However, what we\u2019re not often told about are the risks that come along with that first critical step\u2014the egg retrieval procedure\u2014even though knowledge of these risks is vital for true informed consent. In this article, we\u2019ll cover the procedures and drugs involved in the egg retrieval process, and the risks that accompany them.\u00a0<\/p>\n\n\n\n<h2 class=\"wp-block-heading\" id=\"h-gonadotropins-and-ovarian-stimulation\"><span id=\"gonadotropins-and-ovarian-stimulation\">Gonadotropins and ovarian stimulation<\/span><\/h2>\n\n\n\n<p>The egg retrieval process\u2014arguably better termed egg \u201charvesting\u201d than \u201cretrieval\u201d\u2014<a href=\"https:\/\/www.cnyfertility.com\/ivf-timeline\/\" target=\"_blank\" rel=\"noreferrer noopener\">first involves stimulating the follicles<\/a> (the tiny, fluid-filled sacs containing eggs inside the ovary) through the use of medications called gonadotropins, which help the ovaries to produce multiple fully mature follicles at once\u2014a process known as controlled ovarian hyperstimulation (COH)\u2014in the hopes that multiple eggs can be obtained for future use in IVF.<\/p>\n\n\n\n<p>In a natural menstrual cycle, gonadotropins are released by the pituitary gland every month. These naturally occurring gonadotropins include <a href=\"https:\/\/naturalwomanhood.org\/fam-basics-what-is-the-follicular-phase\/\" target=\"_blank\" rel=\"noreferrer noopener\">follicle stimulating hormone (FSH)<\/a> and <a href=\"https:\/\/naturalwomanhood.org\/naturalwomanhood-org-fam-basics-luteal-phase\/\" target=\"_blank\" rel=\"noreferrer noopener\">luteinizing hormone (LH)<\/a>, which facilitate the maturation of several follicles in preparation for the release of an egg (ovulation) from the ovary (although, typically, only <em>one<\/em> follicle will fully mature to the point of rupture, or ovulation). In the IVF process, synthetic versions of FSH and LH are administered, sometimes in a <a href=\"https:\/\/www.cnyfertility.com\/ivf-shots-injectables\/\" target=\"_blank\" rel=\"noreferrer noopener\">combination medication<\/a> containing both FSH and LH. Some side effects of gonadotropins <a href=\"https:\/\/www.sciencedirect.com\/science\/chapter\/edited-volume\/abs\/pii\/B9780323856874000166\" target=\"_blank\" rel=\"noreferrer noopener\">include<\/a> <a href=\"https:\/\/thekingsleyclinic.com\/resources\/follicle-stimulating-hormone-fsh-uses-dosing-side-effects\/\" target=\"_blank\" rel=\"noreferrer noopener\">blood<\/a> <a href=\"https:\/\/thekingsleyclinic.com\/resources\/luteinizing-hormone-lh-medications-uses-benefits-and-side-effects\/\" target=\"_blank\" rel=\"noreferrer noopener\">clots<\/a>, <a href=\"https:\/\/pmc.ncbi.nlm.nih.gov\/articles\/PMC5993897\/\" target=\"_blank\" rel=\"noreferrer noopener\">Ovarian Hyperstimulation Syndrome<\/a> (OHSS\u2014more on this below), <a href=\"https:\/\/naturalwomanhood.org\/how-do-you-treat-ectopic-pregnancy\/\" target=\"_blank\" rel=\"noreferrer noopener\">ectopic pregnancies<\/a>, and ovarian torsion (a twisting of the ovaries due to heavier-than-usual follicles, which is a medical emergency that can <a href=\"https:\/\/www.healthline.com\/health\/womens-health\/ovarian-torsion\" target=\"_blank\" rel=\"noreferrer noopener\">result in the loss of an ovary)<\/a> [1].\u00a0<\/p>\n\n\n\n<p>Unfortunately, there are few studies on the long-term impacts of these synthetic hormones on the body, so the full breadth of their risks is unknown, though some studies have shown a <a href=\"https:\/\/www.remembryo.com\/asrm-releases-2024-guidance-on-cancer-and-fertility-drugs\/#:~:text=The%20ASRM%20published%20their%202024%20guidelines%20on%20cancer,ability%20of%20fertility%20drugs%20to%20modify%20hormone%20levels.\" target=\"_blank\" rel=\"noreferrer noopener\">possible increase<\/a> in <a href=\"https:\/\/pmc.ncbi.nlm.nih.gov\/articles\/PMC6974869\/\" target=\"_blank\" rel=\"noreferrer noopener\">ovarian<\/a> and <a href=\"https:\/\/www.rbmojournal.com\/article\/S1472-6483(17)30048-2\/fulltext\" target=\"_blank\" rel=\"noreferrer noopener\">breast<\/a> cancers following the use of fertility medications [2] [3].<\/p>\n\n\n\n<h2 class=\"wp-block-heading\" id=\"h-gnrh-agonists-and-antagonists\"><span id=\"gnrh-agonists-and-antagonists\">GnRH agonists and antagonists<\/span><\/h2>\n\n\n\n<p>Sometimes, a <a href=\"https:\/\/www.sart.org\/patients\/a-patients-guide-to-assisted-reproductive-technology\/general-information\/art-medications\/\" target=\"_blank\" rel=\"noreferrer noopener\">GnRH (gonadotropin releasing hormone) agonist<\/a> is used for a few days instead of synthetic FSH and LH to stimulate the pituitary gland to release its own FSH and LH gonadotropins. However, after 7-10 days, GnRH actually begins to suppress the production of any further FSH and LH, and there is evidence that prolonged suppression of natural gonadotropins can lead to <a href=\"https:\/\/link.springer.com\/article\/10.1007\/s12012-025-10025-3#:~:text=Moreover%2C%20GnRH%20therapy%20has%20been%20shown%20to,further%20exacerbate%20cardiovascular%20risk%20%5B%2013%2C%2014%5D\" target=\"_blank\" rel=\"noreferrer noopener\">cardiovascular issues such as hypertension and stroke<\/a>.\u00a0<\/p>\n\n\n\n<p>Since reproductive endocrinologists must collect multiple eggs for IVF before they are ovulated into the fallopian tube, a GnRH antagonist is given to prevent ovulation occurring before the scheduled day of retrieval. <a href=\"https:\/\/link.springer.com\/chapter\/10.1007\/978-3-031-77255-9_5\" target=\"_blank\" rel=\"noreferrer noopener\">Unlike GnRH agonists<\/a>, <a href=\"https:\/\/www.littleangelivf.com\/blog\/ivf\/what-are-gnrh-analogues-for-ivf\/\" target=\"_blank\" rel=\"noreferrer noopener\">GnRH antagonists work immediately<\/a> to <em>block<\/em> the receptors in the pituitary gland that release gonadotropins, so these are often used for preventing ovulation. As with the use of gonadotropins, an insufficient number of studies have been conducted to show the side effects of agonists&#8217; and antagonists\u2019 impacts on the pituitary gland.\u00a0<\/p>\n\n\n\n<p>Next, a shot to trigger ovulation is given, <a href=\"https:\/\/theluckyegg.com\/2022\/03\/29\/ivf-medications-and-protocols-explained-by-a-fertility-doctor\/\">which could include: shots of<\/a> human chorionic gonadotropin (hCG, or the hormone that helps <a href=\"https:\/\/www.ncbi.nlm.nih.gov\/books\/NBK532950\/\" target=\"_blank\" rel=\"noreferrer noopener\">maintain early pregnancy<\/a>); a combination of hCG and the drug Lupron to reduce risk of ovarian hyperstimulation syndrome; or only Lupron for women whose risk of OHSS is so great (like those with PCOS) that they cannot tolerate even the small dose of hCG that is present in the combination shot [4]. However, the risk of OHSS isn\u2019t completely eliminated with the <a href=\"https:\/\/pmc.ncbi.nlm.nih.gov\/articles\/PMC5373266\/\" target=\"_blank\" rel=\"noreferrer noopener\">combination<\/a> or <a href=\"https:\/\/www.fertstert.org\/article\/S0015-0282(20)32086-0\/fulltext\" target=\"_blank\" rel=\"noreferrer noopener\">Lupron-only<\/a> trigger shot [5].<\/p>\n\n\n\n<h2 class=\"wp-block-heading\" id=\"h-ovarian-hyperstimulation-syndrome-ohss\"><span id=\"ovarian-hyperstimulation-syndrome-ohss\">Ovarian hyperstimulation syndrome (OHSS)<\/span><\/h2>\n\n\n\n<p>Ovarian hyperstimulation syndrome occurs when ovaries <a href=\"https:\/\/mft.nhs.uk\/app\/uploads\/sites\/4\/2024\/01\/SMPIL-23-041-OVARIAN-HYPERSTIMULATION-SYNDROME.pdf\" target=\"_blank\" rel=\"noreferrer noopener\">respond too strongly to the stimulating medication<\/a>s used in the retrieval process, causing excessive swelling of the follicles and the leakage of fluids from the ovaries into the abdomen. Symptoms of OHSS <a href=\"https:\/\/integration.reproductivefacts.org\/news-and-publications\/fact-sheets-and-infographics\/side-effects-of-injectable-fertility-drugs-gonadotropins\/\" target=\"_blank\" rel=\"noreferrer noopener\">can include<\/a> blood clots, kidney dysfunction, dehydration, shallow pulse, nausea, vomiting, and <a href=\"https:\/\/healthonline.washington.edu\/sites\/default\/files\/record_pdfs\/Ovarian-Hyperstimulation-Syndrome_05-2025.pdf\" target=\"_blank\" rel=\"noreferrer noopener\">shortness of breath<\/a>. <\/p>\n\n\n\n<p>Even though Lupron decreases the risk of ovarian hyperstimulation syndrome, moderate-to-severe OHSS has still been shown to occur in <a href=\"https:\/\/www.asrm.org\/practice-guidance\/practice-committee-documents\/prevention-and-treatment-of-moderate-and-severe-ovarian-hyperstimulation-syndrome-a-guideline\/\" target=\"_blank\" rel=\"noreferrer noopener\">around 1-5% of IVF cycles<\/a>. To put that number into perspective, according to the <a href=\"https:\/\/www.asrm.org\/news-and-events\/asrm-news\/press-releasesbulletins\/us-ivf-usage-increases-in-2023-leads-to-over-95000-babies-born\/\">American Society for Reproductive Medicine<\/a>, there were approximately 432,641 IVF cycles in 2023, which calculates to between 4,326 to 21,632 women developing OHSS. Furthermore, even though Lupron is used as a precaution against OHSS, the drug <a href=\"https:\/\/naturalwomanhood.org\/puberty-blockers\/\" target=\"_blank\" rel=\"noreferrer noopener\">comes with its own risks<\/a>, including <a href=\"https:\/\/www.medicalnewstoday.com\/articles\/drugs-lupron-depot-side-effects#serious-side-effects\" target=\"_blank\" rel=\"noreferrer noopener\">chest pain, shortness of breath<\/a>, <a href=\"https:\/\/dctd.cancer.gov\/research\/ctep-trials\/trial-development\/side-effects\/leuprolide.docx\" target=\"_blank\" rel=\"noreferrer noopener\">blood clots<\/a>, heart attack or heart failure, <a href=\"https:\/\/www.luprongyn.com\/content\/pdf\/lch.pdf\" target=\"_blank\" rel=\"noreferrer noopener\">severe skin reactions<\/a>, seizures, and loss of bone density, among <a href=\"https:\/\/dctd.cancer.gov\/research\/ctep-trials\/trial-development\/side-effects\/leuprolide.docx\" target=\"_blank\" rel=\"noreferrer noopener\">several other risks<\/a>.\u00a0<\/p>\n\n\n\n<figure class=\"wp-block-pullquote\"><blockquote><p>&#8230; moderate-to-severe OHSS has still been shown to occur in <a href=\"https:\/\/www.asrm.org\/practice-guidance\/practice-committee-documents\/prevention-and-treatment-of-moderate-and-severe-ovarian-hyperstimulation-syndrome-a-guideline\/\" target=\"_blank\" rel=\"noreferrer noopener\">around 1-5% of IVF cycles<\/a>. To put that number into perspective, according to the <a href=\"https:\/\/www.asrm.org\/news-and-events\/asrm-news\/press-releasesbulletins\/us-ivf-usage-increases-in-2023-leads-to-over-95000-babies-born\/\">American Society for Reproductive Medicine<\/a>, there were approximately 432,641 IVF cycles in 2023, which calculates to between 4,326 to 21,632 women developing OHSS.<\/p><\/blockquote><\/figure>\n\n\n\n<h2 class=\"wp-block-heading\" id=\"h-the-role-of-hormonal-contraceptives-in-ivf-egg-retrieval\"><span id=\"the-role-of-hormonal-contraceptives-in-ivf-egg-retrieval\">The role of hormonal contraceptives in IVF egg retrieval<\/span><\/h2>\n\n\n\n<p>In addition to synthetic FSH, LH, Lupron, etc., <a href=\"https:\/\/www.longdom.org\/open-access-pdfs\/the-role-of-birth-control-pills-in-ivf-understanding-their-use-and-impact.pdf\" target=\"_blank\" rel=\"noreferrer noopener\">birth control pills may also be used<\/a> in the controlled ovarian hyperstimulation process to control the timing of the retrieval procedure, and\/or to prevent the natural growth of follicles in order to <a href=\"https:\/\/rmanetwork.com\/blog\/why-do-i-take-birth-control-pills-to-get-pregnant-with-ivf\/\" target=\"_blank\" rel=\"noreferrer noopener\">establish a \u201cbaseline\u201d<\/a> with which to start the follicle stimulation process through externally administered gonadotropins. <\/p>\n\n\n\n<p>The use of hormonal contraceptives may also reduce the likelihood of ovarian cysts developing during the hyperstimulation process, and ASRM notes that hormonal contraceptives <a href=\"https:\/\/www.asrm.org\/practice-guidance\/practice-committee-documents\/the-use-of-hormonal-contraceptives-in-fertility-treatments-a-committee-opinion-2024\/\" target=\"_blank\" rel=\"noreferrer noopener\">may also increase IVF success rates<\/a> in those with polycystic ovary syndrome and endometriosis. However, birth control pills use <a href=\"https:\/\/naturalwomanhood.org\/difference-between-synthetic-hormones-vs-natural-hormones-bioidentical-birth-control-harmful-safe-affect-body-progesterone-progestin-estrogen-08192018\/\" target=\"_blank\" rel=\"noreferrer noopener\">synthetic forms<\/a> of progesterone and estrogen, or hormones that are not bioidentical to ones naturally produced by the body, called <a href=\"https:\/\/naturalwomanhood.org\/reasons-you-need-a-period-the-role-of-the-menstrual-cycle-in-the-immune-system\/\">progestin and estradiol<\/a>. The use of synthetic hormones can have <a href=\"https:\/\/naturalwomanhood.org\/topic\/birth-control-side-effects\/\" target=\"_blank\" rel=\"noreferrer noopener\">negative effects on the body<\/a>, such as heart attack, stroke, <a href=\"https:\/\/naturalwomanhood.org\/birth-control-abortion-and-breast-cancer-risk\/\" target=\"_blank\" rel=\"noreferrer noopener\">breast<\/a> and <a href=\"https:\/\/naturalwomanhood.org\/does-birth-control-prevent-or-cause-cancer\/\" target=\"_blank\" rel=\"noreferrer noopener\">cervical cancer<\/a>, mood changes, breakthrough bleeding, a decrease in bone density, autoimmune disorders, and weight gain.\u00a0<\/p>\n\n\n\n<p>Hormonal contraceptives also come with an increased risk of developing blood clots. <a href=\"https:\/\/journals.sagepub.com\/doi\/abs\/10.1177\/0024363918816683\" target=\"_blank\" rel=\"noreferrer noopener\">As one systematic review on contraceptives and blood clots reported<\/a>:&nbsp;<\/p>\n\n\n\n<blockquote class=\"wp-block-quote is-layout-flow wp-block-quote-is-layout-flow\">\n<p>&#8230;using HC increases a woman\u2019s risk of being diagnosed with VTE (venous thromboembolism) by <em>three to nine times<\/em>. For women under 30, the risk is increased <em>13-fold <\/em>during the first year of use, when the risk for clot formation is highest.\u201d The study also found that \u201c&#8230;the risk of fatal VTE was increased in women aged fifteen to twenty-four by 18.8-fold [6].<\/p>\n<\/blockquote>\n\n\n\n<p>Further, the synthetic hormones in contraceptives can literally alter the functioning of parts of the brain such as the ventromedial prefrontal cortex, the part of the brain that\u2019s responsible for <a href=\"https:\/\/studyfinds.org\/birth-control-women-brain-fear\/\" target=\"_blank\" rel=\"noreferrer noopener\">decision-making, emotional regulation<\/a>, and <a href=\"https:\/\/naturalwomanhood.org\/what-does-birth-control-do-to-womens-ability-to-process-fear\/\" target=\"_blank\" rel=\"noreferrer noopener\">memory<\/a>. One <a href=\"https:\/\/www.frontiersin.org\/news\/2023\/11\/07\/contraceptive-pills-fear-regulating-womens-brains\" target=\"_blank\" rel=\"noreferrer noopener\">study<\/a> found that women using hormonal contraceptives had greater susceptibility to chronic anxiety and greater overall fearfulness because of a thinning of the tissue in the brain that correlates with one\u2019s ability to handle generalized fear, mental and emotional resilience, and the ability to stay calm.<\/p>\n\n\n\n<h2 class=\"wp-block-heading\" id=\"h-egg-retrieval-and-embryo-transfer\"><span id=\"egg-retrieval-and-embryo-transfer\">Egg retrieval and embryo transfer<\/span><\/h2>\n\n\n\n<p>After the controlled ovarian hyperstimulation process comes the egg retrieval procedure. A very fine needle is <a href=\"https:\/\/www.illumefertility.com\/fertility-blog\/ivf-egg-retrieval-process\" target=\"_blank\" rel=\"noreferrer noopener\">inserted into the vagina and used to pierce through the vaginal wall<\/a> to access the ovaries. The needle then punctures each of the swollen follicles to remove their egg-containing fluid. <\/p>\n\n\n\n<p>While the <a href=\"https:\/\/www.scientificamerican.com\/article\/egg-donors-may-face-uncertain-long-term-risks\/\" target=\"_blank\" rel=\"noreferrer noopener\">long-term risks<\/a> of egg retrieval are not well-documented, (arguably making it difficult for a patient to give true informed consent to the procedure), the reported short-term risks of egg retrieval <a href=\"https:\/\/www.stanfordchildrens.org\/en\/services\/fertility-and-reproductive-health\/in-vitro-fertilization\/oocyte-retrieval.html\" target=\"_blank\" rel=\"noreferrer noopener\">include: accidental puncturing<\/a> of the bladder, <a href=\"https:\/\/nap.nationalacademies.org\/read\/11832\/chapter\/5#33\" target=\"_blank\" rel=\"noreferrer noopener\">intestines<\/a>, uterus, ovaries, and blood vessels by the needle; as well as the risk of pelvic and ovarian infections; <a href=\"https:\/\/nap.nationalacademies.org\/read\/11832\/chapter\/5#33\" target=\"_blank\" rel=\"noreferrer noopener\">inflammation<\/a> in the lining of the abdominal cavity; <a href=\"https:\/\/pubmed.ncbi.nlm.nih.gov\/29411339\/\" target=\"_blank\" rel=\"noreferrer noopener\">pelvic abscesses<\/a>; ovarian twisting; <a href=\"https:\/\/www.vinmec.com\/eng\/blog\/possible-complications-after-egg-retrieval-en\" target=\"_blank\" rel=\"noreferrer noopener\">abdominal bleeding<\/a>; and <a href=\"https:\/\/cbc-network.org\/2019\/07\/clinical-complications-oocyte-retrievals\/\" target=\"_blank\" rel=\"noreferrer noopener\">anesthesia complications<\/a> [7].\u00a0<\/p>\n\n\n\n<p>Regarding the embryo transfer process, or when the retrieved eggs have been successfully fertilized with sperm in the lab and one (or two) of the resulting embryos are transferred into the woman\u2019s uterus, a few surgical complications that can arise during the procedure include: bleeding into the uterine cavity or perforation of uterine muscles following catheter insertion; and\/or <a href=\"https:\/\/integration.reproductivefacts.org\/news-and-publications\/fact-sheets-and-infographics\/in-vitro-fertilization-ivf-what-are-the-risks\/\" target=\"_blank\" rel=\"noreferrer noopener\">pelvic infections<\/a> [8].<\/p>\n\n\n\n<h2 class=\"wp-block-heading\" id=\"h-likelihood-of-egg-retrieval-success\"><span id=\"likelihood-of-egg-retrieval-success\">Likelihood of egg retrieval success<\/span><\/h2>\n\n\n\n<p>Women seeking to harvest their eggs for immediate IVF or for <a href=\"https:\/\/naturalwomanhood.org\/egg-freezing\/\" target=\"_blank\" rel=\"noreferrer noopener\">egg freezing for future IVF<\/a> may have to endure multiple cycles, since not every egg extracted will be mature enough to fertilize, and then, only <a href=\"https:\/\/rmanetwork.com\/blog\/number-of-eggs-good-ivf-in-vitro-fertilization\/#:~:text=As%20a%20rule%20of%20thumb%2C%20however%2C%20having%20about,a%20woman%20a%2065%20percent%20chance%20of%20pregnancy\" target=\"_blank\" rel=\"noreferrer noopener\">about 80%<\/a> of the remaining eggs will fertilize.
