{"id":21343,"date":"2024-09-12T08:00:00","date_gmt":"2024-09-12T13:00:00","guid":{"rendered":"https:\/\/naturalwomanhood.org\/?p=21343"},"modified":"2024-10-31T14:45:49","modified_gmt":"2024-10-31T19:45:49","slug":"hrt-despues-de-los-65-anos","status":"publish","type":"post","link":"https:\/\/naturalwomanhood.org\/es\/hrt-after-age-65\/","title":{"rendered":"Tratamiento de los s\u00edntomas de la menopausia despu\u00e9s de los 65 a\u00f1os con terapia hormonal: \u00bfEs seguro?\u00a0"},"content":{"rendered":"\n<p>Menopause,<a href=\"https:\/\/www.ahajournals.org\/doi\/10.1161\/CIRCULATIONAHA.122.061559\" target=\"_blank\" rel=\"noreferrer noopener\"> defined<\/a> as \u201cthe permanent cessation of menstruation caused by a loss of ovarian function,\u201d is the final stage in a woman\u2019s reproductive life [1]. Women going through \u201cthe change\u201d may report <a href=\"https:\/\/naturalwomanhood.org\/hot-flashes\/\" target=\"_blank\" rel=\"noreferrer noopener\">hot flashes<\/a> and other vasomotor symptoms (VMS), anxiety, insomnia, and overall reduced quality of life. These same women are also at higher risk of developing certain cancers, cardiovascular disease, and dementia.\u00a0<\/p>\n\n\n\n<p>A common treatment for women in this stage of life used to be hormone replacement therapy (HRT). But for the past two decades, HRT was believed to be <a href=\"https:\/\/menopause.org\/press-releases\/women-aged-older-than-65-years-may-be-able-to-safely-continue-taking-hormone-therapy\" target=\"_blank\" rel=\"noreferrer noopener\">dangerous<\/a>, especially for women over age 65, and therefore was rarely prescribed. As I\u2019ll cover below, that conclusion has been recently called into question.<\/p>\n\n\n\n<p>We know that for some women, menopause symptoms don\u2019t stop just because they\u2019ve reached retirement age (i.e., age 65). According to The <a href=\"https:\/\/journals.lww.com\/menopausejournal\/fulltext\/2024\/05000\/use_of_menopausal_hormone_therapy_beyond_age_65.3.aspx\" target=\"_blank\" rel=\"noreferrer noopener\">Menopause Society<\/a>, at least 2-7% of women continue to experience severe symptoms<em> well after<\/em> this standard cut-off age [2]. Is it safe to keep treating these women? A 2024 observational study conducted and published by<a href=\"https:\/\/journals.lww.com\/menopausejournal\/fulltext\/2024\/05000\/use_of_menopausal_hormone_therapy_beyond_age_65.3.aspx\" target=\"_blank\" rel=\"noreferrer noopener\"> The Menopause Society<\/a> aimed to better understand just how long HRT, which is now referred to as hormone therapy (HT), is appropriate for women with continued menopausal symptoms [2]. The study aimed to answer the questions: Should we be treating women in their 60s? 70s? What medications are best, and does it matter how many years women take them?\u00a0\u00a0<\/p>\n\n\n\n<h2 class=\"wp-block-heading\" id=\"h-how-was-menopause-historically-treated\"><span id=\"how-was-menopause-historically-treated\">How was menopause historically treated?<\/span><\/h2>\n\n\n\n<p>There have been, and continue to be, <a href=\"https:\/\/journals.lww.com\/menopausejournal\/citation\/2024\/05000\/advancing_menopausal_care__the_importance_of.1.aspx\" target=\"_blank\" rel=\"noreferrer noopener\">major gaps<\/a> in our understanding of the best ways to prescribe HT for menopausal women [3]. One of the primary reasons for this gap is that HT refers to many different combinations of medications, dosages, and routes of administration. Some are considered bioidentical (molecularly identical to the body\u2019s own natural hormones), while some are not.\u00a0<\/p>\n\n\n\n<p>As previously mentioned, HT treatment for early menopause was <a href=\"https:\/\/www.ahajournals.org\/doi\/10.1161\/CIRCULATIONAHA.122.061559\" target=\"_blank\" rel=\"noreferrer noopener\">widely<\/a> accepted in the past [1]. However, in 2002, that view shifted with the publication of the HERS (Heart and Estrogen\/Progestin Replacement Study) and WHI (Women\u2019s Health Initiative) randomized trials, which concluded that HT could <em>increase<\/em> the risk of cardiovascular disease. Since then, many healthcare providers have significantly decreased HT prescriptions for their menopausal patients.\u00a0<\/p>\n\n\n\n<h2 class=\"wp-block-heading\" id=\"h-major-medical-societies-once-again-recommend-ht-but-there-s-a-catch\"><span id=\"major-medical-societies-once-again-recommend-ht-but-theres-a-catch\">Major medical societies once again recommend HT, but there\u2019s a catch<\/span><\/h2>\n\n\n\n<p>Over the past 20 years, <a href=\"https:\/\/www.menopause.org\/docs\/default-source\/press-release\/use-of-ht-beyond-age-65-and-its-effects-by-type.pdf\" target=\"_blank\" rel=\"noreferrer noopener\">dozens<\/a> of studies have been conducted, expanding our knowledge of HT for menopausal women. This research suggests that the findings in the HERS and WHI studies may not have accurately represented the risks and benefits of HT. For example, Northwestern University professor Dr. Lauren Streicher recently told <a href=\"https:\/\/www.npr.org\/sections\/health-shots\/2024\/05\/01\/1248525256\/hormones-menopause-hormone-therapy-hot-flashes\" target=\"_blank\" rel=\"noreferrer noopener\">NPR<\/a> that previous risks were associated with higher hormone doses, plus hormones (she specifically mentioned the <a href=\"https:\/\/naturalwomanhood.org\/depo-provera-birth-control-shot\/\" target=\"_blank\" rel=\"noreferrer noopener\">Depo-Provera shot<\/a>) that \u201cwe don\u2019t prescribe [for that reason] anymore.\u201d\u00a0<\/p>\n\n\n\n<p>Because of the encouraging, more recent evidence on HT, four major U.S. <a href=\"https:\/\/www.ahajournals.org\/doi\/10.1161\/CIRCULATIONAHA.122.061559\" target=\"_blank\" rel=\"noreferrer noopener\">medical societies<\/a>, the American College of Obstetricians and Gynecologists (ACOG), the American Association of Clinical Endocrinology, the Endocrine Society, and The Menopause Society (previously called The North American Menopause Society), all now state that<em> <\/em>HT is a viable option for treating menopausal symptoms. However,<em> these societies lack <\/em><a href=\"https:\/\/journals.lww.com\/menopausejournal\/citation\/2024\/05000\/advancing_menopausal_care__the_importance_of.1.aspx\" target=\"_blank\" rel=\"noreferrer noopener\"><em>agreement<\/em><\/a><em> on the methodology of how HT should be prescribed <\/em>[3].\u00a0<\/p>\n\n\n\n<h2 class=\"wp-block-heading\" id=\"h-is-ht-a-good-idea-after-age-65-it-depends\"><span id=\"is-ht-a-good-idea-after-age-65-it-depends\">Is HT a good idea after age 65? It depends<\/span><\/h2>\n\n\n\n<p>I wrote above that 2-7% of women may experience menopausal symptoms after age 65, and that one of the <a href=\"https:\/\/menopause.org\/press-releases\/women-aged-older-than-65-years-may-be-able-to-safely-continue-taking-hormone-therapy\" target=\"_blank\" rel=\"noreferrer noopener\">challenges<\/a> in previous research on menopausal HT is that there are multiple types, routes, and dose options for HT. The<a href=\"https:\/\/journals.lww.com\/menopausejournal\/fulltext\/2024\/05000\/use_of_menopausal_hormone_therapy_beyond_age_65.3.aspx\" target=\"_blank\" rel=\"noreferrer noopener\"> aforementioned 2024 observational study<\/a>, published by The Menopause Society, assessed the various types of HT administered to women<em> beyond<\/em> the age of 65 years, and aimed to clarify the risk for different combinations of medicines and doses so that women can receive more personalized recommendations [2]. By following the medication and doctor visit records of 10 million senior women on Medicare from 2007-2020, the researchers sought to determine whether HT increased risk of all-cause mortality, five cancers, six cardiovascular-related diseases, and dementia.\u00a0<\/p>\n\n\n\n<p>They <a href=\"https:\/\/menopause.org\/press-releases\/women-aged-older-than-65-years-may-be-able-to-safely-continue-taking-hormone-therapy\">discovered<\/a> that the benefits and risks of long-term HT after 65 years varied significantly \u201cdepending on type, route, and dose prescribed.\u201d This finding was in line with the Menopausal Society\u2019s<a href=\"https:\/\/pubmed.ncbi.nlm.nih.gov\/35797481\/\" target=\"_blank\" rel=\"noreferrer noopener\"> 2022 HT Statement<\/a>, which said that HT prescriptions should be individualized\u2014not based on an age cutoff\u2014and continued as long as the woman is regularly assessed by a medical professional well-versed in the risks and benefits of extended HT [4]. Overall, the study found that, compared with women who never used HT or discontinued it before age 65, extended HT was associated with a reduction in negative menopausal symptoms [2].\u00a0<\/p>\n\n\n\n<p>Specifically, estrogen monotherapy (estrogen alone) was associated with a<em> decreased<\/em> risk of mortality, cancers, dementia, cardiovascular disease, and VMS [2]. Progestin monotherapy significantly reduced the risk of multiple cancers, congestive heart failure, and venous thromboembolism. Combination (estrogen plus progestin) HT was found to <em>increase<\/em> the risk of breast cancer, but these risks were<a href=\"https:\/\/journals.lww.com\/menopausejournal\/citation\/2024\/05000\/advancing_menopausal_care__the_importance_of.1.aspx\" target=\"_blank\" rel=\"noreferrer noopener\"> mitigated<\/a> when given in low doses and transdermally (applied topically to the skin) or vaginally.\u00a0\u00a0<\/p>\n\n\n\n<h2 class=\"wp-block-heading\" id=\"h-what-were-the-limitations-and-what-do-we-still-not-know\"><span id=\"what-were-the-limitations-and-what-do-we-still-not-know\">What were the limitations, and what do we still not know?<\/span><\/h2>\n\n\n\n<p>This study helped to bridge the gap in understanding appropriate extended menopausal HT. More research is needed to fully<a href=\"https:\/\/journals.lww.com\/menopausejournal\/citation\/2024\/05000\/advancing_menopausal_care__the_importance_of.1.aspx\" target=\"_blank\" rel=\"noreferrer noopener\"> understand<\/a> the benefits and risks of the various HT formulations, as well as the risks of extended HT [2]. The <a href=\"https:\/\/journals.lww.com\/menopausejournal\/fulltext\/2024\/05000\/use_of_menopausal_hormone_therapy_beyond_age_65.3.aspx\" target=\"_blank\" rel=\"noreferrer noopener\">limitations<\/a> of this study all lie in the fact that this was an observational study; therefore, the researchers were relying on the availability of medical data. Furthermore, their data collection began for participants at age 65 years, so information on past health conditions or hysterectomies could not be noted in the study. They also could not assess variables such as healthy and unhealthy behaviors among the patients.\u00a0<\/p>\n\n\n\n<h2 class=\"wp-block-heading\" id=\"h-the-bottom-line-on-whether-ht-should-be-prescribed-after-age-65\"><span id=\"the-bottom-line-on-whether-ht-should-be-prescribed-after-age-65\">The bottom line on whether HT should be prescribed after age 65<\/span><\/h2>\n\n\n\n<p>Like everything else when it comes to women\u2019s reproductive health, every woman\u2019s experience of menopause is unique and requires individualized treatment.
