Normalmente, si tienes la regla y no tomas ning\u00fan tipo de anticonceptivo hormonal, est\u00e1s ovulando. La mayor\u00eda de las veces. Pero a veces, una mujer puede experimentar lo que se denomina un ciclo anovulatorio. Esto significa que tienes un sangrado que podr\u00edas pensar que es la regla, pero como la ovulaci\u00f3n no se ha producido, no es una menstruaci\u00f3n verdadera. <\/p>\n\n\n\n<p>\u00bfPor qu\u00e9 se producen los ciclos anovulatorios y c\u00f3mo podemos detectarlos? \u00bfSon problem\u00e1ticos y requieren atenci\u00f3n m\u00e9dica? <\/p>\n\n\n\n<h2 class=\"wp-block-heading\" id=\"h-the-difference-between-menstruation-and-breakthrough-bleeding\"><span id=\"the-difference-between-menstruation-and-breakthrough-bleeding\">Diferencia entre menstruaci\u00f3n y hemorragia intermenstrual<\/span><\/h2>\n\n\n\n<p>La verdadera menstruaci\u00f3n siempre sigue a la ovulaci\u00f3n, normalmente unas dos semanas despu\u00e9s. Una hemorragia que se produce sin que haya ovulaci\u00f3n previa se denomina hemorragia intermenstrual o, m\u00e1s t\u00e9cnicamente, hemorragia uterina anormal de tipo anovulatorio. El endometrio proliferativo supera su suministro de sangre y comienza a desprenderse, lo que provoca la hemorragia. (N\u00f3tese que esto es diferente de la idea de \"hemorragia por deprivaci\u00f3n\", que es secundaria a la deprivaci\u00f3n de progesterona). <\/p>\n\n\n\n<p>Desglosemos los distintos tipos de condiciones en las que las mujeres pueden experimentar anovulaci\u00f3n explorando las situaciones de algunos escenarios diferentes.<\/p>\n\n\n\n<h2 class=\"wp-block-heading\" id=\"h-anovulation-due-to-stress\"><span id=\"anovulation-due-to-stress\">Anovulaci\u00f3n debida al estr\u00e9s<\/span><\/h2>\n\n\n\n<p>Imaginemos a una joven llamada Annie que no toma ning\u00fan tipo de anticonceptivo hormonal, cuyos ciclos son en su mayor\u00eda regulares, y que es estudiante universitaria. Un mes, justo antes de los ex\u00e1menes y mientras est\u00e1 buscando una nueva compa\u00f1era de piso, tiene lo que ella cree que es la regla. El sangrado es mucho m\u00e1s ligero de lo habitual. Lo que Annie no sabe es que en realidad se trata de un sangrado intermenstrual. Con todo el estr\u00e9s de terminar los proyectos de fin de semestre y encontrar una nueva compa\u00f1era de piso, su cuerpo se ha \"saltado\" subrepticiamente la ovulaci\u00f3n este mes.&nbsp;<\/p>\n\n\n\n<p>\u00bfQu\u00e9 le pasa a Annie hormonalmente? El estr\u00e9s que experiment\u00f3 en realidad \"envi\u00f3\" una se\u00f1al a sus ovarios para detener la producci\u00f3n de fol\u00edculos este mes. En un ciclo normal, no s\u00f3lo se recluta un fol\u00edculo -en realidad hay varios \"esperando entre bastidores\"- y finalmente se elige un fol\u00edculo dominante para liberar un \u00f3vulo. Pero cuando no se elige un fol\u00edculo dominante debido a una situaci\u00f3n de estr\u00e9s agudo, los fol\u00edculos aspirantes pueden esperar pacientemente, liberando peque\u00f1as cantidades de estr\u00f3geno. <\/p>\n\n\n\n<p>En respuesta a los niveles continuamente bajos de estr\u00f3genos, el endometrio (el revestimiento del \u00fatero) se engrosa, pero s\u00f3lo ligeramente. Un par de semanas m\u00e1s tarde, y a pesar de no haberse producido la ovulaci\u00f3n, el endometrio se desprende. Esto es en parte una respuesta inmunitaria, y tambi\u00e9n puede estar relacionado con un flujo sangu\u00edneo inadecuado al endometrio folicular engrosado, para evitar una acumulaci\u00f3n endometrial innecesaria sin perspectivas de que un \u00f3vulo fecundado llegue a excavar en \u00e9l en breve. Y como los niveles de estr\u00f3genos eran bajos, la hemorragia resultante es leve (aunque, si la amenorrea se prolongara lo suficiente, las hemorragias podr\u00edan empezar a ser abundantes).&nbsp;<\/p>\n\n\n\n<p>El ciclo anovulatorio de Annie no es preocupante, sino m\u00e1s bien una respuesta aguda a una situaci\u00f3n aguda. Es probable que sus ciclos regulares vuelvan una vez que terminen los ex\u00e1menes y encuentre una nueva situaci\u00f3n de vida; en otras palabras, una vez que su temporada a corto plazo de <a href=\"https:\/\/naturalwomanhood.org\/es\/como-afecta-el-estres-al-ciclo-menstrual-conocimiento-de-la-fertilidad-fabm-nfp-05052018\/\">alto estr\u00e9s<\/a> ha terminado. (Otras situaciones que pueden someter al organismo a estr\u00e9s e inducir anovulaci\u00f3n y\/o amenorrea son el ejercicio excesivo o cuando el peso corporal es demasiado bajo, como en la anorexia o la bulimia).<\/p>\n\n\n\n<h2 class=\"wp-block-heading\" id=\"h-anovulation-from-perimenopause-and-other-transitional-seasons\"><span id=\"anovulation-from-perimenopause-and-other-transitional-seasons\">Anovulaci\u00f3n de la perimenopausia y otras estaciones de transici\u00f3n<\/span><\/h2>\n\n\n\n<p>Perm\u00edtanme presentarles a Nina. Nina es madre de tres hijos y, aunque la mayor\u00eda de sus ciclos son regulares, desde el a\u00f1o pasado parecen estar cada vez m\u00e1s espaciados. Tiene unos cuarenta a\u00f1os y jura que es demasiado joven para la menopausia. <\/p>\n\n\n\n<p>Aunque los cuarenta y pocos son ciertamente j\u00f3venes para la menopausia, es <a href=\"https:\/\/naturalwomanhood.org\/es\/graficos-perimenopausia-fertilidad-conocimiento-metodos-signos-confusos-ciclo-cambios-hormonales\/\"><em>no<\/em> demasiado joven para la perimenopausia<\/a>que puede ser una d\u00e9cada entera antes del cese real de los ciclos, que es la menopausia. Nina sospecha que ya no ovula, pero sus menstruaciones son igual de abundantes que a\u00f1os atr\u00e1s y, a veces, incluso m\u00e1s.&nbsp;<\/p>\n\n\n\n<p>Hormonalmente, Nina no est\u00e1 ovulando debido a los niveles fluctuantes de hormonas. Sus niveles de estr\u00f3geno todav\u00eda pueden ser relativamente altos, pero no altos. <em>suficiente <\/em>para liberar un \u00f3vulo. (A esta edad, tambi\u00e9n es importante tener en cuenta que Nina se est\u00e1 quedando sin \u00f3vulos, ya que las mujeres nacen con todos los \u00f3vulos que tendr\u00e1n jam\u00e1s).<\/p>\n\n\n\n<p>Aunque la perimenopausia es una parte totalmente normal del espectro reproductivo, es posible que Nina desee trabajar con un profesional para tratar los s\u00edntomas que a menudo surgen de los niveles altos y persistentes de estr\u00f3geno, como el insomnio, los sofocos, los cambios en el metabolismo y los dolores de cabeza.<\/p>\n\n\n\n<p>Otras estaciones de transici\u00f3n durante las cuales las mujeres pueden experimentar ciclos anovulatorios de forma natural son <a href=\"https:\/\/naturalwomanhood.org\/es\/fabm-posparto-pnf-la-lactancia-materna-previene-el-embarazo\/\">lactancia materna<\/a>temprano <a href=\"https:\/\/naturalwomanhood.org\/es\/ciclo-mindfulness-que-ocurre-cuando-se-ensena-a-las-adolescentes-a-ser-conscientes-de-la-fertilidad\/\">pubertad<\/a>y cuando <a href=\"https:\/\/naturalwomanhood.org\/es\/dejar-la-pildora-4-claves-para-una-transicion-saludable\/\">dejar los anticonceptivos hormonales<\/a>.<\/p>\n\n\n\n<h2 class=\"wp-block-heading\" id=\"h-anovulation-on-the-birth-control-pill\"><span id=\"anovulation-on-the-birth-control-pill\">Anovulaci\u00f3n con la p\u00edldora anticonceptiva<\/span><\/h2>\n\n\n\n<p>Veamos ahora el caso de Camilla. Camilla tiene treinta y pocos a\u00f1os y toma la p\u00edldora combinada, que incluye estr\u00f3genos artificiales y progesterona artificial, conocida como progestina. Recibe una peque\u00f1a dosis diaria de estas dos hormonas sint\u00e9ticas cuando toma la p\u00edldora cada ma\u00f1ana. Pero los niveles son muy inferiores a los que su cuerpo producir\u00eda de forma natural (<a href=\"https:\/\/naturalwomanhood.org\/es\/diferencia-entre-hormonas-sinteticas-y-naturales-anticonceptivos-bioidenticos-efectos-nocivos-y-seguros-progesterona-progestina-estrogeno-08192018\/\">y las hormonas en s\u00ed no son bio-id\u00e9nticas<\/a> tampoco). <\/p>\n\n\n\n<p>Cada tres semanas, toma una \"p\u00edldora placebo\", que es b\u00e1sicamente una p\u00edldora de az\u00facar que no contiene hormonas. Las hemorragias que experimenta Camilla mientras toma la p\u00edldora placebo son leves, y en realidad no son menstruaciones, ya que Camilla no est\u00e1 ovulando debido a su m\u00e9todo anticonceptivo.&nbsp;<\/p>\n\n\n\n<p>En cierto sentido, <a href=\"https:\/\/naturalwomanhood.org\/es\/control-hormonal-de-la-natalidad-respuesta-al-estres-cronico-ansiedad-eje-apa-2019\/\">Camilla est\u00e1 en un barco similar al de Annie<\/a>-pero la de Camilla es inducida sint\u00e9ticamente. La p\u00edldora combinada que toma env\u00eda una se\u00f1al negativa a su gl\u00e1ndula pituitaria para que detenga la producci\u00f3n end\u00f3gena (natural) de estr\u00f3geno y progesterona, administrando a su cuerpo una peque\u00f1a dosis constante de estr\u00f3geno y progesterona artificiales. El endometrio de Camilla prolifera ligeramente debido a los bajos niveles de estr\u00f3geno sint\u00e9tico y, cuando el cuerpo recibe la se\u00f1al de las pastillas de az\u00facar, elimina el endometrio.&nbsp;<\/p>\n\n\n\n<h2 class=\"wp-block-heading\" id=\"h-anovulation-from-pcos-and-other-hormonal-disorders\"><span id=\"anovulation-from-pcos-and-other-hormonal-disorders\">Anovulaci\u00f3n por SOP y otros trastornos hormonales<\/span><\/h2>\n\n\n\n<p>Por \u00faltimo, tenemos a Tamara. Tamara tiene unos veinte a\u00f1os y siempre ha tenido menstruaciones irregulares, dolorosas y largas. Pasa meses sin sangrar y a veces tiene que quedarse en casa y no ir a trabajar porque los dolores son muy fuertes. Tamara tambi\u00e9n tiene <a href=\"https:\/\/naturalwomanhood.org\/es\/pcos-causa-depresion-sintomas-de-infertilidad-19\/\">s\u00edndrome de ovario poliqu\u00edstico<\/a> (SOP) y una serie de <a href=\"https:\/\/naturalwomanhood.org\/es\/relacion-entre-los-anticonceptivos-y-las-enfermedades-autoinmunes-030619\/\">trastornos autoinmunitarios<\/a>. Algunas hemorragias son leves y otras intolerables.<\/p>\n\n\n\n<p>Tamara tiene una situaci\u00f3n hormonal similar a la de Nina, pero la de Tamara no est\u00e1 relacionada con una etapa del espectro reproductivo. En el caso de Tamara, son sus trastornos endocrinos los que provocan una ovulaci\u00f3n irregular y, por lo tanto, el sangrado irregular que a veces se produce al no ovular en absoluto. Los niveles elevados, persistentes y descontrolados de estr\u00f3genos que no conducen a la ovulaci\u00f3n provocan un gran crecimiento del endometrio, lo que a su vez causa hemorragias abundantes, independientemente de que se haya producido o no la ovulaci\u00f3n.&nbsp;<\/p>\n\n\n\n<p>La situaci\u00f3n de Tamara es preocupante y <a href=\"https:\/\/naturalwomanhood.org\/es\/pcos-diagnostico-sintomas-como-se-diagnostica-infertilidad-gestion-tratamiento-quedarse-embarazada-071818\/\">justifica la consulta con un profesional m\u00e9dico<\/a>. Sin duda le vendr\u00eda bien que un m\u00e9dico examinara sus niveles hormonales e intentara evaluar c\u00f3mo controlar tanto su SOP como sus trastornos autoinmunes. En este caso, los ciclos anovulatorios podr\u00edan estar causando muchos otros s\u00edntomas indeseables para Tamara, como acn\u00e9, aumento de peso, hinchaz\u00f3n y s\u00edndrome premenstrual, y ciertamente no est\u00e1n haciendo que sus sangrados (ovulatorios o no) sean manejables.<\/p>\n\n\n\n<h2 class=\"wp-block-heading\" id=\"h-how-do-you-know-if-you-re-ovulating-distinguishing-an-ovulatory-cycle-from-an-anovulatory-one\"><span id=\"how-do-you-know-if-youre-ovulating-distinguishing-an-ovulatory-cycle-from-an-anovulatory-one\">\u00bfC\u00f3mo saber si est\u00e1s ovulando? Distinguir un ciclo ovulatorio de uno anovulatorio<\/span><\/h2>\n\n\n\n<p>Quiz\u00e1 te preguntes: \"Bueno, \u00bfc\u00f3mo puedo saber si estoy teniendo hemorragias anovulatorias si sigo teniendo una hemorragia cada mes?\". Afortunadamente, hay una respuesta bastante f\u00e1cil: <a href=\"https:\/\/naturalwomanhood.org\/es\/que-es-un-metodo-de-conocimiento-de-la-fertilidad-y-como-empezar-a-trazar-graficos\/\">Todo est\u00e1 en el gr\u00e1fico del ciclo.<\/a> <\/p>\n\n\n\n<p>Si usted es <a href=\"https:\/\/naturalwomanhood.org\/es\/cuatro-formas-de-inducir-la-ovulacion-de-forma-natural-regular-los-ciclos\/\">trazar sus biomarcadores de fertilidad para confirmar la ovulaci\u00f3n<\/a>puede determinar bastante bien si ha tenido una hemorragia menstrual o anovulatoria. \u00c9sta es s\u00f3lo una raz\u00f3n m\u00e1s para amar los m\u00e9todos de conocimiento de la fertilidad (FAM).
