{"id":20675,"date":"2024-05-07T08:00:00","date_gmt":"2024-05-07T13:00:00","guid":{"rendered":"https:\/\/naturalwomanhood.org\/?p=20675"},"modified":"2024-07-09T14:00:47","modified_gmt":"2024-07-09T19:00:47","slug":"que-es-la-medicina-especifica-de-genero","status":"publish","type":"post","link":"https:\/\/naturalwomanhood.org\/es\/what-is-gender-specific-medicine\/","title":{"rendered":"\u00bfQu\u00e9 es la medicina espec\u00edfica de g\u00e9nero y c\u00f3mo ayudar\u00e1 a las mujeres a recibir mejor atenci\u00f3n m\u00e9dica?"},"content":{"rendered":"\n<p class=\"wp-block-paragraph\">During her 2022 Supreme Court confirmation hearing, Justice Kentanji Brown Jackson was asked a surprisingly hot-button question: \u201cCan you provide a definition for the word \u2018woman\u2019?\u201d The now-Supreme Court Justice replied, \u201cI can\u2019t\u2026 I\u2019m not a biologist.\u201d<\/p>\n\n\n\n<p class=\"wp-block-paragraph\">But does it really take a biologist to define what it means to be a woman or a man?<em> <\/em>After all, even infants as young as three months of age can distinguish between male and female <a href=\"https:\/\/www.ncbi.nlm.nih.gov\/pmc\/articles\/PMC6680589\/#:~:text=By%203%20months%20of%20age,conceptual%2C%20kind%2Dbased%20manner.\">faces<\/a> [1].&nbsp;<\/p>\n\n\n\n<p class=\"wp-block-paragraph\">Justice Jackson&#8217;s answer does point to a bit of truth, however. Certain aspects of what makes a woman a woman (or what makes a man a man) <em>can<\/em> be gleaned from scientific studies that go more than skin-deep. Gender-specific medicine (also called <a href=\"https:\/\/www.ncbi.nlm.nih.gov\/pmc\/articles\/PMC10085661\/#:~:text=Definition%20and%20necessity%20of%20sex%2Fgender%2Dspecific%20medicine&amp;text=Although%20it%20is%20generally%20believed,and%20clinical%20course%20of%20diseases.\" target=\"_blank\" rel=\"noreferrer noopener\">sex\/gender-specific medicine<\/a> or <a href=\"https:\/\/www.thelancet.com\/journals\/eclinm\/article\/PIIS2589-5370(20)30049-3\/fulltext\" target=\"_blank\" rel=\"noreferrer noopener\">sex-and-gender-sensitive medicine<\/a> in medical research), is a new field of science that seeks to discover exactly this: how male and female bodies differ in their disease development and response due to differences below the surface, from their hormones, to their brain structures, to their internal physiology, and even down to their DNA. In other words, gender-specific medicine recognizes (and aims to further discover) the inherent differences between men and women and the vast implications those differences have for how medicine can best treat both male and female patients&nbsp;<\/p>\n\n\n\n<h2 class=\"wp-block-heading\" id=\"h-hormonal-differences-between-men-and-women\"><span id=\"hormonal-differences-between-men-and-women\">Hormonal differences between men and women<\/span><\/h2>\n\n\n\n<p class=\"wp-block-paragraph\">We can\u2019t begin a discussion about the physiological differences between men and women without first discussing the three most important sex hormones: estrogen, progesterone, and testosterone.&nbsp;<\/p>\n\n\n\n<p class=\"wp-block-paragraph\">Most people can name the \u201cfemale hormones\u201d (estrogen and progesterone) and the \u201cmale hormone\u201d (testosterone). While this simple categorization is mostly correct, the reality of how these hormones operate is a bit more complex. Both men and women actually have all three of these hormones (plus <a href=\"https:\/\/www.epa.gov\/endocrine-disruption\/overview-endocrine-system#:~:text=Although%20hormones%20reach%20all%20parts,low%20amounts%20within%20the%20body.\" target=\"_blank\" rel=\"noreferrer noopener\">dozens more<\/a>), but they exist in very different amounts and have different roles in one sex versus the other.&nbsp;<\/p>\n\n\n\n<h3 class=\"wp-block-heading\" id=\"h-men-vs-women-s-hormonal-cycles\"><span id=\"men-vs-womens-hormonal-cycles\">Men vs. women\u2019s hormonal cycles<\/span><\/h3>\n\n\n\n<p class=\"wp-block-paragraph\">A woman operates on a <a href=\"https:\/\/naturalwomanhood.org\/no-cycle-on-the-pill\/\" target=\"_blank\" rel=\"noreferrer noopener\">monthly hormonal cycle<\/a> predominated by the ebb and flow of estrogen and progesterone, unless she is blunting her cycle with hormonal contraceptives. When a woman has normal hormone levels and healthy ovulation, her cycles are <a href=\"https:\/\/www.instagram.com\/sarahehillphd\/reel\/C1Kb8Hug1Tg\/\" target=\"_blank\" rel=\"noreferrer noopener\">predictable and measurable<\/a>, with normal (i.e., non-painful, non-heavy) periods occurring at regular intervals.&nbsp;<\/p>\n\n\n\n<p class=\"wp-block-paragraph\">Men, on the other hand, do not have such predictable hormonal cycles. They do experience a <a href=\"https:\/\/www.hollandandbarrett.com\/the-health-hub\/conditions\/mens-health\/men-master-your-hormones\/\" target=\"_blank\" rel=\"noreferrer noopener\">24-hour cycle of testosterone<\/a>, with a peak around 9am in the morning (which, unsurprisingly, is when many workplaces begin their day \u2014 right when a man is at his maximum level of focus and energy!). But testosterone levels can also vary wildly throughout the day due to dozens of outside factors, such as a man\u2019s <a href=\"https:\/\/www.ncbi.nlm.nih.gov\/pmc\/articles\/PMC4939866\/#:~:text=in%20blood15).-,Levels%20of%20job%20stress%20have%20been%20shown%20to%20be%20inversely,control%20in%20female%20teachers16).\" target=\"_blank\" rel=\"noreferrer noopener\">level of job stress and satisfaction<\/a>, if he\u2019s recently <a href=\"https:\/\/pubmed.ncbi.nlm.nih.gov\/16866740\/\" target=\"_blank\" rel=\"noreferrer noopener\">interacted with a gun<\/a>, or if his <a href=\"https:\/\/www.sciencedirect.com\/science\/article\/abs\/pii\/S0031938498001474#:~:text=In%20both%20studies%2C%20testosterone%20levels,testosterone%20levels%20across%20the%20day.\" target=\"_blank\" rel=\"noreferrer noopener\">favorite sports team just won the game or not<\/a> (yes, really), among many others [2][3][4].<\/p>\n\n\n\n<h3 class=\"wp-block-heading\" id=\"h-new-parents-illustrate-the-hormonal-differences-between-men-and-women\"><span id=\"new-parents-illustrate-the-hormonal-differences-between-men-and-women\">New parents illustrate the hormonal differences between men and women<\/span><\/h3>\n\n\n\n<p class=\"wp-block-paragraph\">The results of hormonal differences between men and women are especially fascinating after a couple has a baby. Most obviously, the new mother is on a rollercoaster of <a href=\"https:\/\/www.copperstateobgyn.com\/postpartum-hormones-explained\/\" target=\"_blank\" rel=\"noreferrer noopener\">postpartum hormone changes<\/a> as her progesterone, in particular, plummets after birth. She also experiences a surge in oxytocin when she\u2019s cuddling or breastfeeding her newborn. Oxytocin release makes women <a href=\"https:\/\/www.theguardian.com\/science\/2015\/apr\/15\/mothers-more-sensitive-to-crying-babies-thanks-to-hormone-study-says\" target=\"_blank\" rel=\"noreferrer noopener\">especially perceptive<\/a> to the sound of their babies crying. This is why typically the mother will be the first one to wake up in the middle of the night when her newborn cries out.&nbsp;<\/p>\n\n\n\n<p class=\"wp-block-paragraph\">All these changes in the mom are known and unsurprising. But what about new dads? Research increasingly shows that they too undergo significant hormonal changes. Just as women\u2019s estrogen and progesterone levels drop off after birth, fathers\u2019 testosterone drops too. And, <a href=\"https:\/\/www.researchgate.net\/publication\/51631751_Longitudinal_Evidence_that_Fatherhood_Decreases_Testosterone_in_Human_Males\" target=\"_blank\" rel=\"noreferrer noopener\">research<\/a> suggests, as summarized <a href=\"https:\/\/www.happiestbaby.com\/blogs\/parents\/fatherhood-mens-bodies#:~:text=A%202011%20study%20found%20that,chores%20and%20baby%2Drelated%20duties.\" target=\"_blank\" rel=\"noreferrer noopener\">here<\/a>, \u201ctestosterone levels in new dads drop below the average levels of non-dads\u201d [5]. Fascinatingly, \u201cthe more a new dad\u2019s testosterone drops, the greater his involvement is in household chores and baby-related duties.\u201d Oxytocin <a href=\"https:\/\/www.fatherly.com\/health\/dad-psychology-science-fatherhood\" target=\"_blank\" rel=\"noreferrer noopener\">surges<\/a> in new dads, too and this appears to make them more home-focused and family-oriented. \u00a0 \u00a0\u00a0\u00a0<\/p>\n\n\n\n<h2 class=\"wp-block-heading\" id=\"h-gender-specific-medicine-also-seeks-to-shed-light-on-how-psychological-differences-between-males-and-females-are-already-present-in-early-childhood-which-may-indicate-that-these-differences-are-inborn\"><span id=\"gender-specific-medicine-also-seeks-to-shed-light-on-how-psychological-differences-between-males-and-females-are-already-present-in-early-childhood-which-may-indicate-that-these-differences-ar\">Gender-specific medicine also seeks to shed light on how psychological differences between males and females are already present in early childhood\u2013which may indicate that these differences are inborn<\/span><\/h2>\n\n\n\n<p class=\"wp-block-paragraph\">Have you ever heard it said that there is no such thing as \u201cgirl\u2019s toys\u201d and \u201cboy\u2019s toys\u201d? While it might be true that both boys and girls can enjoy playing with a wide variety of different toys, there\u2019s actually a reason that we typically think of a babydoll as a \u201cgirl\u201d toy and matchbox cars as a \u201cboy\u201d toy. There have been <a href=\"https:\/\/www.ncbi.nlm.nih.gov\/pmc\/articles\/PMC7031194\/\">many<\/a> <a href=\"https:\/\/journals.sagepub.com\/doi\/10.1080\/01650250042000078\" target=\"_blank\" rel=\"noreferrer noopener\">experiments<\/a> <a href=\"https:\/\/www.sciencedirect.com\/science\/article\/abs\/pii\/S0163638300000321?via%3Dihub\">done<\/a> on whether or not it is a learned or innate trait for boys to prefer one type of toy and girls another [6]. Research consistently demonstrates <a href=\"https:\/\/journals.sagepub.com\/doi\/10.1080\/01650250042000078\" target=\"_blank\" rel=\"noreferrer noopener\">\u201csignificant preferences for gender stereotyped toys\u201d<\/a> by around 18 months of age [6].&nbsp;<\/p>\n\n\n\n<p class=\"wp-block-paragraph\">This isn\u2019t just due to parents pushing doll houses onto their daughters and monster trucks onto their sons, either. Even young <em>monkeys<\/em> <a href=\"https:\/\/www.ncbi.nlm.nih.gov\/pmc\/articles\/PMC2755553\/\" target=\"_blank\" rel=\"noreferrer noopener\">display<\/a> a strong preference for wheeled toys if they\u2019re male and plush toys if they\u2019re female, pointing to inborn, psychological differences between the sexes [7].<\/p>\n\n\n\n<h3 class=\"wp-block-heading\" id=\"h-the-differences-between-male-brains-and-female-brains-have-important-implications-for-psychiatric-and-neurological-diseases-nbsp\"><span id=\"the-differences-between-male-brains-and-female-brains-have-important-implications-for-psychiatric-and-neurological-diseases\">The differences between male brains and female brains have important implications for psychiatric and neurological diseases&nbsp;<\/span><\/h3>\n\n\n\n<p class=\"wp-block-paragraph\">Why does it matter if males and females exhibit differing behavioral preferences? <em>Because these differences may be the outward expression of internal structural differences between male and female brains.<\/em>&nbsp;<\/p>\n\n\n\n<p class=\"wp-block-paragraph\">A recent Stanford University <a href=\"https:\/\/www.pnas.org\/doi\/10.1073\/pnas.2310012121\" target=\"_blank\" rel=\"noreferrer noopener\">study<\/a> used Artificial Intelligence (AI) to determine whether the functional organization of human brains differ consistently between men and women. \u201cSex is an important biological factor that influences human behavior, impacting brain function and the manifestation of psychiatric and neurological disorders,\u201d the researchers wrote. \u201cHowever, previous research on how brain organization differs between males and females has been inconclusive\u201d [8].&nbsp;<\/p>\n\n\n\n<p class=\"wp-block-paragraph\">After combing through large multi-cohort fMRI (functional MRI) datasets, the researcher\u2019s \u201cdeep learning\u201d AI tool was able to tell with almost perfect accuracy if a brain was male or female, just by analyzing how each brain was organized and structured. The researchers concluded: \u201cOur findings underscore the crucial role of sex as a biological determinant in human brain organization, [and] have significant implications for developing personalized sex-specific biomarkers in psychiatric and neurological disorders.\u201d<\/p>\n\n\n\n<h2 class=\"wp-block-heading\" id=\"h-gender-based-medicine-examines-why-women-s-heart-attack-symptoms-differ-from-men-s-nbsp-nbsp\"><span id=\"gender-based-medicine-examines-why-womens-heart-attack-symptoms-differ-from-mens\">Gender-based medicine examines why women\u2019s heart attack symptoms differ from men\u2019s&nbsp;&nbsp;<\/span><\/h2>\n\n\n\n<p class=\"wp-block-paragraph\">But our sex-based differences don\u2019t simply reside in our brains! Gender-specific medicine is important for treating the rest of the body, too\u2013and not just those parts that fall under the \u201cbikini view,\u201d as Dr. Marianne Legato, founder of <a href=\"https:\/\/gendermed.org\/\" target=\"_blank\" rel=\"noreferrer noopener\">The Foundation for Gender Specific Medicine<\/a> and pioneer of gender-specific medicine, puts it [9].&nbsp;<\/p>\n\n\n\n<p class=\"wp-block-paragraph\">For example, one important area of gender-specific medicine that researchers are finally beginning to examine is also the deadliest killer of Americans: heart disease. Although heart disease is often more closely associated with men, it\u2019s actually the <a href=\"https:\/\/www.cedars-sinai.org\/newsroom\/time-women-die-from-heart-attacks-more-often-than-men-heres-why--and-what-doctors-are-doing-about-it\/\" target=\"_blank\" rel=\"noreferrer noopener\">number one killer<\/a> of both men and women. <a href=\"https:\/\/www.columbiadoctors.org\/news\/heart-disease-women-not-heart-disease-men#:~:text=More%20women%20than%20men%20die,their%20conditions%20are%20often%20misdiagnosed.&amp;text=%E2%80%9CHeart%20disease%20is%20the%20No,College%20of%20Physicians%20and%20Surgeons.\" target=\"_blank\" rel=\"noreferrer noopener\">In fact, more women<\/a> die of the disease than men every year.&nbsp;<\/p>\n\n\n\n<p class=\"wp-block-paragraph\">Heart disease also presents differently in men versus women, largely due to the <a href=\"https:\/\/gendermed.org\/about-gsm\/just-the-facts\/\" target=\"_blank\" rel=\"noreferrer noopener\">fact<\/a> that male hearts are larger, while female hearts beat faster and have narrower blood vessels. Men are also prone to plaque buildup in their largest arteries, while women tend towards buildup in the smallest blood vessels. This is why <a href=\"https:\/\/give.brighamandwomens.org\/7-differences-between-men-and-women\/#:~:text=Men%20typically%20develop%20this%20plaque,to%20the%20accumulation%20of%20cholesterol.\" target=\"_blank\" rel=\"noreferrer noopener\">heart attacks do not look the same<\/a> for men and women \u2014 men get the classic chest pain, whereas women might experience chest pain but will also probably have nausea, sweating, vomiting, and pain in their neck, jaw, throat, abdomen, or back.&nbsp;<\/p>\n\n\n\n<h3 class=\"wp-block-heading\" id=\"h-female-hormones-impact-the-cardiovascular-system-too\"><span id=\"female-hormones-impact-the-cardiovascular-system-too\">Female hormones impact the cardiovascular system too<\/span><\/h3>\n\n\n\n<p class=\"wp-block-paragraph\">In fact, after menopause, women\u2019s risk for cardiovascular disease <a href=\"https:\/\/gendermed.org\/about-gsm\/just-the-facts\/\" target=\"_blank\" rel=\"noreferrer noopener\">increases fourfold<\/a> from their premenopausal risk. Men tend to <a href=\"https:\/\/www.ncbi.nlm.nih.gov\/pmc\/articles\/PMC3018605\/\" target=\"_blank\" rel=\"noreferrer noopener\">develop<\/a> cardiovascular disease a full decade before women and this may be because (until menopause) women\u2019s reproductive hormones protect them from heart disease [10].&nbsp;<\/p>\n\n\n\n<p class=\"wp-block-paragraph\">Concerningly, women who have <a href=\"https:\/\/www.ncbi.nlm.nih.gov\/pmc\/articles\/PMC10000901\/#:~:text=Endometriosis%2C%20a%20prevalent%20condition%20among,chronic%20inflammation%20and%20early%20menopause.\" target=\"_blank\" rel=\"noreferrer noopener\">endometriosis<\/a> don\u2019t appear to enjoy the heart disease protection that women without endometriosis do [11]. Premenopausal women with endometriosis are at <a href=\"https:\/\/www.ahajournals.org\/doi\/full\/10.1161\/CIRCOUTCOMES.115.002224\" target=\"_blank\" rel=\"noreferrer noopener\">increased risk<\/a> of heart disease like menopausal women who don\u2019t have endo [12] .<\/p>\n\n\n\n<h2 class=\"wp-block-heading\" id=\"h-female-and-male-immune-systems-differ-with-sex-specific-differences-in-infection-and-autoimmunity-nbsp\"><span id=\"female-and-male-immune-systems-differ-with-sex-specific-differences-in-infection-and-autoimmunity\">Female and male immune systems differ, with sex-specific differences in infection and autoimmunity&nbsp;<\/span><\/h2>\n\n\n\n<p class=\"wp-block-paragraph\">Because women\u2019s bodies must accommodate foreign organisms (that is, growing babies!), it should come as no surprise that women\u2019s immune systems also differ from men\u2019s, which can have sex-specfic implications for a host of immunity-related health concerns..&nbsp;<\/p>\n\n\n\n<p class=\"wp-block-paragraph\">Interestingly, women have stronger immune responses to most kinds of infections than men do, meaning that <a href=\"https:\/\/journals.plos.org\/plospathogens\/article?id=10.1371\/journal.ppat.1005374\" target=\"_blank\" rel=\"noreferrer noopener\">men are more susceptible<\/a> to parasitic or bacterial infections [13]. Higher levels of testosterone also <a href=\"https:\/\/gendermed.org\/about-gsm\/just-the-facts\/\" target=\"_blank\" rel=\"noreferrer noopener\">promote parasite breeding<\/a>, making fighting off these kinds of infections particularly challenging for men.&nbsp;<\/p>\n\n\n\n<h3 class=\"wp-block-heading\" id=\"h-why-women-are-more-likely-to-experience-autoimmune-disease\"><span id=\"why-women-are-more-likely-to-experience-autoimmune-disease\">Why women are more likely to experience autoimmune disease<\/span><\/h3>\n\n\n\n<p class=\"wp-block-paragraph\">On the flip side, women are more likely to develop autoimmune disorders, with as many as <a href=\"https:\/\/med.stanford.edu\/news\/all-news\/2024\/02\/women-autoimmune.html\" target=\"_blank\" rel=\"noreferrer noopener\">4 out of 5 autoimmune sufferers being female<\/a>. Autoimmune disorders include Hashimoto\u2019s, Rheumatoid arthritis, multiple sclerosis (MS), lupus, and many more.&nbsp;<\/p>\n\n\n\n<p class=\"wp-block-paragraph\">The reason for this comes down in part to our <a href=\"https:\/\/med.stanford.edu\/news\/all-news\/2024\/02\/women-autoimmune.html\" target=\"_blank\" rel=\"noreferrer noopener\">very genes<\/a>. Men have XY chromosomes while women have XX. Researchers have connected a genetic process in females called <a href=\"https:\/\/www.ncbi.nlm.nih.gov\/pmc\/articles\/PMC4329817\/#:~:text=Autoimmune%20diseases%20occur%20more%20often,contribute%20to%20the%20disease%20process.\" target=\"_blank\" rel=\"noreferrer noopener\">X-chromosome inactivation<\/a> with <a href=\"https:\/\/med.stanford.edu\/news\/all-news\/2024\/02\/women-autoimmune.html\" target=\"_blank\" rel=\"noreferrer noopener\">autoimmunity<\/a>, which is why lupus, thyroid, and other autoimmune problems are so much more common in women (though autoimmunity is still possible in men with their single X chromosome) [14].&nbsp;<\/p>\n\n\n\n<p class=\"wp-block-paragraph\">Having a second X chromosome also gives women a <a href=\"https:\/\/www.ncbi.nlm.nih.gov\/pmc\/articles\/PMC9958616\/\" target=\"_blank\" rel=\"noreferrer noopener\">genetic predisposition<\/a> toward developing inflammatory bowel disease (such as Crohn\u2019s disease or ulcerative colitis) at higher rates [15]. What\u2019s more, females with this disease often report a worse quality of life than male patients.&nbsp;<\/p>\n\n\n\n<h2 class=\"wp-block-heading\" id=\"h-gender-specific-medicine-aims-to-improve-how-doctors-treat-both-men-and-women-nbsp\"><span id=\"gender-specific-medicine-aims-to-improve-how-doctors-treat-both-men-and-women\">Gender-specific medicine aims to improve how doctors treat both men and women&nbsp;<\/span><\/h2>\n\n\n\n<p class=\"wp-block-paragraph\">Medicine still has a <a href=\"https:\/\/www.degruyter.com\/document\/doi\/10.1515\/cclm-2012-0849\/html\" target=\"_blank\" rel=\"noreferrer noopener\">long way to go<\/a> in understanding all of the ways men and women differ in the response to (or development of) various disorders and diseases. But it\u2019s becoming increasingly clear that, when caring for female patients, doctors cannot merely add mammograms for breast cancer prevention and pap smears for cervical infections \u2013 and then proceed to more or less treat them in the same way as their male patients. Heart disease, autoimmunity, cancers, and even <a href=\"https:\/\/journals.sagepub.com\/doi\/10.2217\/17455057.3.6.735?icid=int.sj-full-text.similar-articles.2\" target=\"_blank\" rel=\"noreferrer noopener\">diabetes<\/a> can all look different in men and women, and we still have much to understand about the sex-specific variations in these diseases [16].<\/p>\n\n\n\n<p class=\"wp-block-paragraph\">Although Supreme Court Justice Jackson might be wrong in her assessment that <em>only <\/em>a biologist can distinguish between men and women, medical researchers <em>do <\/em>have a responsibility to further examine the differences between the two sexes beyond the visibly obvious. Men and women are innately different, not just in our hormones and reproductive parts, but in every tissue and cell of our bodies.
