{"id":20936,"date":"2024-06-11T08:00:00","date_gmt":"2024-06-11T13:00:00","guid":{"rendered":"https:\/\/naturalwomanhood.org\/?p=20936"},"modified":"2024-07-25T14:16:40","modified_gmt":"2024-07-25T19:16:40","slug":"hbc-afectan-al-cerebro","status":"publish","type":"post","link":"https:\/\/naturalwomanhood.org\/es\/hbc-affect-brain\/","title":{"rendered":"C\u00f3mo afectan los anticonceptivos hormonales al cerebro"},"content":{"rendered":"\n<p>As an undergraduate, I once spent an entire summer dissecting fruit fly ovaries (and you thought fruit flies were small!). The reason? I was part of a research team that was investigating <em>how<\/em> cells migrate, and collecting fruit fly ovaries to study is an efficient research method that only costs the sanity of one or two college students. We know <em>that<\/em> cells move from place to place, like in cancer, growth, or wound repair, but \u201c<em>how?\u201d<\/em> was a question we had no answer to at the time.&nbsp;<\/p>\n\n\n\n<p>Similarly to my cellular migration study, we know that hormonal birth control (HBC) is associated with <a href=\"https:\/\/naturalwomanhood.org\/oral-contraception-depression-teens\/\" target=\"_blank\" rel=\"noreferrer noopener\">negative mental health effects<\/a>, but understanding \u201chow?\u201d is a question we are only beginning to understand. A <a href=\"https:\/\/www.sciencedirect.com\/science\/article\/pii\/S0091302222000590?via%3Dihub\" target=\"_blank\" rel=\"noreferrer noopener\">2022 article<\/a> from the journal <em>Frontiers in Neuroendocrinology<\/em> reviews three potential models for how HBC may affect the brain and highlights areas where more research is needed [1].<\/p>\n\n\n\n<h2 class=\"wp-block-heading\" id=\"h-three-models-for-how-hormonal-birth-control-may-impact-the-brain\"><span id=\"three-models-for-how-hormonal-birth-control-may-impact-the-brain\">Three models for how hormonal birth control may impact the brain<\/span><\/h2>\n\n\n\n<h3 class=\"wp-block-heading\" id=\"h-1-too-much-of-a-good-thing-hypergonadal-state\"><span id=\"1-too-much-of-a-good-thing-hypergonadal-state\">#1 Too much of a good thing (hypergonadal state)<\/span><\/h3>\n\n\n\n<p>The first model for how HBC might affect the brain assumes that the brain responds similarly to the body\u2019s endogenous (i.e., natural) hormones and HBC\u2019s synthetic hormones. It also assumes that keeping these hormonal levels elevated most of the time (as HBC does), instead of a few days of elevation each month (as the natural menstrual cycle does), <em>overexposes<\/em> the brain to these hormones.&nbsp;<\/p>\n\n\n\n<p>While the <em>Frontiers <\/em>article expressly states that \u201cestrogens and progestogens play an essential role in basic cognition, brain health, and mental illness etiology,\u201d HBC use eliminates the \u201cyin and yang\u201d relationship between endogenous <a href=\"https:\/\/naturalwomanhood.org\/no-cycle-on-the-pill\/\" target=\"_blank\" rel=\"noreferrer noopener\">estrogen and progesterone<\/a> that <a href=\"https:\/\/naturalwomanhood.org\/reasons-you-need-a-period-the-role-of-the-menstrual-cycle-in-the-immune-system\/\" target=\"_blank\" rel=\"noreferrer noopener\">many bodily systems<\/a> rely on. Rather than the body undergoing an estrogen peak, followed by a progesterone peak accompanied with a small estrogen rise (as it does during a typical menstrual cycle), the body under the influence of HBC is \u201cstuck\u201d in an ongoing, artificial \u201cluteal phase,\u201d with high progesterone and typically some estradiol.\u00a0<\/p>\n\n\n\n<h3 class=\"wp-block-heading\" id=\"h-2-store-bought-doesn-t-cut-it-hypogonadal-state\"><span id=\"2-store-bought-doesnt-cut-it-hypogonadal-state\">#2 Store-bought doesn\u2019t cut it (hypogonadal state)<\/span><\/h3>\n\n\n\n<p>The second model for how HBC might affect the brain assumes that the brain does <em>not<\/em> respond similarly to synthetic hormones as endogenous ones. In this model, since synthetic hormones suppress their naturally-occurring counterparts, this would <em>underexpose<\/em> the brain to these vital compounds. We do know that other synthetic hormones, such as <a href=\"https:\/\/naturalwomanhood.org\/oxytocin-vs-pitocin-whats-the-difference\/\" target=\"_blank\" rel=\"noreferrer noopener\">Pitocin<\/a>, deprive women of some of the benefits we normally see from their endogenous counterparts.<\/p>\n\n\n\n<h3 class=\"wp-block-heading\" id=\"h-3-it-s-complicated-mixed-state\"><span id=\"3-its-complicated-mixed-state\">#3 It\u2019s complicated (mixed-state)<\/span><\/h3>\n\n\n\n<p>This third and final model for how HBC might affect the brain assumes that the answer may involve both hypergonadal traits and hypogonadal traits. This model factors in many of the things we don\u2019t know about HBC, such as how its metabolites (i.e., what it breaks down into) affect the brain and other hormones involved in brain function, and the effects of individual genetic factors.<\/p>\n\n\n\n<h3 class=\"wp-block-heading\" id=\"h-the-verdict\"><span id=\"the-verdict\">The verdict?<\/span><\/h3>\n\n\n\n<p>In evaluating different studies against these three models, the <em>Frontiers<\/em> article identified that <em>both<\/em> the <a href=\"https:\/\/naturalwomanhood.org\/naturalwomanhood-org-fam-basics-luteal-phase\/\" target=\"_blank\" rel=\"noreferrer noopener\">luteal phase<\/a> of the menstrual cycle <em>and<\/em> HBC use led to<em> increased<\/em> prefrontal cortex connectivity and <em>decreased<\/em> parietal lobe connectivity. This would suggest that model #1 (hypergonadal, i.e., \u201ctoo much of a good thing\u201d) is the most likely, though the authors stress that much more research is needed to confirm this hypothesis.<\/p>\n\n\n\n<h2 class=\"wp-block-heading\" id=\"h-how-are-brain-connectivity-changes-tied-to-anxiety\"><span id=\"how-are-brain-connectivity-changes-tied-to-anxiety\">How are brain connectivity changes tied to anxiety?<\/span><\/h2>\n\n\n\n<p>The <a href=\"https:\/\/naturalwomanhood.org\/what-does-birth-control-do-to-womens-ability-to-process-fear\/\" target=\"_blank\" rel=\"noreferrer noopener\">prefrontal cortex<\/a> is the area of the brain associated with executive skills, that is, decision making, concentration, and planning, amongst other functions. The <a href=\"https:\/\/my.clevelandclinic.org\/health\/body\/24628-parietal-lobe\" target=\"_blank\" rel=\"noreferrer noopener\">parietal lobe<\/a> helps you to place yourself in the world around you: this includes spatial awareness and selecting which environmental stimuli require your attention. The <em>Frontiers <\/em>article doesn\u2019t speculate on the mental or emotional effects of these brain connectivity changes, but we <em>can <\/em>make some educated guesses, specifically in regards to anxiety.\u00a0<\/p>\n\n\n\n<p>A 2007 article on the prefrontal cortex published in the journal <em>Reviews in the Neurosciences <\/em>mentions that as the prefrontal cortex involves planning, an overactive prefrontal cortex can turn planning into frequent worrying [2]. Decreased parietal lobe connectivity could potentially also contribute to anxiety; we know that coping strategies like the \u201c<a href=\"https:\/\/www.healthline.com\/health\/333-rule-anxiety\" target=\"_blank\" rel=\"noreferrer noopener\">333 rule<\/a>\u201d work by utilizing grounding techniques that help reconnect you to your body and environment.\u00a0<\/p>\n\n\n\n<p>There is also a connection between increased progesterone and anxiety. A 2018 study found that women self-reported higher attachment anxiety during the luteal phase, when progesterone was highest [3]. Rather than this progesterone peak lasting several days in a normal cycle, (synthetic) progesterone is elevated the majority of the time with HBC use.&nbsp;<\/p>\n\n\n\n<h3 class=\"wp-block-heading\" id=\"h-why-the-connection-between-hbc-and-anxiety-is-complicated\"><span id=\"why-the-connection-between-hbc-and-anxiety-is-complicated\">Why the connection between HBC and anxiety is complicated<\/span><\/h3>\n\n\n\n<p>While these various studies indicate that there may be a physical, neurological component to why some women experience anxiety while taking HBC, not all women experience anxiety while on birth control. Furthermore, some women experience other mental health symptoms such as depression, indicating that individual factors like genetics and environment are also important to consider.<\/p>\n\n\n\n<h2 class=\"wp-block-heading\" id=\"h-there-s-so-much-we-don-t-know-about-how-hbc-affects-the-brain\"><span id=\"theres-so-much-we-dont-know-about-how-hbc-affects-the-brain\">There\u2019s so much we don\u2019t know about how HBC affects the brain\u2026<\/span><\/h2>\n\n\n\n<p>Besides summing up the three models and the current research, the <em>Frontiers <\/em>article authors describe current limitations to understanding the connection between HBC and the brain. There is still a lot to learn about how hormones and neurotransmitters naturally interact. Different research teams also use different methods, which makes comparing studies difficult at this stage. There are also many different forms of HBC on the market, with different formulas, dosages, and routes of administration that may change how these drugs affect the body. Other factors may include whether a person uses HBC for a long or short amount of time, and at what age they start using it.&nbsp;<\/p>\n\n\n\n<p>The <em>Frontiers <\/em>article also points out that people who have negative mental health effects from birth control are more likely to stop using HBC, eliminating the people who are most affected by HBC from relevant studies. All of this is in addition to the fact that neuroendocrinology is still an emerging field, with many frontiers left to explore.&nbsp;<\/p>\n\n\n\n<h2 class=\"wp-block-heading\" id=\"h-but-we-do-know-fertility-awareness-could-help-us-learn-more\"><span id=\"but-we-do-know-fertility-awareness-could-help-us-learn-more\">\u2026but we do know fertility awareness could help us learn more<\/span><\/h2>\n\n\n\n<p>Another issue mentioned is the difficulty of determining which phase of her cycle a woman is in. Concerningly, all five studies analyzed in the <em>Frontiers <\/em>article listed \u201ccalendar\u201d for their \u201ccycle phase determination method.\u201d While some studies also used hormone tests to better determine cycle phase, the rhythm method or <a href=\"https:\/\/naturalwomanhood.org\/officially-declaring-the-death-of-the-rhythm-method\/\" target=\"_blank\" rel=\"noreferrer noopener\">calendar method is not a modern, evidence-based fertility awareness method<\/a> (FAM), and its use poses serious limitations to determining actual cycle phase. Imagine how data collection could improve if results could be compared against carefully-tracked menstrual cycle biomarkers via a FAM! When we know we\u2019re accurately identifying menstrual cycle phases, we can be more confident we\u2019re making apples-to-apples comparisons about the effects of natural vs synthetic hormones on the brain.
