{"id":17720,"date":"2023-07-14T09:55:34","date_gmt":"2023-07-14T14:55:34","guid":{"rendered":"https:\/\/naturalwomanhood.org\/?p=17720"},"modified":"2024-09-20T07:53:20","modified_gmt":"2024-09-20T12:53:20","slug":"reparto-decidual","status":"publish","type":"post","link":"https:\/\/naturalwomanhood.org\/es\/decidual-cast\/","title":{"rendered":"\u00bfQu\u00e9 es una escayola decidual y qu\u00e9 significa tener una?"},"content":{"rendered":"\n<p>Even though they are an extremely rare medical event, decidual casts made a splash on TikTok recently after <a href=\"https:\/\/www.tiktok.com\/@sweglestory\/video\/7169334367037213995?embed_source=121331973%2C120811592%2C120810756%3Bnull%3Bembed_name&amp;refer=embed&amp;referer_url=iframe.nbcnews.com%2F9BCTK7I%3F_showcaption%3Dtrue%26app%3D1&amp;referer_video_id=7169334367037213995\" target=\"_blank\" rel=\"noreferrer noopener\">Madi Swegle<\/a> shared her story (also covered by <a href=\"https:\/\/www.today.com\/health\/womens-health\/decidual-cast-rcna65284\" target=\"_blank\" rel=\"noreferrer noopener\">Today<\/a>) of passing a decidual cast. Swegle is a Labor and Delivery nurse, but she had never heard of a decidual cast before. This is no surprise. As <a href=\"https:\/\/www.thegms.co\/publichealth\/pubheal-ra-22041401.pdf\" target=\"_blank\" rel=\"noreferrer noopener\">this analysis<\/a> noted, decidual casts are so rare that up until 2021 \u201cless than 40 cases [were] reported in the medical literature over the last 109 years.\u201d [1]\u00a0\u00a0<\/p>\n\n\n\n<p>Swegle said the worst part of her painful and frightening experience was the anxiety of not knowing what was happening. Again, passing a decidual cast is a rare medical event. But here\u2019s the rundown of what a decidual cast is and what to do if you experience one&#8230; just in case!<\/p>\n\n\n\n<h2 id=\"what-is-a-decidual-cast\" class=\"wp-block-heading\">What is a decidual cast?<\/h2>\n\n\n\n<p>First, let\u2019s break down the phrase \u201cdecidual cast,\u201d also referred to as \u201cdecidual cast shedding.\u201d The decidua is the functional layer of the <a href=\"https:\/\/naturalwomanhood.org\/fam-basics-everything-you-need-to-know-about-your-endometrium\/\" target=\"_blank\" rel=\"noreferrer noopener\">endometrium<\/a>, which is the lining of the uterus or womb. The decidua is the layer in which a blastocyst implants if conception occurs.\u00a0<\/p>\n\n\n\n<p>After ovulation, the <a href=\"https:\/\/naturalwomanhood.org\/fam-basics-what-is-the-corpus-luteum\/\" target=\"_blank\" rel=\"noreferrer noopener\">corpus luteum<\/a> produces <a href=\"https:\/\/naturalwomanhood.org\/fam-basics-progesterone\/\" target=\"_blank\" rel=\"noreferrer noopener\">progesterone<\/a>. This signals the cells in the endometrium to change in order to prepare to receive a newly conceived blastocyst and to make the placenta. This process is called decidualization. So, we can think of the decidua as the layer of the endometrium that is ready for pregnancy or already part of a pregnant womb.&nbsp;<\/p>\n\n\n\n<p>In art, a&nbsp; \u201ccast\u201d is a mold made by an artist as part of a sculpture or model. So, a decidual cast is a mold of your uterus made out of the decidua.&nbsp;<\/p>\n\n\n\n<h3 class=\"wp-block-heading\" id=\"h-this-is-what-s-supposed-to-happen-in-the-uterus-each-cycle\"><span id=\"this-is-whats-supposed-to-happen-in-the-uterus-each-cycle\">This is what\u2019s supposed to happen in the uterus each cycle<\/span><\/h3>\n\n\n\n<p>Normally, a new menstrual cycle starts when the old decidua breaks up and dissolves. The decidua lining is shed over the course of a few days. This is the normal period experience most women are used to.<\/p>\n\n\n\n<h3 class=\"wp-block-heading\" id=\"h-this-is-what-happens-when-you-have-a-decidual-cast\"><span id=\"this-is-what-happens-when-you-have-a-decidual-cast\">This is what happens when you have a decidual cast<\/span><\/h3>\n\n\n\n<p>In the case of a decidual cast, the decidua doesn\u2019t break up at all. Instead, it\u2019s expelled as a single piece of tissue roughly the size of the inside of your uterus. This means it\u2019s about two inches long and about an inch thick. <a href=\"https:\/\/my.clevelandclinic.org\/health\/diseases\/24742-decidual-cast#:~:text=Decidual%20casts%20are%20rare%20and,usually%20due%20to%20progesterone%20contraceptives.\" target=\"_blank\" rel=\"noreferrer noopener\">Cleveland Clinic<\/a> says patients who have experienced decidual casts report them to range in size from a walnut to a small lime. A cast may be triangular in shape and look like a tiny replica of the uterus. This is understandably a rather alarming object to pass vaginally!&nbsp;<\/p>\n\n\n\n<h3 class=\"wp-block-heading\" id=\"h-decidual-casts-are-not-the-same-as-blood-clots\"><span id=\"decidual-casts-are-not-the-same-as-blood-clots\">Decidual casts are not the same as blood clots<\/span><\/h3>\n\n\n\n<p>Endometrial tissue will feel firm and solid and will likely be a reddish color. Cast tissue is not to be confused with a blood clot, which may look more blackish\/red and have a jelly-like consistency.<\/p>\n\n\n\n<h3 class=\"wp-block-heading\" id=\"h-pain-is-part-of-most-women-s-experience\"><span id=\"pain-is-part-of-most-womens-experience\">Pain is part of most women\u2019s experience<\/span><\/h3>\n\n\n\n<p>Passing a decidual cast is also quite painful. Normally, the cervix only needs to open (dilate) a small amount to let the blood, mucus, and dissolved tissue of a typical period to pass through. But in the case of a large piece of tissue like a decidual cast, the uterus has to work hard to dilate the cervix enough to pass the tissue [2]. Thus, the experience is more like labor cramps rather than normal period cramps&#8230; except without the benefit of having time to prepare, as you typically would before giving birth. Once the cast is passed, symptoms and pain will cease. Passing a decidual cast is similar to having all the aspects of a period condensed into a short amount of time rather than spread out over the course of several days.<\/p>\n\n\n\n<h2 id=\"what-causes-decidual-casts\" class=\"wp-block-heading\">What causes decidual casts?<\/h2>\n\n\n\n<h3 class=\"wp-block-heading\" id=\"h-causes-in-a-pregnant-woman\"><span id=\"causes-in-a-pregnant-woman\">Causes in a pregnant woman<\/span><\/h3>\n\n\n\n<p>Decidual casts are typically associated with ectopic pregnancies, even though most women who experience an ectopic pregnancy will <em>not <\/em>experience decidual casts. However, in some cases a decidual cast <em>reveals<\/em> an ectopic pregnancy. If a woman has had a positive pregnancy test, and passes a mass of tissue that <em>does not<\/em> contain the expected products of conception, like the gestational sac, her embryo is likely developing <em>outside<\/em> of the uterus. Despite having passed the decidual cast, the woman will still need to <a href=\"https:\/\/naturalwomanhood.org\/how-do-you-treat-ectopic-pregnancy\/\" target=\"_blank\" rel=\"noreferrer noopener\">receive treatment<\/a> for her ectopic pregnancy in order to save her life. This is why it is always important to have a medical evaluation after unusual gynecological symptoms like severe pelvic pain or passage of mysterious tissue.<\/p>\n\n\n\n<p>I was only able to find one known <a href=\"https:\/\/pubmed.ncbi.nlm.nih.gov\/33722911\/\" target=\"_blank\" rel=\"noreferrer noopener\">case<\/a> of someone who experienced a decidual cast that was also a miscarriage. The miscarriage was classified as a complication of the patient\u2019s <a href=\"https:\/\/naturalwomanhood.org\/risk-of-hiv-transmission-depo-provera-birth-control-shot\/\" target=\"_blank\" rel=\"noreferrer noopener\">Depo Provera <\/a>injection. If you have not used progestin-based contraceptives and experience a decidual cast, you could have an ectopic pregnancy.<\/p>\n\n\n\n<h3 class=\"wp-block-heading\" id=\"h-causes-in-a-non-pregnant-woman-are-more-common-with-progestin-only-birth-control-users\"><span id=\"causes-in-a-non-pregnant-woman-are-more-common-with-progestin-only-birth-control-users\">Causes in a non-pregnant woman are more common with progestin-only birth control users<\/span><\/h3>\n\n\n\n<p>Most documented cases of decidual casts in women who <em>weren\u2019t <\/em>pregnant involved progestin-containing <a href=\"https:\/\/pubmed.ncbi.nlm.nih.gov\/25353701\/\" target=\"_blank\" rel=\"noreferrer noopener\">contraception<\/a> [3] . Some users were on the <a href=\"https:\/\/ispub.com\/IJGO\/9\/1\/11420\" target=\"_blank\" rel=\"noreferrer noopener\">Pill<\/a>, which contains both synthetic progesterone (progestin) and estrogen [4]. But most took progestin-only contraception like <a href=\"https:\/\/pubmed.ncbi.nlm.nih.gov\/33722911\/\" target=\"_blank\" rel=\"noreferrer noopener\">Depo Provera<\/a> or the <a href=\"https:\/\/www.aafp.org\/pubs\/afp\/issues\/2018\/1001\/p449.html#afp20181001p449-b3\" target=\"_blank\" rel=\"noreferrer noopener\">Nexplanon<\/a> implant [5][6]. These case studies included patients who had recently ceased using hormonal contraception. In Madi Swegle\u2019s case, she took hormonal birth control before she started in vitro fertilization (IVF) treatment (more on birth control and infertility <a href=\"https:\/\/naturalwomanhood.org\/pill-cause-infertility-yes-no\/\" target=\"_blank\" rel=\"noreferrer noopener\">here<\/a>).\u00a0(Note that the recently approved over-the-counter <a href=\"https:\/\/naturalwomanhood.org\/over-the-counter-birth-control\/\" target=\"_blank\" rel=\"noreferrer noopener\">Opill<\/a> birth control pill is also progestin-only.)<\/p>\n\n\n\n<p>The surge in reported cases of decidual casts during 2021 prompted some researchers to question whether there was a connection to COVID-19 vaccines. A <a href=\"https:\/\/www.semanticscholar.org\/paper\/COVID-19-and-the-surge-in-Decidual-Cast-Shedding-Parotto-Thorp\/2a6a540116aa004dfe082af4d013ba299d02e01d\" target=\"_blank\" rel=\"noreferrer noopener\">survey<\/a> of women who experienced menstrual cycle irregularities during the pandemic and in particular after receiving a dose or doses of COVID-19 vaccine found that only 11% of women who self-reported a decidual cast were on hormonal contraception at the time [1].\u00a0<\/p>\n\n\n\n<p>Since a decidual cast is more commonly associated with ectopic pregnancies, it is important to contact an OB\/GYN and take a pregnancy test to rule out this dangerous possibility, should you experience one. This is especially true since <a href=\"https:\/\/naturalwomanhood.org\/contraception-birth-control-iud-fabm-effectiveness-failure-2020\/\" target=\"_blank\" rel=\"noreferrer noopener\">unexpected pregnancies<\/a> are a known risk of hormonal contraception, and certain forms of contraception may increase your risk of <a href=\"https:\/\/naturalwomanhood.org\/topic\/birth-control-side-effects\/\" target=\"_blank\" rel=\"noreferrer noopener\">ectopic pregnancy<\/a>.\u00a0<\/p>\n\n\n\n<h2 id=\"is-a-decidual-cast-bad\" class=\"wp-block-heading\">Is a decidual cast bad?<\/h2>\n\n\n\n<p>A decidual cast itself is not bad, although it is often quite painful. But it does indicate that something else is wrong. If the cast is caused by birth control, the pain and symptoms will stop as soon as the cast is past. But this signals that the progestin in birth control is just not the same as the progesterone your body makes. On birth control, the endometrium isn\u2019t being formed, matured, and shed in the way it would be if a woman was only exposed to the natural progesterone and estrogen of her cycle.&nbsp;<\/p>\n\n\n\n<p>Luckily, there are side effect-free ways to prevent and space pregnancies through <a href=\"https:\/\/naturalwomanhood.org\/topic\/fertility-awareness-methods\/what-is-fam\/\" target=\"_blank\" rel=\"noreferrer noopener\">fertility awareness<\/a>! With fertility awareness you can learn about your fertility and work with your body to support your whole-body health. Fertility awareness allows your hormones to fulfill the important functions for which they\u2019re intended within <a href=\"https:\/\/naturalwomanhood.org\/topic\/fertility-awareness-methods\/reasons-women-need-periods\/\" target=\"_blank\" rel=\"noreferrer noopener\">multiple organ systems<\/a>, not just within your endometrium.<\/p>\n\n\n\n<p>When decidual casts occur <em>during<\/em> pregnancy (and are therefore unrelated to synthetic hormone use), the pain, bleeding, and tissue passed may be mistaken for a miscarriage. If the tissue is confirmed to be only endometrial tissue, and climbing human chorionic gonadotropin (hCG) levels continue to indicate that the woman is pregnant, not miscarrying, that pregnancy is necessarily outside of the womb. This is an ectopic pregnancy. In this case, the decidual cast itself is not bad or dangerous. It is an indication of a dangerous condition that requires immediate medical attention.<\/p>\n\n\n\n<h2 class=\"wp-block-heading\" id=\"h-the-bottom-line\"><span id=\"the-bottom-line\">The bottom line<\/span><\/h2>\n\n\n\n<p>Despite the internet\u2019s recent interest in decidual casts, the likelihood that you\u2019ll need to be on the lookout for one is low. But as we&#8217;ve discussed above, certain things may potentially increase your risk, such as using progestin-only birth control. Regardless, open discussions about women\u2019s health and the downsides of synthetic hormones are <em>always <\/em>worth having, especially as women are often not fully informed of the risks and side effects that come with hormonal birth control use.
