No daily pill to take, no maintenance\u2026there\u2019s even the promise of light and easy periods or periods that disappear altogether. According to the advertising, a woman with an IUD shouldn\u2019t have to think about the device at all for 5-10 years or until she\u2019s looking to get pregnant. However, one common side effect of IUDs can make it rather hard to forget: continuous bleeding.<\/p>\n\n\n\n<p>There are four brands of hormonal IUDs available in the U.S., Mirena,&nbsp;Skyla, Liletta, and Kyleena. Each works as contraception by thickening cervical mucus (to prevent sperm from swimming to the egg) and thinning the uterine lining (to make it difficult for a newly conceived human to implant). It\u2019s this second mechanism that\u2019s related to symptoms like the breakthrough bleeding that many women experience with the IUD.<\/p>\n\n\n\n<h2 class=\"wp-block-heading\" id=\"h-how-common-is-continuous-bleeding-with-a-hormonal-iud\"><span id=\"how-common-is-continuous-bleeding-with-a-hormonal-iud\">How common is continuous bleeding with a hormonal IUD?<\/span><\/h2>\n\n\n\n<p>IUD brand <a href=\"https:\/\/www.mirena-us.com\/about-mirena\/how-mirena-iud-works?utm_source=google&amp;utm_medium=cpc&amp;utm_campaign=MRN_DTC_cpc_google_BRN&amp;utm_content=BRN_X_DK_PHR_X_X&amp;utm_term=mirena%20iud%20side%20effects&amp;gclsrc=aw.ds&amp;&amp;gclid=Cj0KCQjw_qexBhCoARIsAFgBlesiKeKyrHWV7sMvI78ZJKA5jKwEj9cnLTOEDGHmI-nYsIeEYYmj4vsaAr0dEALw_wcB&amp;gclsrc=aw.ds\" target=\"_blank\" rel=\"noreferrer noopener\">Mirena<\/a> lists changes in bleeding as one of their common side effects. They state \u201cyou may have bleeding and spotting between menstrual periods, especially during the first 3 to 6 months. Sometimes the bleeding is heavier than usual at first.\u201d<\/p>\n\n\n\n<p>According to <a href=\"https:\/\/www.acog.org\/womens-health\/experts-and-stories\/the-latest\/what-you-should-know-about-breakthrough-bleeding-with-birth-control#:~:text=Breakthrough%20bleeding%20happens%20more%20often,in%202%20to%206%20months.\" target=\"_blank\" rel=\"noreferrer noopener\">ACOG<\/a>, breakthrough bleeding (bleeding that\u2019s not part of your period) can happen with any type of birth control but is more common with certain types. The \u201ccertain types\u201d are low-dose birth control pills, the implant, and hormonal IUDs like Mirena. With IUDs, it may take 2 to 6 months for irregular bleeding to subside.&nbsp;<\/p>\n\n\n\n<p>A <a href=\"https:\/\/www.reddit.com\/r\/TwoXChromosomes\/comments\/3p8yu2\/mirena_iud_never_ending_period_all_new_blood_im\/\" target=\"_blank\" rel=\"noreferrer noopener\">Reddit<\/a> thread on \u201cMirena IUD: never ending period&#8230;\u201d has firsthand accounts of women who\u2019ve experienced frequent or continuous bleeding after IUD insertion. For some women, this side effect lasted several weeks, while other women reported frequent bleeding for an entire year before deciding to make a new birth control plan with their OB\/GYN.<\/p>\n\n\n\n<h2 class=\"wp-block-heading\" id=\"h-what-about-copper-iuds\"><span id=\"what-about-copper-iuds\">What about copper IUDs?<\/span><\/h2>\n\n\n\n<p>Although copper IUDs do not use synthetic hormones, the effect they produce is similar to hormonal IUDs. Copper is a naturally cytotoxic metal (meaning toxic to cells) and <a href=\"https:\/\/pubmed.ncbi.nlm.nih.gov\/7191636\/\" target=\"_blank\" rel=\"noreferrer noopener\">damages sperm<\/a> [1]. Like the hormonal IUD, the copper IUD also creates an inflammatory response within the uterus. Heavier, longer periods are common side effects of <a href=\"https:\/\/naturalwomanhood.org\/is-copper-iud-toxic-painful-cause-hormone-imbalance-side-effects-thyroid-natural-birth-control-08112018\/\" target=\"_blank\" rel=\"noreferrer noopener\">copper IUDs<\/a>, and the <a href=\"https:\/\/www.paragard.com\/safety-information\/\" target=\"_blank\" rel=\"noreferrer noopener\">Paragard <\/a>website also lists spotting between periods as a common side effect.\u00a0A Healthline <a href=\"https:\/\/www.healthline.com\/health\/birth-control-and-fibroids#safer-methods\" target=\"_blank\" rel=\"noreferrer noopener\">article<\/a> warns that Paragard may be particularly inappropriate for some women with uterine fibroids because it can make their bleeding heavier and more painful. <\/p>\n\n\n\n<p>The mechanism for <em>why<\/em> IUDs cause bleeding is difficult to determine, especially since recent research into the negative effects of IUDs is rather sparse. <a href=\"https:\/\/pubmed.ncbi.nlm.nih.gov\/12267402\/#:~:text=Scientists%20in%20general%20believe%20that,as%20does%20the%20uterine%20fluid.\" target=\"_blank\" rel=\"noreferrer noopener\">This report from 1980<\/a> provides a clue, however [2]. It states that IUDs increase local fibrinolytic (meaning blood-thinning) activity that makes it difficult for bleeding to stop. There\u2019s also the physical reality that there is a foreign body in the uterus that can cause pressure or irritation to the uterine walls and cause bleeding.<\/p>\n\n\n\n<h2 class=\"wp-block-heading\" id=\"h-is-continuous-bleeding-a-serious-problem\"><span id=\"is-continuous-bleeding-a-serious-problem\">Is continuous bleeding a serious problem?<\/span><\/h2>\n\n\n\n<p>On its own, continuous bleeding may not be serious. Many women see an improvement in symptoms after 2 to 6 months without further complications. If bleeding is frequent or heavy enough, women may need to take an iron supplement to treat IUD-caused anemia.<\/p>\n\n\n\n<p>According to <a href=\"https:\/\/www.drugdangers.com\/mirena\/#:~:text=Mirena%20bleeding%20may%20be%20associated,Device%20migration%20into%20the%20abdomen\" target=\"_blank\" rel=\"noreferrer noopener\">Drug Dangers<\/a>, the FDA has received over 45,000 reports of IUD-related bleeding complications. They also note that a concern with continuous bleeding is that it may mask the early warning signs of other serious complications such as an ectopic pregnancy, displaced IUD, or infection or even uterine cancer. Without early detection, these complications could become life-threatening.&nbsp;<\/p>\n\n\n\n<h2 class=\"wp-block-heading\" id=\"h-reality-vs-what-iud-companies-promise\"><span id=\"reality-vs-what-iud-companies-promise\">Reality vs. what IUD companies promise<\/span><\/h2>\n\n\n\n<p>Pharmaceutical companies advertise a low-dose hormone (Mirena) or hormone-free (Paragard) option for pregnancy prevention. According to Mayo Clinic, <a href=\"https:\/\/www.mayoclinic.org\/tests-procedures\/mirena\/about\/pac-20391354\" target=\"_blank\" rel=\"noreferrer noopener\">Mirena<\/a> is also considered a good treatment option for women with heavy or painful periods, endometriosis, anemia, and fibroids. <\/p>\n\n\n\n<p>In reality, an IUD may actually complicate periods by <em>increasing<\/em> bleeding and introducing breakthrough bleeding. In addition, the risk of u<a href=\"https:\/\/naturalwomanhood.org\/pregnancy-after-iud-or-tubal-ligation\/\" target=\"_blank\" rel=\"noreferrer noopener\">nexpected pregnancy may be higher<\/a> than originally thought. Rather than being worry-free, IUDs may come with additional difficulties such as <a href=\"https:\/\/naturalwomanhood.org\/anxiety-and-other-warning-signs-side-effects-of-iud\/\" target=\"_blank\" rel=\"noreferrer noopener\">anxiety<\/a>, <a href=\"https:\/\/naturalwomanhood.org\/removing-my-copper-iud-removed-my-depression-and-saved-my-life-2020\/\" target=\"_blank\" rel=\"noreferrer noopener\">depression<\/a>, or <a href=\"https:\/\/naturalwomanhood.org\/how-my-awful-experience-with-the-copper-iud-led-me-to-truly-natural-birth-control-1-19\/\" target=\"_blank\" rel=\"noreferrer noopener\">severe reactions<\/a>.&nbsp;<\/p>\n\n\n\n<p>One of the most difficult parts of an IUD experience gone wrong is that a decision that promised to give a woman <em>more <\/em>control over her body leads to her feeling <em>less <\/em>in control than ever before. Luckily, women don\u2019t need to \u201ccontrol\u201d their bodies to treat reproductive symptoms or prevent pregnancies. Learning a fertility awareness method can provide the knowledge and empowerment to understand your body and learn the root cause of reproductive or hormonal problems, without any side effects.&nbsp;<\/p>\n\n\n\n<p><em>This article was updated on January 27, 2025, to note that copper IUDs may be inappropriate for women who experience pain and\/or bleeding due to uterine fibroids.
