We all struggle with them from time to time. But have you ever wondered why they might seem worse during different times of the month? Some women notice an increase in intrusive thoughts during the<a href=\"https:\/\/www.ncbi.nlm.nih.gov\/pmc\/articles\/PMC6450738\/#:~:text=Several%20other%20studies%20find%20similar,of%20OCD%20symptoms%20in%20women\" target=\"_blank\" rel=\"noreferrer noopener\"> luteal phase<\/a> of the cycle, coinciding with other premenstrual syndrome (PMS) symptoms [1]. Obsessive compulsive disorder (OCD) is very different from occasional intrusive thoughts and a very serious, diagnosable illness. However, women who experience OCD are also influenced by the changes in their cycle. Here is how. \u00a0<\/p>\n\n\n\n<h2 class=\"wp-block-heading\" id=\"h-what-is-ocd\"><span id=\"what-is-ocd\">What is OCD?<\/span><\/h2>\n\n\n\n<p>For the approximately 2% of the population diagnosed with OCD, recurring, intensely disturbing thoughts can be a daily event. <a href=\"https:\/\/peaceofmind.com\/about-ocd\/\" target=\"_blank\" rel=\"noreferrer noopener\">OCD<\/a> is characterized by persistent, unwanted, distressing thoughts (obsessions) that are out of sync with one\u2019s character or moral code. While everyone periodically has intrusive thoughts, most people are able to automatically dismiss them as unimportant or not real. For OCD sufferers, the intrusive thoughts get \u201cstuck,\u201d and they feel unable to let go of them.\u00a0<\/p>\n\n\n\n<p>Compulsions are repetitive physical or mental responses sufferers engage in to alleviate the mental pain and anxiety caused by the thought of one\u2019s obsessions potentially coming true. While compulsions <em>initially<\/em> reduce anxiety, in the long run they <em>worsen<\/em> it to such an extent that significant mental and\/or physical energy is expended every single day trying to escape the obsession-compulsion cycle.&nbsp;<\/p>\n\n\n\n<h2 class=\"wp-block-heading\" id=\"h-ocd-symptoms-may-worsen-during-big-hormonal-shifts\"><span id=\"ocd-symptoms-may-worsen-during-big-hormonal-shifts\">OCD symptoms may worsen during big hormonal shifts<\/span><\/h2>\n\n\n\n<p>Major changes in<a href=\"https:\/\/pubmed.ncbi.nlm.nih.gov\/15816784\/\" target=\"_blank\" rel=\"noreferrer noopener\"> reproductive life<\/a>, such as menarche (a girl\u2019s first period), pregnancy, <a href=\"https:\/\/peaceofmind.com\/education\/types-of-ocd\/postpartum-ocd\/\" target=\"_blank\" rel=\"noreferrer noopener\">postpartum<\/a>, and menopause, are known to impact OCD symptoms [2]. About 3-5% of new mothers may experience postpartum OCD, <a href=\"https:\/\/adaa.org\/learn-from-us\/from-the-experts\/blog-posts\/consumer\/unexpected-ocd-postpartum#:~:text=OCD%20symptoms%20in%20women%20tend,6%20weeks%20after%20giving%20birth\" target=\"_blank\" rel=\"noreferrer noopener\">according<\/a> to the Anxiety and Depression Association of America (ADAA). Due to hormonal fluctuations throughout the <a href=\"https:\/\/www.treatmyocd.com\/what-is-ocd\/info\/related-symptoms-conditions\/why-do-i-have-intrusive-thoughts-before-my-period\" target=\"_blank\" rel=\"noreferrer noopener\">menstrual cycle<\/a>, women with OCD may also experience increased intrusive thoughts (and stronger tendencies to respond compulsively, or more difficulty resisting compulsive responses) at certain times of the month.\u00a0<\/p>\n\n\n\n<h2 class=\"wp-block-heading\" id=\"h-what-s-the-relationship-between-ocd-and-the-menstrual-cycle-specifically\"><span id=\"whats-the-relationship-between-ocd-and-the-menstrual-cycle-specifically\">What&#8217;s the relationship between OCD and the menstrual cycle, specifically?<\/span><\/h2>\n\n\n\n<p>We\u2019ve previously<a href=\"https:\/\/naturalwomanhood.org\/postpartum-depression-pill\/\" target=\"_blank\" rel=\"noreferrer noopener\"> written<\/a> about the effects female reproductive hormones can have on mental health, in particular, anxiety and depression. Rises in<a href=\"https:\/\/www.ncbi.nlm.nih.gov\/pmc\/articles\/PMC6450738\/#:~:text=Several%20other%20studies%20find%20similar,of%20OCD%20symptoms%20in%20women\" target=\"_blank\" rel=\"noreferrer noopener\"> estrogen<\/a>, such as the one that occurs during the follicular phase of the menstrual cycle, have been shown to decrease negative mental health symptoms [1]. Meanwhile, an increase in mental health symptoms may accompany the drop in progesterone and estrogen levels at the end of the luteal phase before menstruation [1].<\/p>\n\n\n\n<p>Remember how I wrote earlier that everyone has intrusive thoughts, though most people are able to let them go? According to<a href=\"https:\/\/pmc.ncbi.nlm.nih.gov\/articles\/PMC6450738\/#:~:text=Several%20other%20studies%20find%20similar,of%20OCD%20symptoms%20in%20women\" target=\"_blank\" rel=\"noreferrer noopener\"> a 2019 study published in <em>Psychoneuroendocrinology<\/em><\/a>, 49% of healthy young women experienced an increase in checking behaviors (\u201cinspection of one\u2019s own behaviors to reduce anxiety about potential adverse outcomes\u201d) during the mid-luteal phase [1].&nbsp;<\/p>\n\n\n\n<p>A <a href=\"https:\/\/pubmed.ncbi.nlm.nih.gov\/23497833\/\" target=\"_blank\" rel=\"noreferrer noopener\">2013 study<\/a> suggests that in women with OCD, a subset (49.3% of study participants) experience a worsening of sexual\/religious obsessions, anxiety, depression, and suicidality, during the premenstrual phase [3]. Recall that the premenstrual phase, the few days before your period starts, is a time of major hormonal withdrawal as<em> both<\/em> estrogen and progesterone levels decrease.&nbsp;<\/p>\n\n\n\n<h2 class=\"wp-block-heading\" id=\"h-can-ocd-symptoms-be-alleviated\"><span id=\"can-ocd-symptoms-be-alleviated\">Can OCD symptoms be alleviated?<\/span><\/h2>\n\n\n\n<p>If you experience OCD and find the few days before your \u201ctime of the month\u201d to be particularly difficult, you\u2019re likely wondering, <em>is there treatment available for OCD symptoms<\/em>? The answer, thankfully, is a resounding yes.&nbsp;<\/p>\n\n\n\n<h3 class=\"wp-block-heading\" id=\"h-exposure-and-response-prevention-therapy-erp\"><span id=\"exposure-and-response-prevention-therapy-erp\">Exposure and response prevention therapy (ERP)<\/span><\/h3>\n\n\n\n<p>According to<a href=\"https:\/\/www.treatmyocd.com\/what-is-ocd\/info\/related-symptoms-conditions\/why-do-i-have-intrusive-thoughts-before-my-period\" target=\"_blank\" rel=\"noreferrer noopener\"> NOCD<\/a>, Exposure and Response Prevention (ERP) therapy is the gold-standard (and usually affordable!) treatment for individuals with OCD.&nbsp;<\/p>\n\n\n\n<p>This therapy involves gradual exposure to one\u2019s OCD triggers to help them develop fear habituation (desensitization) <em>without<\/em> performing compulsive mental or physical behaviors. NOCD helpfully explains, \u201cOver time, this allows them to tolerate anxiety without relying on compulsions or avoidance to feel better, and their intrusive thoughts cause less distress in the long run.\u201d&nbsp;<\/p>\n\n\n\n<h3 class=\"wp-block-heading\" id=\"h-acceptance-and-commitment-therapy\"><span id=\"acceptance-and-commitment-therapy\">Acceptance and commitment therapy<\/span><\/h3>\n\n\n\n<p>OCD sufferers may also benefit from <a href=\"https:\/\/iocdf.org\/expert-opinions\/expert-opinion-what-is-act\/\" target=\"_blank\" rel=\"noreferrer noopener\">Acceptance and Commitment Therapy (ACT)<\/a> in tandem with ERP. ACT teaches people with OCD to distinguish the difference between thoughts or feelings (you cannot control) versus actions (you can control). ACT helps individuals see their thoughts as just thoughts, not as dangers. A 2018 study found that ERP + ACT and ERP alone \u201cwere both highly effective treatments for OCD\u201d [4]. The full text of the study, including a review of research on the role of medications and traditional cognitive behavioral therapy (CBT) in treating OCD, is <a href=\"https:\/\/www.utahact.com\/uploads\/5\/1\/3\/4\/51340265\/adding_acceptance_and_commitment_therapy_to_exposure_and_response.pdf\" target=\"_blank\" rel=\"noreferrer noopener\">here<\/a>.&nbsp;<\/p>\n\n\n\n<h3 class=\"wp-block-heading\" id=\"h-progesterone-supplementation\"><span id=\"progesterone-supplementation\">Progesterone supplementation<\/span><\/h3>\n\n\n\n<p>An <a href=\"https:\/\/adaa.org\/learn-from-us\/from-the-experts\/blog-posts\/consumer\/unexpected-ocd-postpartum#:~:text=OCD%20symptoms%20in%20women%20tend,6%20weeks%20after%20giving%20birth\" target=\"_blank\" rel=\"noreferrer noopener\">article<\/a> on the ADAA website observes, \u201cStudies have shown that women with OCD are likely to have abnormal hormone levels and that these hormones play a role in triggering or worsening the condition.\u201d The article continues, \u201cThere are also treatments that can help with hormone levels to alleviate OCD symptoms. Women who have elevated levels of estrogen may consider <a href=\"https:\/\/naturalwomanhood.org\/fam-basics-progesterone\/\" target=\"_blank\" rel=\"noreferrer noopener\">progesterone therapy<\/a> to balance <a href=\"https:\/\/naturalwomanhood.org\/what-is-estrogen-hormone\/\" target=\"_blank\" rel=\"noreferrer noopener\">high estrogen levels<\/a>.\u201d The idea of progesterone support is far from a surprise for those familiar with restorative reproductive medicine supplementation protocols for women suffering from <a href=\"https:\/\/naturalwomanhood.org\/natural-treatments-for-bloating-before-period-other-pms-symptoms\/\" target=\"_blank\" rel=\"noreferrer noopener\">premenstrual syndrome<\/a> (PMS), <a href=\"https:\/\/naturalwomanhood.org\/premenstrual-dysphoric-disorder\/\" target=\"_blank\" rel=\"noreferrer noopener\">premenstrual dysphoric disorder<\/a> (PMDD), or <a href=\"https:\/\/naturalwomanhood.org\/facts-about-postpartum-depression\/\" target=\"_blank\" rel=\"noreferrer noopener\">postpartum depressio<\/a>n (PPD).&nbsp;<\/p>\n\n\n\n<p>The article further stipulates, \u201cGrowth hormone and pregnenolone (a steroid hormone involved in the sex hormones) can also play a role\u201d in treating OCD patients with hormonal imbalances. Pregnenolone is a <a href=\"https:\/\/cp.care\/a-technical-primer-on-over-the-counter-pregnenolone-vs-prescription-allopregnanolone-and-their-use-in-psychiatric-and-pain-disorders\/\" target=\"_blank\" rel=\"noreferrer noopener\">precursor<\/a> to cortisol, testosterone, estrogen, and progesterone. A pregnenolone metabolite, allopregnanolone, is the active ingredient in the only FDA-approved medications (an <a href=\"https:\/\/naturalwomanhood.org\/promising-new-treatment-for-postpartum-depression-brexanolone-1-2019\/\" target=\"_blank\" rel=\"noreferrer noopener\">IV option<\/a> and an <a href=\"https:\/\/naturalwomanhood.org\/postpartum-depression-pill\/\" target=\"_blank\" rel=\"noreferrer noopener\">oral option<\/a> exist) to treat postpartum depression.<\/p>\n\n\n\n<h2 class=\"wp-block-heading\" id=\"h-women-with-ocd-and-pms-should-learn-to-chart-their-menstrual-cycles\"><span id=\"women-with-ocd-and-pms-should-learn-to-chart-their-menstrual-cycles\">Women with OCD and PMS should learn to chart their menstrual cycles<\/span><\/h2>\n\n\n\n<p>For some women with OCD, intrusive thoughts will worsen during a certain part of the menstrual cycle. Because of the role progesterone supplementation may play in alleviating premenstrual symptoms, and the role hormonal support can play in resolving hormonal disorders generally, fertility awareness charting is especially important for women with OCD and PMS. While fertility awareness charting is beneficial for any woman, it can help these women in particular gain an understanding of why they\u2019re feeling the way they feel, so that they can respond more mindfully and engage more purposefully in ERP and possibly ACT.&nbsp;<\/p>\n\n\n\n<p>Furthermore, fertility awareness can help reproductive medicine-trained professionals identify hormonal disorders, resolve imbalances, and hopefully in doing so, also improve the effectiveness of their patient\u2019s other therapies.&nbsp;<\/p>\n\n\n\n<h4 class=\"wp-block-heading\" id=\"h-references\"><span id=\"references\">References:<\/span><\/h4>\n\n\n\n[1] Mulligan EM, Hajcak G, Klawohn J, Nelson B, Meyer A.
