which data is available, women over aged 40 were almost <em>five times more likely<\/em> to die during pregnancy or the first six weeks postpartum compared to women aged 25 and younger. Specifically, \u201crates in 2023 were 12.5 deaths per 100,000 live births for women younger than age 25, 18.1 for those ages 25\u201339, and 59.8 for those age 40 and older.\u201d<\/p>\n\n\n\n<figure class=\"wp-block-pullquote\"><blockquote><p>According to the CDC, using data from 2023, women over aged 40 were almost <em>five times more likely<\/em> to die during pregnancy or the first six weeks postpartum compared to women aged 25 and younger. <\/p><\/blockquote><\/figure>\n\n\n\n<p>Of note, these high maternal mortality rates for women over aged 40 represent a <em>marked<\/em> <em>improvement<\/em> from sky-high 2022 rates of 87.1 deaths\/100,000 live births. The 2023 numbers represent a return to 2018 levels, as seen <a href=\"https:\/\/www.cdc.gov\/nchs\/data\/hestat\/maternal-mortality\/2023\/fig01.png\" target=\"_blank\" rel=\"noreferrer noopener\">here<\/a>, which had already begun to rise pre-COVID and then sharply increased between 2020 and 2021.&nbsp;<\/p>\n\n\n\n<p>Even with the relative reduction of maternal mortality rates in 2023, when the data were restricted to <em>black<\/em> mothers over age 40, the rate was an almost unfathomable 239.9 maternal deaths\/100,000 live births.<\/p>\n\n\n\n<h2 class=\"wp-block-heading\" id=\"h-what-are-the-top-causes-of-maternal-mortality\"><span id=\"what-are-the-top-causes-of-maternal-mortality\">What are the top causes of maternal mortality?<\/span><\/h2>\n\n\n\n<p>Based on this <a href=\"https:\/\/stacks.cdc.gov\/view\/cdc\/51660\" target=\"_blank\" rel=\"noreferrer noopener\">2018 report<\/a> from the Nine Maternal Mortality Review Committees, new mothers of any age were most likely to die in the first six weeks postpartum (45%) compared to during pregnancy (37.6%) or six weeks through one year postpartum (17.5%).&nbsp;<\/p>\n\n\n\n<p>The top causes of death, regardless of age, were postpartum hemorrhage and cardiovascular or coronary (heart blood vessel) conditions (tied at 14% each), infection (10.7%), cardiomyopathy (heart muscle disease, 10.7%), amniotic fluid embolism (8.4%), <a href=\"https:\/\/naturalwomanhood.org\/preeclampsia\/\" target=\"_blank\" rel=\"noreferrer noopener\">preeclampsia<\/a> and eclampsia (7.4%), and <a href=\"https:\/\/naturalwomanhood.org\/understanding-and-recognizing-postpartum-depression-2020\/\" target=\"_blank\" rel=\"noreferrer noopener\">mental health conditions<\/a> (7%).&nbsp;<\/p>\n\n\n\n<h2 class=\"wp-block-heading\" id=\"h-why-are-women-over-aged-40-so-much-more-likely-to-die-during-pregnancy-or-early-postpartum-nbsp\"><span id=\"why-are-women-over-aged-40-so-much-more-likely-to-die-during-pregnancy-or-early-postpartum\">Why are women over aged 40 so much more likely to die during pregnancy or early postpartum?&nbsp;<\/span><\/h2>\n\n\n\n<p>\u2018Advanced maternal age,\u2019 a term that makes virtually every woman wince or roll her eyes, is defined as pregnancy at age 35 and above. To be clear, ACOG itself acknowledges the age 35 cutoff\u00a0 \u201cis an arbitrary threshold.\u201d In fact, that specific number reflects research suggesting a relative age-related increase in risk of miscarriage and\/or chromosomal abnormality <em>for the baby<\/em>, not medical risks <em>for the mother<\/em>.\u00a0<\/p>\n\n\n\n<p>Still, ACOG <a href=\"https:\/\/www.acog.org\/clinical\/clinical-guidance\/obstetric-care-consensus\/articles\/2022\/08\/pregnancy-at-age-35-years-or-older\" target=\"_blank\" rel=\"noreferrer noopener\">notes<\/a>, \u201cstudies evaluating the effect of chronic medical conditions such as diabetes, hypertension, and obesity, which may exacerbate pregnancy-related morbidity, appear to demonstrate <em>an increasing risk with increasing age at the time of pregnancy<\/em>\u201d (emphasis added) [2][3].\u00a0<\/p>\n\n\n\n<p>A small <a href=\"https:\/\/pubmed.ncbi.nlm.nih.gov\/20965486\/\" target=\"_blank\" rel=\"noreferrer noopener\">2010 study<\/a> of pregnant women over age 45 found higher rates of high blood pressure, diabetes, C-sections, <a href=\"https:\/\/naturalwomanhood.org\/placental-problems\/\" target=\"_blank\" rel=\"noreferrer noopener\">placenta previa<\/a>, and <a href=\"https:\/\/naturalwomanhood.org\/third-stage-of-labor\/\" target=\"_blank\" rel=\"noreferrer noopener\">postpartum hemorrhage<\/a> compared to younger women [4].\u00a0<\/p>\n\n\n\n<h3 class=\"wp-block-heading\" id=\"h-preexisting-medical-conditions\"><span id=\"preexisting-medical-conditions\">Preexisting medical conditions<\/span><\/h3>\n\n\n\n<p>The most obvious answer as to why older mothers are more likely to die during pregnancy or early postpartum is that they are more likely to have preexisting conditions such as high blood pressure and <a href=\"https:\/\/naturalwomanhood.org\/type-2-diabetes-pregnant\/\" target=\"_blank\" rel=\"noreferrer noopener\">diabetes<\/a>. Unsurprisingly, these can make pregnancy much harder on their bodies.\u00a0<\/p>\n\n\n\n<h3 class=\"wp-block-heading\" id=\"h-multiples-pregnancy\"><span id=\"multiples-pregnancy\">Multiples pregnancy<\/span><\/h3>\n\n\n\n<p>Older mothers are also more likely to have a multiples pregnancy (being pregnant with two or more babies at one time), and this can be for one of two reasons. As women age, they are more likely to conceive twins.&nbsp;<\/p>\n\n\n\n<p>Additionally, for first-time mothers who experienced infertility, assisted reproductive technologies (ART) such as in vitro fertilization (IVF) or <a href=\"https:\/\/naturalwomanhood.org\/egg-retrieval-ivf-freezing-donation\/\" target=\"_blank\" rel=\"noreferrer noopener\">ovarian hyperstimulation with gonadotropin medications<\/a> are <a href=\"https:\/\/www.acog.org\/clinical\/clinical-guidance\/committee-opinion\/articles\/2017\/09\/multifetal-pregnancy-reduction\" target=\"_blank\" rel=\"noreferrer noopener\">far more likely<\/a> than naturally conceived pregnancies to yield multiples [5]. (Of note, there are significantly fewer ART-related multiples pregnancies compared to even a decade ago due to <a href=\"https:\/\/pmc.ncbi.nlm.nih.gov\/articles\/PMC10197809\/\" target=\"_blank\" rel=\"noreferrer noopener\">fewer embryos transferred<\/a>, and to abortion, also known as <a href=\"https:\/\/www.acog.org\/clinical\/clinical-guidance\/committee-opinion\/articles\/2017\/09\/multifetal-pregnancy-reduction\" target=\"_blank\" rel=\"noreferrer noopener\">&#8216;selective reduction<\/a>,&#8217; of one or more successfully implanted embryos.)\u00a0<\/p>\n\n\n\n<p>As could be expected, the highest risks from multiples pregnancies come for the babies themselves. But mothers expecting multiples are also at increased <a href=\"https:\/\/pmc.ncbi.nlm.nih.gov\/articles\/PMC1955760\/\" target=\"_blank\" rel=\"noreferrer noopener\">risk<\/a> of high blood pressure, <a href=\"https:\/\/naturalwomanhood.org\/preeclampsia\/\" target=\"_blank\" rel=\"noreferrer noopener\">preeclampsia<\/a>, <a href=\"https:\/\/naturalwomanhood.org\/everything-you-need-to-know-about-gestational-diabetes\/\" target=\"_blank\" rel=\"noreferrer noopener\">gestational diabetes<\/a>, and postpartum hemorrhage [6].\u00a0<\/p>\n\n\n\n<h2 class=\"wp-block-heading\" id=\"h-how-can-we-drive-down-maternal-mortality-rates-amongst-women-aged-40-and-over\"><span id=\"how-can-we-drive-down-maternal-mortality-rates-amongst-women-aged-40-and-over\">How can we drive down maternal mortality rates amongst women aged 40 and over?<\/span><\/h2>\n\n\n\n<p>Keep in mind that <em>increased<\/em> risk of death during pregnancy or early postpartum does not mean that mothers aged 40 or above are <em>likely<\/em> to die. It simply means that their risk, on average, is higher than the risk for younger women. In order to drive down risk for <em>all<\/em> pregnant women, we need a cultural shift to recognize the value of reproductive life planning (as described at length in this <a href=\"https:\/\/naturalwomanhood.org\/podcast\/nw-podcast-s3ep10-repro-life-planning\/\" target=\"_blank\" rel=\"noreferrer noopener\">Natural Womanhood podcast episode<\/a>).\u00a0<\/p>\n\n\n\n<figure class=\"wp-block-pullquote\"><blockquote><p>Keep in mind that <em>increased<\/em> risk of death during pregnancy or early postpartum does not mean that mothers aged 40 or above are <em>likely<\/em> to die. It simply means that their risk, on average, is higher than the risk for younger women. In order to drive down risk for all pregnant women, we need a cultural shift to recognize the value of reproductive life planning.<\/p><\/blockquote><\/figure>\n\n\n\n<p>Additionally, we can ensure that <a href=\"https:\/\/naturalwomanhood.org\/unnecessary-c-section\/\" target=\"_blank\" rel=\"noreferrer noopener\">every C-section is a medically necessary C-section<\/a>. Access to midwives, particularly for pregnant women whose only \u2018risk factor\u2019 is age, and to <a href=\"https:\/\/naturalwomanhood.org\/naturalwomanhood-org-doula\/\" target=\"_blank\" rel=\"noreferrer noopener\">doula care during labor<\/a> (including <a href=\"https:\/\/naturalwomanhood.org\/maternal-and-infant-mortality-rates-in-the-us\/\" target=\"_blank\" rel=\"noreferrer noopener\">giving dads doula training<\/a>!) may have an outsized impact on meeting this goal.\u00a0<\/p>\n\n\n\n<p>Since the majority of <a href=\"https:\/\/naturalwomanhood.org\/addressing-our-maternal-mortality-crisis-with-fertility-awareness-postpartum-health-2020\/\">maternal mortality<\/a> occurs <em>before<\/em> the traditional six-week in-office checkup to sign off on return to working out and sex, earlier, more frequent, in-home postpartum care is necessary. New Jersey\u2019s <a href=\"https:\/\/naturalwomanhood.org\/maternal-and-infant-mortality-rates-in-the-us\/\" target=\"_blank\" rel=\"noreferrer noopener\">nurse home visitor program<\/a> is an excellent example of this concept in action. Across the board, a sea change amongst OB\/GYNs is necessary to implement <a href=\"https:\/\/www.acog.org\/clinical\/clinical-guidance\/committee-opinion\/articles\/2018\/05\/optimizing-postpartum-care\" target=\"_blank\" rel=\"noreferrer noopener\">ACOG\u2019s recommendation of two postpartum checkups<\/a>, including one at or by three weeks postpartum.\u00a0<\/p>\n\n\n\n<p>The more eyes we can get on a woman in her home setting, the better chance we have for <a href=\"https:\/\/naturalwomanhood.org\/understanding-and-recognizing-postpartum-depression-2020\/\" target=\"_blank\" rel=\"noreferrer noopener\">catching problems<\/a> (be they physical or <a href=\"https:\/\/naturalwomanhood.org\/when-the-bough-breaks\/\" target=\"_blank\" rel=\"noreferrer noopener\">mental health-related<\/a>) before they become fatal. Lactation consultants, postpartum doulas, and direct primary care physicians all represent birth workers who may go into the home. The last, but perhaps most crucial and most achievable, line of defense is <a href=\"https:\/\/naturalwomanhood.org\/mismatch-hypothesis-ppd\/\" target=\"_blank\" rel=\"noreferrer noopener\">ensuring broader community support <\/a>to help postpartum women access <a href=\"https:\/\/naturalwomanhood.org\/navigating-the-fourth-trimester-good-postpartum-nutrition\/\" target=\"_blank\" rel=\"noreferrer noopener\">adequate nutrition<\/a>, <a href=\"https:\/\/naturalwomanhood.org\/podcast\/nw-podcast-s4ep11-red-light-therapy-photobiomodulation\/\" target=\"_blank\" rel=\"noreferrer noopener\">sun exposure<\/a>, <a href=\"https:\/\/naturalwomanhood.org\/the-fourth-trimester-guidebook-postpartum-healing-and-exercise\/\" target=\"_blank\" rel=\"noreferrer noopener\">exercise<\/a>, childcare, and breastfeeding support.
